Radiographic and ultrasonographic changes of the patellar ligament following tibial tuberosity advancement in 25 dogs.

INTRODUCTION: Patellar ligament thickening is a recognized response following osteotomy of the proximal tibia as a treatment for cranial cruciate disease. In humans this is seen as a response to increased loading, but the reason for this thickening in dogs is unclear. A prospective study was undertaken to assess the degree and frequency of patellar ligament desmopathy in 25 consecutive cases undergoing tibial tuberosity advancement (TTA). OBJECTIVES: To determine the incidence of patellar ligament thickening following TTA. METHODS: Consecutive cases undergoing TTA had ultrasonographic and ra

Zone of injury of the medial patellofemoral ligament after acute patellar dislocation in children and adolescents.

BACKGROUND: Patellar dislocation is a common traumatic injury in the pediatric and adolescent population. The primary constraint to lateral subluxation and dislocation of the patella is the medial patellofemoral ligament (MPFL), which serves to resist lateral translation of the patella. Injury to the MPFL may predispose to recurrent dislocation but the anatomic site of injury is poorly characterized in children and adolescents. PURPOSE: The authors addressed 2 questions: (1) What is the zone of injury to the MPFL in a pediatric/adolescent population after primary patellar dislocation? (2) Wha

Gene transfer to the rabbit patellar tendon: potential for genetic enhancement of tendon and ligament healing.

Injuries to ligaments and tendons occur very commonly, but often heal very poorly. A number of growth factors show promise as biological agents with which to augment the reparative responses of these tissues, but they are extremely difficult to deliver clinically. We have suggested the use of gene transfer as a possible way to obviate this limitation. Here we have used the lacZ marker gene to evaluate methods for gene delivery to ligaments and tendons. Cell cultures were established from the following tissues of the New Zealand white rabbit: anterior and posterior cruciate ligaments, medial co

Histochemical aspects of the proteoglycans of patellar tendon autografts used to replace the posterior cruciate ligament.

In the female German black-faced sheep the posterior cruciate ligament was replaced by a free patellar tendon autograft and after 2, 6, 16, 26 and 52 weeks tissue samples of the graft's center (axial region far from bones) were removed for histochemistry and electron microscopy. To localize the proteoglycans Alcian Blue and 0.3 M MgCl2 were added to the fixative solution. The distribution of the proteoglycans in the graft was compared to that of a normal patellar tendon and of a normal posterior cruciate ligament. In the patellar tendon spindle-shaped cells predominated and proteoglycans appea

Relationship between MRI T2 mapping parameters and patellar ligament degeneration in patients with knee osteoarthritis.

This study aims to investigate the relationship between MRI T2 mapping parameters and patellar ligament degeneration in patients with knee osteoarthritis (KOA) and evaluate its value in disease grading and prediction of clinical symptoms. A total of 149 KOA patients (284 knees) and 80 healthy subjects (141 knees) who underwent knee MRI between February 2023 and February 2024 were retrospectively analyzed. T2 mapping was performed, and the patellar ligament was divided into proximal, middle, and distal regions. Regions of interest were delineated to measure the mean (T2mean) and maximum (T2max)

Cost-Effectiveness of Medial Patellofemoral Ligament Reconstruction for First-Time Patellar Dislocations: A Markov Analysis.

BACKGROUND: Nonoperative management versus medial patellofemoral ligament reconstruction (MPFLR) for first-time patellar dislocations remains controversial, and it is unclear whether the increased up-front costs from MPFLR are justified. PURPOSE: To compare the cost-effectiveness of an initial trial of physical therapy alone to that of early MPFLR to determine the preferred cost-effective treatment strategy for first-time patellar dislocations. STUDY DESIGN: Economic and decision analysis; Level of evidence, 3. METHODS: A Markov chain Monte Carlo probabilistic model was developed to evaluat

Anterior Cruciate Ligament Revision Using Bone-Patellar Tendon-Bone Autograft and Lateral Extra-articular Tenodesis.

Revision of anterior cruciate ligament reconstruction presents various challenges not encountered in the primary settings, including malpositioned tunnels, tunnel widening, and the lack of consensus on the ideal graft to be used. This Technical Note describes a one-stage anterior cruciate ligament reconstruction revision using a bone-patellar tendon-bone autograft combined with lateral extra-articular tenodesis. This technique represents the ideal approach to tackle complex revision cases primarily characterized by tibial and femoral tunnel osteolysis and rotational knee instability. The bone-

Recurrence of Patellar Instability in Adolescents Undergoing Surgery for Osteochondral Defects Without Concomitant Ligament Reconstruction.

BACKGROUND: First-time patellar dislocation with an associated chondral or osteochondral loose body is typically treated operatively to address the loose fragment. The incidence of recurrent instability in this patient population if the medial patellofemoral ligament (MPFL) is not reconstructed is unknown. PURPOSE: To determine the recurrent instability rate in patients undergoing surgery for patellar instability with chondral or osteochondral loose bodies, as well as to identify and stratify risk factors for recurrent instability. Tibial tubercle fragmentation: a clue to simultaneous patellar ligament avulsion in pediatric tibial tubercle fractures.

Simultaneous avulsions of the tibial tubercle and patellar ligament have been reported, but are rare. We present an 11-year-old boy who was initially diagnosed with a Type IIIA tibial tubercle avulsion fracture after falling off of his bicycle. Intraoperatively, following exposure and fixation of the tibial tubercle fragment, knee range of motion under live fluoroscopy revealed a stationary patella with no tension in the patellar ligament. The incision was extended and a distal avulsion of the patellar ligament from the tibial tubercle was identified, with a small flap of tibial tubercle perio

A prospective randomized study of patellar versus hamstring tendon autografts for anterior cruciate ligament reconstruction.

BACKGROUND: Bone-patellar tendon-bone graft has been the most commonly used graft material in anterior cruciate reconstructions, but there has been increasing use of hamstring tendon grafts. However, no existing clinical studies show adequate support for the choice of one graft over the other. HYPOTHESIS: Hamstring tendons are equally as good as patellar tendon in anterior cruciate ligament reconstructions. STUDY DESIGN: Prospective randomized clinical trial. METHODS: Ninety-nine patients with laxity caused by a torn anterior cruciate ligament underwent arthroscopically assisted reconstruct

Central third bone-patellar tendon-bone anterior cruciate ligament reconstruction: a 5-year follow-up.

Arthroscopically assisted anterior cruciate ligament (ACL) reconstruction using the central third bone-patellar tendon-bone as a free autologous graft is now a widely used procedure. However, little has been published regarding its long-term success. In this retrospective study, we report on the results of 32 patients who were followed-up over 5 years after their reconstruction. Twenty-five patients had improved on their prereconstruction Lysholm and Tegner Activity scores and the same number denied having any symptoms of instability. The mean Lysholm knee score was 88.5. All patients had with
