[Palliative care and dying in emergency medical services-Attitudes, perspectives and knowledge of emergency physicians].

BACKGROUND: Emergency physicians are confronted with the topic of death and dying almost daily in their work. Demographic changes and the accompanying increase in chronic and incurable diseases at the end of life as well as the frequently expressed wish to die at home, are leading to an increase in emergency situations with palliative patients. To date, there is insufficient data available on how emergency physicians deal with palliative patients. METHOD: Emergency physicians from two service providers participated in the survey 'Palliative Care and Dying in Emergency Medical Service'. The em

Prevalence of patients with unmet palliative care needs in a University Emergency Department in Germany - a cross-sectional screening study.

BACKGROUND: People with life-limiting diseases can benefit from early integration of palliative care, yet early identification of these patients is challenging. Emergency Departments (ED) often serve as the initial contact point for hospital admissions due to acute medical problems, providing an opportunity to recognize palliative care needs. This study aims to determine the prevalence of unmet palliative care needs among patients presenting to the ED of a German university hospital and to assess the proportion of these patients already receiving outpatient palliative care services. METHODS:

How do palliative care rotations impact junior doctors?

BACKGROUND: There is a current inequitable distribution of palliative care services and an increasing need for specialist and generalist palliative care workforce globally. Despite this, there are difficulties in retaining and recruiting palliative care specialists and in developing generalist palliative care skills for all clinicians. This study aims to investigate how palliative care rotations influence junior doctors in their career choices regarding palliative medicine, to explore how junior doctors develop generalist palliative care skills, and to explore possible barriers to providing ac

Evaluation of Palliative Care Technical Skills in an Acute Care Setting: A Simulation-Based, Qualitative Assessment.

ObjectivesEmergency physicians frequently encounter patients with serious illness, yet training in palliative care competencies remains inconsistent during emergency medicine (EM) residency. Simulation-based education may provide a method for integrating palliative care principles into EM training. The objective of this study was to qualitatively examine EM residents' perceptions of the intersection between palliative care and emergency medicine following participation in palliative care-focused simulation scenarios.MethodsWe conducted a qualitative pilot study using thematic analysis of semi-

[Palliative care in the light of legal and regulatory requirements in Germany].

In Germany, palliative care has developed rapidly since the establishment of the first palliative care unit in 1983. More improvements in patient-centered care are only possible if legal requirements as well as education of physicians, nurses, and other professionals involved in palliative care are adapted to current needs. This paper provides an overview of legal and regulatory requirements including a critical appreciation of their influence on palliative care in Germany. Only recently has medical education in palliative medicine been implemented as an integral part of medical studies at Ger

Changing practice through the art of palliative care - An infographic guide.

Despite decades of clinical practice, education and integration with other healthcare teams, palliative care remains enshrouded by myth and misconception. Through the powerful medium of comics, this article presents ten of the most misunderstood areas in palliative care. The topics explored were produced in conjunction with practising UK palliative care trainee doctors and realised to artistic form by a physician expert in graphic medicine. A novel infographic approach was carefully developed to be approachable yet thought-provoking. Each infographic is paired with content created to encourage

Integrative Veterinary Hospice and Palliative Care.

Hospice and Palliative care is focused on improving and maintaining quality of life for both the pet and their family. This goal is often accomplished by using conventional pharmaceuticals, which can be very effective but may also have side effects or concerns about use in specific patients. Hospice and palliative care may be greatly enhanced by the use of integrative options such as acupuncture, herbal medicine, and essential oils, as well as physical modalities (chiropractic medicine, physical rehabilitation, massage, etc) in proving relief from the common conditions seen in the hospice and

Why Canadian MAiD is Not a Medical Procedure.

Canada introduced Medical assistance in dying (MAiD) in 2016, and it now accounts for more than 5% of all deaths in the country. Although it is framed as a healthcare service, MAiD is fundamentally incompatible with medicine. There are procedural, philosophical, and ethical distinctions between MAiD and palliative care, which are also confirmed by major national and international organizations including the World Health Organization (WHO) and the Canadian Society of Palliative Medicine. MAiD contradicts the foundational principles of medicine, including the Hippocratic Oath's prohibition again

[Treatment and Support for Adults at the End of Life in Intensive Care : A Recommendation by the Ethics Section of DIVI and the German Society for Palliative Medicine. Part 1: Key Aspects of End-of-Life Care and Dying in Intensive Care. Part 2: Therapeutic Measures and Support at the End of Life in Intensive Care].

The primary goal of intensive care medicine is to overcome a critical phase of illness using all available means and to enable patients to survive and return to an independent life without intensive care. However, this therapeutic goal cannot always be achieved. The possibility of death, the need to alleviate suffering, and respect for the end of life make it necessary to integrate palliative approaches into intensive care medicine. Supporting seriously ill and dying patients and their relatives in intensive care units requires an attitude that holistically recognizes and respects their indivi

Environmental sustainability in palliative care: a narrative review.

BACKGROUND AND OBJECTIVE: Climate change represents an escalating threat to human health and disproportionately affects vulnerable populations. The healthcare sector contributes significantly to climate change, accounting for approximately 4.4% of greenhouse gas (GHG) emissions globally. While several medical specialties aim to improve sustainability of clinical practice, there is limited literature within palliative medicine. This narrative review aims to identify and evaluate existing work on the ecologic impact of palliative medicine and highlight climate conscious clinical recommendations

Dying like a dog: the convergence of concepts of a good death in human and veterinary medicine.

Standard views of good death in human and veterinary medicine considerably differ from one another. Whereas the good death ideal in palliative medicine emphasizes the positive aspects of non-induced dying, veterinarians typically promote a quick and painless killing with the aim to end suffering. Recent developments suggest a convergence of both professions and professional attitudes, however. Palliative physicians are confronted with patients wishing to be 'put to sleep', while veterinarians have begun to integrate principles and practices from hospice care. We will argue that the discourses

Unique Palliative Care Challenges in a Young Adult With a Rare Malignancy.

The incorporation of a palliative evaluation and care plan development for patients with serious illness is becoming a standard of care in modern medicine. For patients with complex challenges, palliative care physicians are well trained to offer specialized treatment plans and interventions to mitigate symptoms. This case presents the challenges of a mixed hepatocellular carcinoma and hepatoblastoma initially diagnosed in a 17-year-old male. Despite attempts at aggressive cancer-directed treatment, disease progression prompted referral for a palliative evaluation at age 20. Challenges unique

[Pain therapy in urology-overview of current S3 guideline recommendations].

Dealing efficiently with patients suffering from pain is a central medical task. Pain, as an important function in developmental physiology, warns against damage to the body caused by external noxious agents as well as internal malfunctions and requires special attention in modern medicine. Peri- and postoperative pain is known to have a negative influence on postoperative convalescence. Treatment of tumor-related pain represents another relevant challenge in uro-oncology and palliative medicine. The updated guideline on perioperative pain therapy and palliative medicine for patients with incu

A Review of Prognostic Tools in Heart Failure.

A minority of patients with end-stage disease are referred to palliative medicine for consultation in advanced heart failure. Educating stakeholders, including primary care, cardiology, and critical care of the benefits of hospice and palliative medicine for patients with poor prognosis, may increase appropriately timed referrals and improve quality of life for these patients. This article reviews multiple tools useful in prognostication in the setting of advanced heart failure.
