The results of adductor magnus tenodesis in adolescents with recurrent patellar dislocation.

Recurrent dislocation of the patella is a common orthopaedic problem which occurs in about 44% of cases after first-time dislocation. In most cases of first-time patellar dislocation, the medial patellofemoral ligament (MPFL) becomes damaged. Between 2010 and 2012, 33 children and adolescents (39 knees) with recurrent patellar dislocation were treated with MPFL reconstruction using the adductor magnus tendon. The aim of our study is to assess the effectiveness of this surgical procedure. The outcomes were evaluated functionally (Lysholm knee scale, the Kujala Anterior Knee Pain Scale, and isok

An algorithm guiding the evaluation and treatment of acute primary patellar dislocations.

Primary patellar dislocation continues to be a common problem facing clinicians today. These injuries are associated with significant morbidity and a substantial recurrence rate. A myriad of operative and nonoperative options have been described to treat these injuries though the evidence-based literature is sparse. As a result, much controversy exists regarding the ideal management of the primary patellar dislocation. This article reviews the literature on this topic, from the initial examination and management to definitive treatment. Furthermore, an algorithm is presented to help the clinic

Classification of lesions of the medial patello-femoral ligament in patellar dislocation.

The remnants of the medial patello-femoral ligament (MPFL) of 67 knees, 18 with acute patellar dislocation and 49 with chronic patellar dislocation, were studied. The MPFL injuries of the acute cases were categorised into 2 groups: an avulsion tear type and a substantial tear type. The chronic cases were put into 3 groups: those with loose femoral attachment (9 knees), those with scar tissue formation or abnormal scar branch formation (29 knees), and those with no evidence or continuity of the ligament (absent type) (11 knees). It is concluded that incompetence of the medial patello-femoral li

[Advances on the treatment of the first-time patellar dislocation in children and adolescents].

The first patellar dislocation in children and adolescents is a common clinical sports injury. The controversy in the treatment strategies poses a significant challenge to long-term functional prognosis of patients. Studies have shown that demographic factors (young age, female gender), skeletal maturity, and anatomical abnormalities (such as trochlear dysplasia (TD), increased distance between tibial tubercle and trochlear groove (TT-TG) were closely related to patellar dislocation. Although conservative treatment is the preferred option, it has a high recurrence rate and is prone to leaving

Assessing Femoral Anteversion in Patients With Bilateral Recurrent Patellar Dislocations.

BACKGROUND: After first-time lateral patellar dislocation, 44% to 70% of patients sustain redislocations. Increased femoral anteversion (FA) is considered to result in increased lateralizing forces on the patella, which might predispose one to patellar instability. When recurrent patellar dislocations (RPDs) are bilateral, it is unclear if the FA is even more increased. HYPOTHESIS: Patients with bilateral RPD would have greater FA than patients with unilateral RPD. STUDY DESIGN: Case-control study; Level of evidence, 3. METHODS: A total of 52 skeletally mature patients with RPD and a clinic

The influence of directional preference on lateral patellar dislocation: a case report.

BACKGROUND: There is little consensus on the conservative management of lateral patellar dislocations (LPD). Mechanical diagnosis and therapy (MDT) is an established classification system in the spinal and extremity population. This case report describes the use of MDT in the management and classification of a patient with LPD. CASE DESCRIPTION: The patient was a 20-year-old female with a 3-month history of left knee pain precipitated by a lateral patellar dislocation. The patient described pain and a feeling of instability with standing and walking and limitations in work and recreational ac

Local torsion of distal femur is a risk factor for patellar dislocation.

PURPOSE: It has been widely reported that femoral anteversion is a risk factor for patellar dislocation. This study aims to evaluate whether internal torsion of the distal femur is noticeable in patients without increased femoral anteversion and to assess whether it is a risk factor for patellar dislocation. METHODS: A retrospective analysis was conducted on 35 patients (24 females, 11 males) with recurrent patellar dislocation but without increased femoral anteversion treated in our hospital from January 2019 to August 2020. All patients underwent knee X-rays, digital radiography of lower-li

Patellar dislocation is associated with increased tibial but not femoral rotational asymmetry.

PURPOSE: Patellar dislocation is associated with a range of anatomical abnormalities affecting the trochlea, extensor mechanism and the tibia. The relationship between patellofemoral instability and rotational abnormalities of the posterior condyles, trochlear groove and proximal tibia has not been adequately determined. This study aimed to identify the frequency and severity of anatomical risk factors to determine their relative contribution to patellofemoral instability. METHODS: A retrospective morphological study was undertaken comparing multiple anatomical measurements with magnetic reso

The cartilaginous femoral sulcus in children with patellar dislocation: an ultrasonographic study.

One hundred and sixteen knees of 33 patients with patellar dislocation and 25 normal children 12-18 years of age were examined by ultrasonography to measure the cartilaginous sulcus angle on the patellar surface of the femur. In knees with patellar dislocation, the cartilaginous sulcus angle measured between 154 and 195 degrees, exceeding the corresponding values of normal knees (134-153 degrees). In patient knees, the cartilaginous sulcus was also consistently wider than the underlying osseous sulcus. This suggests that in pediatric patients with patellar dislocation, the actual patella-stabi

Neglected Busch-Hoffa fracture following the patellar dislocation: a rare case report.

INTRODUCTION AND IMPORTANCE: Busch-Hoffa fractures are rare intra-articular coronal plane fractures of the femoral condyle that often remain unnoticed, particularly when complications develop. Due to their instability and risk of long-term problems, these fractures require immediate anatomical reduction and stable fixation. The occurrence of a Busch-Hoffa fracture in conjunction with patellar dislocation is extremely rare and has been rarely reported in the current literature. CASE PRESENTATION: We report a case involving a 21-year-old male who experienced a Bagaria type 2 Hoffa fracture of t
