Impact of a passive palatal plate on orofacial functions in children with cleft lip and palate: a systematic review.

Cleft lip and/or palate (CL/P) are common malformations that affect the structure of the face and oral cavity, requiring multidisciplinary care. Newborns with CL/P face functional difficulties related to feeding, speech, breathing, and craniofacial growth. The use of palatal plates (PP) aims to improve these functions. This systematic review assessed the effectiveness of PP in newborns with CL/P. Although existing studies showed varied results, it appears that PP may influence maxillary growth and contribute to the improvement of orofacial functions such as feeding, speech, and language develo

Modified technique of presurgical infant maxillary orthopedics for complete unilateral cleft lip and palate.

This article introduces a modified device fabrication and facial taping method that increases the efficiency and efficacy of presurgical infant maxillary orthopedic therapy for babies born with complete cleft lip and palate. Interarch and intra-arch relationships of the maxillary and mandibular dental arches were evaluated on mounted stone models before and after treatment. The palatal plate device was custom-fabricated in a manner that bypassed the need for periodic acrylic addition and removal, thereby eliminating the risk of natural maxillary growth restriction during therapy. An unusual type of sucking habit in a patient with cleft lip and palate.

Digit sucking, a form of non-nutritive sucking, is a habit of concern to specialist in various fields such as psychiatrist, psychologist, pediatricians, pediatric dentists, orthodontist, speech pathologist and plastic surgeon. The habits have harmful unbalanced pressures to be born by the immature highly malleable alveolar ridges. Sucking behaviors have long been recognized to affect occlusion and dental arch characteristics. As early as 1870s, Campbell and Chandler recognized that prolonged finger or thumb sucking habits had deleterious effects on certain occlusal traits including anterior op

The cleft seal for bottle-feeding--A report on case studies.

Clefting of the lip and palate is the most frequent craniofacial malformation that occurs in newborn babies. The greatest immediate challenge facing a neonate with a cleft is to take nourishment. In developing countries and particularly in rural areas, palatal obturators and other aids that can assist in feeding are not easily available or accessible. The aim of this study was to develop, evaluate and report on the cleft seal, a simple and inexpensive device that is used with a feeding bottle to promote nourishment of a neonate with a cleft palate. The cleft seal is made of either silicone or

Syphilitic rectal ulcer associated with perforation of the hard palate: case report.

We report a case of a patient that presented with a perforated hard palate as a late complication due to an unsuspected syphilis. This disease first presented as a rectal ulcer which was misdiagnosed as an amebic proctitis. The patient received antiamebic treatment with a satisfactory outcome. He did not return for late control of the latter treatment and returned seeking medical advice six years later with the former complication. He tested positive for syphilis and appropriate treatment was performed. In addition, the ORL department recommended a palate prosthesis.

Transverse pharyngeal flaps: a dynamic repair for velopharyngeal insufficiency.

During the past three years, forty-eight patients, ranging from five to thirty years of age, have undergone bilateral transverse pharyngeal pedicles to the palate. Thirty-nine patients had not had pharyngeal flaps prior to this procedure and the remaining nine patients had failure of production of acceptable speech after undergoing inferiorly or superiorly based flap operations. One patient suffered separation of the suture line. Dramatic improvement in speech has been noted in the remaining forty-seven patients. While it is difficult to compare the speech results to other procedures, a signif

Large Pleomorphic Adenoma of Hard Palate.

Pleomorphic adenoma (PA) is a benign tumor of the salivary glands commonly seen in the parotid and submandibular salivary glands. Rarely, it is seen in the minor salivary glands located at lips, palate, and other parts of the upper aerodigestive tract. We report a case of an unusually large PA of the hard palate in a female patient involving left maxillary sinus and nasal cavity. After excising, the tumor with adequate clinical margins (1 cm), a large postsurgical defect was managed by the use of obturator and it gradually healed by secondary intention in about 6-month period. Hard Palate Perforation in an Elderly Man With Dementia.

A 74-year-old male with a history of mild cognitive impairment presented to the emergency department with failure to thrive and generalized weakness. He was having difficulty swallowing leading to 30 pounds of unintentional weight loss in the last three months. His social history was significant for 12.5 pack-year smoking and drinking (two to three glasses of wine/day). The oral cavity examination revealed a large (3 × 2 cm2) defect with the erythematous border that encompassed the mid-palatal structures and emanated from the hard palate into his nasal cavity. Auto-immune work-up was negative.

Utilizing the GentleWave® System for Debridement of Undetected Apical Anatomy.

AIM AND BACKGROUND: Debriding and disinfecting complex anatomies within the root canal system pose a major challenge during root canal therapy. Even with current chemomechanical techniques, debris and bacterial remnants are commonly left behind, which are generally believed to increase the risk of endodontic failure. This case details the use of a new technique to debride complex apical anatomy in a maxillary molar. CASE REPORT: A 48-year-old female presented to the clinic with a chief complaint of increasing pain in her tooth. Clinical examination of the right first maxillary molar (#3) reve

[Functional velopalatal insufficiency. Therapeutic possibilities].

The function of the soft palate is complex; it takes part in speech, swallowing, and masticatory mechanisms. Its pathology ranges from morphological deficits to functional ones; these are commonly due to a central nervous system damage. The clinical analysis is based upon examination and instrumental prove, and leads to an optimum choice between surgical and prosthetic solutions. The latter is the best one in terms of quick execution, low costs and future possibilities of improvement.

A custom made extraoral aid for retaining interim obturator in edentulous patients with bilateral maxillectomy: A report of four patients.

It is a considerable challenge for a prosthodontist to rehabilitate and sustain the prosthesis in edentulous patients with bilateral maxillectomy. Compelling evidence is lacking with respect to the treatment outcome when the maxillary defects are closed surgically in comparison to their prosthetic rehabilitation. Four edentulous male patients, with bilateral maxillectomy defects were referred for prosthodontic intervention to meet their nutritional and speech issues. In the absence of intra oral supporting and retentive structures, it was decided to retain the obturator utilizing extraoral aid

A modified technique to fabricate a complete hollow obturator for bilateral maxillectomy in a patient with mucormycosis-A technical case report.

Maxillectomy performed in acquired lesions is often very extensive or bilateral, sparing very less hard and soft tissue in the oral cavity. These defects need both surgical and prosthetic rehabilitation. A definitive prosthesis is inserted after complete healing, when the patient is accustomed to the interim prosthesis. Definitive obturator warrants better retention and stability and needs a meticulous planning in design for long-term use. Conventional design has a few disadvantages; it needs modification many times to improve retention and psychological comfort of the patient. The article exp

Pharyngeal Tube Flap and Palatoglossal Rotation Flap in Subtotal Soft Palate Reconstruction.

Reconstruction of the soft plate after oncologic resection is complex owing to the palate's complex role in speech and swallow. This challenge becomes more complex as defects increase in size. Current management of soft plate reconstruction includes obturation as well as local or microvascular flaps. These methods, though, are limited by poor function and complexity. In regard to large subtotal central soft palate defects, we expand on previous descriptions of a tubed posterior pharyngeal constrictor flap to include a palatoglossal rotational flap that is facile and hardy and has a decreased r

[Case report of maxillary obturator prosthesis rehabilitation following maxillectomy].

PATIENT: The patient was a 75-year-old woman who underwent maxillary bone resection due to maxillary left gingival carcinoma. The treatment denture for defected jaw was applied, and adjustment was done several times. Thereafter, the final denture for defected jaw was inserted. DISCUSSION: We applied the hollow-type obturator for the treatment denture for defected jaw. Because we considered that adjustment of the hollow-type obturator was easier than buccal flange type because of the patient's deficient ability of mouth opening. Adjustment was done several times; thereafter, the final denture

An innovative method for manufacturing the Tuebingen palatal plate for infants with Robin sequence.

BACKGROUND: Robin sequence (RS) is characterized by mandibular micro- and retrognathia, glossoptosis, upper airway obstruction and optionally a cleft palate. With an incidence of 1:8000, it belongs to the so-called rare diseases; 30-50% of patients have RS as part of a syndrome. A comparatively well-studied treatment option is the Tuebingen Palatal Plate (TPP), which has proven effective in both, isolated and syndromic RS, but often requires multiple endoscopies for perfect fit and effectiveness. We report on a new method for fitting the TPP with only one session of nasopharyngeal endoscopy re

A novel approach for prosthodontic management of patient with cleft of palate.

Nutrition is important in every stage of child development. A child born with cleft lip and palate may experience difficulties while feeding due to the lack of seal of the oral cavity due to incomplete facial and palatal structures. Difficulty in feeding leads to inadequate nutrition and affects the health. Children with cleft lip and palate need certain modifications to thrive and grow. Feeding difficulties should be assessed and intervened as early as possible, as they are an important aspect of multidisciplinary team approach in management and may have an impact on long-term outcome.
