[Essential psychological care in palliative medicine].

Because of progress in cancer treatment, the development of cancer is not necessarily a death sentence. On the other hand, approximately half of patients actually die from cancer even today, and many cancer patients require care for psychological distress. From the viewpoint of psychiatric diagnosis, the most common psychiatric conditions experienced by cancer patients are adjustment disorders and/or major depression. From the viewpoint of psychological status, however, cancer patients without psychiatric disorders may still suffer from fears of recurrence during the survival period and psycho

Managing lymphoedema in palliative care patients.

The development of lymphoedema in advanced disease is distressing for patients and their carers and can prove difficult to manage for health-care professionals involved in their care. This article will provide an overview of co-morbidities that cancer patients face that will have an impact on the development, progression or management of lymphoedema. The principles of assessing and managing lymphoedema in palliative care patients is presented, based on the Scottish governments action plan Living and Dying Well. The need for collaboration with other members of the multi-disciplinary team to pro

Palliative care in amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis (ALS) is the most common degenerative motor neuron disease in adults. The clinical picture consists of generalized fasciculations, progressive atrophy and weakness of the skeletal muscles, spasticity and pyramidal tract signs, dysarthria, dysphagia, and dyspnea. Pseudobulbar affect is common. Disease-specific treatment options are still unsatisfactory. Therapeutic nihilism is not justified as a large array of palliative measures available to enhance the quality of life of patients and their families. Because of its clinical characteristics, ALS represents a paradi

Subcutaneous midazolam infusion in palliative care.

This article describes the use of a subcutaneous infusion of midazolam to control restlessness and agitation in 23 patients during the final stages of advanced cancer. Midazolam effectively controlled symptoms in 22 of the patients at an initial dose of 0.4-0.8 mg/hr rising to a mean maximum dose of 2.9 mg/hr. Midazolam mixed in the same syringe as diamorphine was well tolerated at injection sites. The wide dose range emphasizes the need for careful titration of dose for individual patients. At high doses, the volume of injection required presented some practical difficulties, but otherwise th

Delphi method validation of a tool for planning home visits in palliative care: palliative care parametrisation tool.

BACKGROUND: Palliative care parametrisation tool (PCPT) was developed to standardise the planning of home visits in palliative care, specifically for the adult oncology population in Argentina. This study aimed to validate the nursing score (NS) component of the tool using the Delphi method. METHODS: The NS was designed to quantify the caregiving burden based on patient-related factors. Two Delphi rounds were conducted with 19 expert palliative care professionals from Argentina, including 13 physicians and 6 nurses. The content validity of the NS was assessed using the Aiken V test and consen

Ethics of artificial intelligence prognostication in palliative care: perspectives from a national survey of palliative care physicians.

OBJECTIVES: Artificial intelligence (AI) tools have the potential to transform access to and delivery of palliative care globally-an urgent unmet need-in part by early identification of patients experiencing serious illness-related suffering. Although global consensus is emerging about the importance of ethical principles for AI, such as respecting autonomous choice, promoting patient well-being, reducing bias, trust, and more, little is known about how palliative care physicians perceive these issues. This study aims to improve understanding of palliative care physicians' perspectives about t

Care in palliative care: a challenging concept with normative issues.

Palliative care is an approach for seriously ill patients. Illnesses and knowledge of limited life expectancy often limit self-determination among patients. Along with the concepts of patient autonomy and heteronomy, care is central to the everyday personal and institutional lives of the ill. However, the term 'care' has not yet been systematically examined. I argue for a clear distinction between care and paternalism in the discussion about the individual approach to a situation in everyday life, which are two different forms of action in which the patient's will is considered to varying degr

[Wound care management of ulceration in a palliative care patient].

The wound care management of a palliative care patient with sinusitis pinoidalis is documented. The treatment is validated by the expert's opinion. There is only little evidence-based treatment of such a lesion. In our experience, photo documentation is helpful in decision-making during the treatment process. In addition, healing of larger defects seems to improve upon frequent and extensive liquid cleansing.

Practice of Palliative Sedation in Patients Followed by a Palliative Home Care Team.

CONTEXT: Information about the use of palliative sedation (PS) at home is limited. OBJECTIVES: The aim of this study was to assess the frequency, indications, and modalities of PS in patients with incurable disease, including cancer and non cancer patients, followed at home. METHODS: A consecutive sample of patients who were admitted to home palliative care was prospectively assessed. From the entire sample, patients who underwent PS were selected. Data regarding indications for PS, drugs and their maximum doses, intent (proportional and deep PS), and duration of PS were collected. The level

Knowledge and Attitudes towards Palliative Care: Validation of the Spanish Version of Questionnaire on Palliative Care for Advanced Dementia.

Background: Palliative care is essential in the care of people with advanced dementia, due to the increasing number of patients requiring care in the final stages of life. Nurses need to acquire specific knowledge and skills to provide quality palliative care. The Questionnaire on Palliative Care for Advanced Dementia (qPAD) is useful for assessing knowledge and attitudes toward palliative care, but its adaptation to the Spanish language and analysis of its effectiveness and usefulness for the Spanish culture is lacking. Objective: To report on the Spanish language adaptation and psychometric

The National Palliative Care Research Center and the Center to Advance Palliative Care: a partnership to improve care for persons with serious illness and their families.

The elimination of suffering and the cure of disease are the fundamental goals of medicine. While medical advances have transformed previously fatal conditions such as cancer and heart disease into illnesses that people can live with for many years, they have not been accompanied by corresponding improvements in the quality of life for these patients and their families. Living with a serious illness should not mean living in pain or experiencing symptoms like shortness of breath, nausea, or fatigue. Yet, multiple studies over the past decade suggest that medical care for patients with advanced

Integration of Palliative Care in Acute Care Hospitals: A Multicenter Survey Study on the Organization of Palliative Care Teams and Their Collaboration With Ward Staff.
