The records of all patients who underwent pylorus-preserving pancreatic resection (29 subtotal and 4 total pancreatectomies) during a 10-year period at the Mayo Clinic were reviewed. Thirty-day operative mortality was 6%. Early postoperative morbidity occurred in 45% of patients and necessitated reoperation in four patients. One patient had a hemorrhage from a marginal ulcer in the early postoperative period. The incidence of late postoperative morbidity was 18%. Marginal ulcers developed in the late postoperative period in three additional patients, for an overall incidence of 13%. One patien

Seventy-four patients were treated with a radical or a nonradical pancreatectomy for ductal cell carcinoma of the head of the pancreas. Their survival rates and the selection of the operative procedure were evaluated. In 32 patients, a radical pancreatectomy was attempted where there was sufficient clearance of regional or juxta-regional lymph nodes beyond the group of suspected metastatic nodes, as well as a resection of a greater margin of soft tissue around the pancreas. These patients' cumulative 5-year survival rate was 33.4%. In 14 Stage I or Stage II patients, the cumulative 5-year surv

This scoping review investigated the incidence and risk factors of new-onset diabetes mellitus (NODM) after pancreatectomy. A systematic search of PubMed, Embase, Cochrane Library, and Web of Science (2014-2025) identified 14 relevant articles. Results showed NODM incidence of 25% at 1 year, 35% at 2 years, and 39% at 3 years postoperatively. Key risk factors included prediabetic conditions (n = 7), aberrant metabolic parameters (n = 6), age (n = 4), postoperative pancreatic volume (n = 4), and type of surgery (n = 3). As NODM incidence increases over time, targeted monitoring and management g

BACKGROUND: Laparoscopic distal pancreatectomy (LDP) has emerged as the preferred approach for both benign and malignant lesions located in the pancreatic body and tail. Nevertheless, a notable deficiency persists in the absence of a standardized, procedure-specific metric for evaluating and comparing surgical quality. A composite measure termed "textbook outcome (TO)", which encompasses key short-term endpoints, has been validated in laparoscopic pancreatoduodenectomy but has not yet been established in dedicated LDP cohorts. The definition and prediction of TO in this context could aid in fa

BACKGROUND: Although endocrine insufficiency after distal pancreatectomy (DP) has been extensively studied, data on pancreatic exocrine insufficiency (PEI) after DP remain limited. This study aimed to evaluate the characteristics and clinical relevance of PEI after DP.
METHODS: A total of 184 patients who underwent DP between June 2013 and December 2022 were retrospectively analyzed. Demographics, perioperative parameters, and postoperative outcomes were evaluated. The median follow-up duration was 24 months.
RESULTS: Postoperative de novo PEI occurred in 48.4% of patients. Patients who deve

BACKGROUND: The number of robotic pancreaticoduodenectomy (R-PD) and robotic distal pancreatectomy (R-DP) procedures has been increasing worldwide. However, there are no reports on clinical cases of robotic remnant total pancreatectomy (R-RTP). This report presents a standardized surgical technique for R-RTP after R-PD based on the authors' experience with three clinical cases. The procedure is presented with accompanying surgical videos, and its feasibility and potential advantages are discussed.
PURPOSE: This study presents a standardized surgical technique for R-RTP following R-PD, based o

PURPOSE: Postoperative hemorrhagic complications in pancreatic surgery are classified according to the International Study Group for Pancreatic Surgery (ISGPS). However, following total pancreatectomy, the predominant bleeding causes associated with pancreatic fistula or insufficiency of the pancreatico-enteric anastomosis are eliminated. The objective of this study is to examine bleeding sources following total pancreatectomy, and propose a novel classification, termed Post-Total Pancreatectomy Hemorrhage (PTPH).
METHODS: An overall of 195 patients was included and reviewed for baseline char

This article details complications occurring after pancreatectomy. Morbidity following pancreatectomy is high, and failure to rescue can lead to mortality. Complications can happen early (≥30 days) or late (<30 days). Nonoperative management of complications has become first line.

Total pancreatectomy is one of the radical treatments for various severe pancreatic conditions, including pancreatic cancer, chronic pancreatitis, and pancreatic trauma. However, the postoperative complication rate is higher compared to other surgical methods, and all patients inevitably develop insulin-dependent diabetes due to the loss of islet function. Previously, studies focused on pancreas/islet transplantation or stem cell therapy conducted in diabetic patients have achieved significant success, suggesting that adopting similar strategies to preserve or restore islet function after tota

We present three cases of spleen-preserving total pancreatectomy using the Warshaw technique for neoplasms of the pancreatic head. The first case involved a 5 cm mucinous pancreatic neoplasm invading the middle colic artery. The second case featured a patient with a BMI of 40 and adenocarcinoma of the pancreatic head. The third case involved a serous cystadenoma in the pancreatic head in a patient with a history of gastric surgery for a gastric ulcer, requiring surgical intervention due to weight loss and abdominal symptoms. These cases highlight the surgical nuances and benefits of the Warsha

OBJECTIVE: This meta-analysis aims to evaluate the effect of prophylactic abdominal drainage on post-pancreatectomy complications, a topic that is still debated in the medical community.
MATERIALS AND METHODS: Following PRISMA guidelines, the authors conducted a systematic search across databases such as PubMed, EMBASE, Scopus, Cochrane Library, Ovid, clinicaltrials.gov, Web of Science, CNKI, and WanFang Data, focusing on studies comparing intraperitoneal drainage with no drainage after pancreatic surgery. Key outcomes included postoperative pancreatic fistula (POPF), clinically relevant POPF

We introduce a novel notation system for pancreatectomy designed to provide a clear and concise representation of surgical procedures. As surgical techniques and the scope of pancreatic surgeries continue to diversify, existing communication methods among medical professionals regarding the specifics of the surgeries have proven inadequate. Our proposed notation system clearly indicates the approach (open, laparoscopic, or robot-assisted), type of surgery (e.g., pancreatoduodenectomy, distal pancreatectomy), and extent of resection and accompanying resected organs or vasculature. These element

Intraperitoneal prophylactic drain (IPD) use in distal pancreatectomy (DP) is still controversial. A survey was carried out through the Italian community of pancreatic surgeons using institutional emails, Twitter, and Facebook accounts of the Italian Association for the Study of the Pancreas (AISP) and the Italian Association of Hepato-biliary-pancreatic Surgery (AICEP). The survey was structured to learn surgeons' practice in using IPD through questions and one clinical vignette. Respondents were asked to report their regrets for omission and commission regarding the IPD use for the clinical

AIMS: Children evaluated for abdominal pain are increasingly recognized to have pancreatic lesions by imaging modalities. Malignant lesions of the pancreas have also been diagnosed at regular intervals, the most common being solid cystic pseudopapillary neoplasm (SPT) - Borderline (uncertain malignant potential). Surgical resection of this tumor should provide adequate tumor free margins and also should preserve pancreatic tissue. Radical resection of the pancreas will lead to pancreatic insufficiency. Herein, we describe the technique of central pancreatectomy wherein tumor excision gives ade

OBJECTIVES: Pancreatic body and tail tumors account for one-third of all pancreatic tumors and can be diagnosed later than pancreatic head tumors because they present symptoms much later. When analyzed, most of them are generally unresectable because they are invaded into adjacent organs and vascular structures. We aimed to present our cases of vascular resection and anastomosis, where isolated tumor resection cannot be performed due to invasion of adjacent vascular structures.
METHODS: Between January 2019 and January 2021, the files of eight patients who had one or more vascular invasions d

BACKGROUND: Multifocal neuroendocrine tumors (NET) usually occur in the context of a multiple neuroendocrine neoplasia type 1 (MEN1). When the proximal part of the pancreatic body is spared by NET, Miura et al. have proposed a "middle-segment preserving" pancreatectomy (MSP) as alternative to total pancreatectomy [1-3].
VIDEO: A 28-year-old woman with MEN1 was referred for surgical resection of a multifocal pancreatic tumor with single metastasis located and a single liver metastasis in close contact with the left hepatic duct. The preoperative work-up by DOTATOC-PETSCAN revealed multifocal t
