Predictors of pathologic complete response in early-stage triple-negative breast cancer treated with neoadjuvant chemo-immunotherapy: a multi-institution study.

INTRODUCTION: The KEYNOTE-522 clinical trial demonstrated that the addition of pembrolizumab to 8 cycles of neoadjuvant chemotherapy (NAC) improves pathologic complete response (pCR) rates and overall survival in early-stage triple-negative breast cancer (TNBC). However, predictors of response and the benefit of alternative NAC backbones with immunotherapy are not known. This multi-institutional study evaluates clinical factors and treatment variables associated with pCR following NAC plus pembrolizumab in a diverse, real-world cohort. METHODS: This multi-institution retrospective study analy

Predictive value of 18F-FDG-PET/CT for pathologic complete response (pCR) of lesions after neoadjuvant chemotherapy in advanced ovarian cancer.

OBJECTIVE: This study aimed to evaluate the ability of 18F-FDG-PET/CT to predict pathological complete response (pCR) in lesions of patients with advanced epithelial ovarian cancer (EOC) following neoadjuvant chemotherapy (NACT). We assessed whether 18F-FDG-PET/CT could predict lesion-specific pCR prior to interval debulking surgery (IDS), thus guiding precise lesion resection. METHODS: Twenty-four patients with advanced EOC underwent IDS after NACT. 18F-FDG-PET/CT imaging was performed before and after NACT, with the maximum standardized uptake value (SUVmax) recorded for each metabolically

Baseline and Early Treatment MRI Model for Predicting Complete Pathologic Response to Neoadjuvant Chemoimmunotherapy in Patients With Triple-Negative Breast Cancer.

BACKGROUND. Triple-negative breast cancer (TNBC) is a highly aggressive breast cancer subtype lacking targeted therapies. Neoadjuvant chemoimmunotherapy (NACI) improves pathologic complete response (pCR) rates, although patient selection is challenging. OBJECTIVE. The purpose of the study was to develop and test a model incorporating baseline and early-treatment MRI features, including dynamic contrast enhancement (DCE) features, for predicting pCR in patients with TNBC undergoing NACI. METHODS. This retrospective study included patients with TNBC undergoing NACI who underwent breast MRI exami

Accuracy of Magnetic Resonance Imaging-Guided Biopsy to Verify Breast Cancer Pathologic Complete Response After Neoadjuvant Chemotherapy: A Nonrandomized Controlled Trial.

IMPORTANCE: After neoadjuvant chemotherapy (NAC), pathologic complete response (pCR) is an optimal outcome and a surrogate end point for improved disease-free and overall survival. To date, surgical resection remains the only reliable method for diagnosing pCR. OBJECTIVE: To evaluate the accuracy of magnetic resonance imaging (MRI)-guided biopsy for diagnosing a pCR after NAC compared with reference-standard surgical resection. DESIGN, SETTING, AND PARTICIPANTS: Single-arm, phase 1, nonrandomized controlled trial in a single tertiary care cancer center from September 26, 2017, to July 29, 20

High TFAP2C/low CD44 expression is associated with an increased rate of pathologic complete response following neoadjuvant chemotherapy in breast cancer.

BACKGROUND: In luminal breast cancer cell lines, TFAP2C regulates expression of key genes in the estrogen receptor-associated cluster and represses basal-associated genes including CD44. We examined the effect of TFAP2C overexpression in a basal cell line and characterized the expression of TFAP2C and CD44 in breast cancer specimens to determine if expression was associated with clinical response. METHODS: MDA-MB-231 breast cancer cells were treated with a TFAP2C-containing plasmid and evaluated for effects on CD44 expression. Pretreatment biopsy cores from patients receiving neoadjuvant chem

Incorporating Individual Early Response in the Dose-Effect Relationship of Complete Pathological Response Following Neoadjuvant Radiochemotherapy for Rectal Cancer.

PURPOSE: To develop and externally validate a model predicting pathological complete response (pCR) in patients with rectal cancer treated with neoadjuvant radiochemotherapy. The approach combines the classical logit dose-effect curve with individual early response assessed by magnetic resonance imaging during radiochemotherapy. METHODS AND MATERIALS: The dose-response model incorporating the early regression index (ERI) was derived from 2 published data sets. A population-averaged dose-response curve was obtained by fitting data from a recent meta-analysis. Then, an ERI-based model for pCR p

Pathologic complete response after first-line tislelizumab, cetuximab, and dabrafenib in microsatellite-stable BRAF V600E-mutant metastatic colorectal cancer: a case report.

BRAF V600E-mutant metastatic colorectal cancer (mCRC) is characterized by aggressive disease behavior and limited responsiveness to conventional systemic therapy. Although BRAF/EGFR-targeted therapies have improved clinical outcomes, durable complete remission remains uncommon, particularly in microsatellite-stable (MSS) disease. We report the case of a 26-year-old woman with MSS, BRAF V600E-mutant sigmoid colon adenocarcinoma and synchronous liver and omental metastases (cT4aN2M1c, stage IVC). Pretreatment biopsy demonstrated high programmed death-ligand 1 (PD-L1) expression (combined positiv

Early prediction of pathologic complete response to neoadjuvant chemotherapy based on longitudinal total choline of MR spectroscopy in patients with breast cancer.

BACKGROUND: Accurately predicting pathological complete response (pCR) to neoadjuvant chemotherapy (NAC) early during treatment could enable therapy adaptation. Total choline (tCho) measured by Proton MR spectroscopy (1H-MRS) reflects tumor metabolism and may serve as an early biomarker, but its predictive performance requires further validation. OBJECTIVES: To investigate the performance of tCho in singlevoxel 1H-MRS to early predict pCR to NAC in breast cancer patients. MATERIALS & METHODS: Patients with primary invasive breast cancer scheduled for NAC were prospectively enrolled from August

Spontaneous colonic transection following pathologic complete response to pembrolizumab in high microsatellite instability colorectal cancer: A case report and review of literature.

BACKGROUND: High microsatellite instability (MSI-H) colorectal cancer (CRC), caused by deficient mismatch repair, accounts for about 15% of all CRC cases and is more common in right-sided tumors. While early-stage MSI-H CRC has a relatively good prognosis, advanced cases often respond poorly to standard chemotherapy. Immune checkpoint inhibitors, such as pembrolizumab, have shown strong and lasting effects in MSI-H CRC. Pembrolizumab is now approved as a first-line treatment for metastatic MSI-H CRC due to its superior outcomes compared to traditional chemotherapy. CASE SUMMARY: A 44-year-old

Pathologic complete response of ductal carcinoma in situ to neoadjuvant systemic therapy in HER2-positive invasive breast cancer patients: a nationwide analysis.

PURPOSE: Ductal carcinoma in situ (DCIS) is present in more than half of HER2-positive invasive breast cancer (IBC). Recent studies show that DCIS accompanying HER2-positive IBC can be completely eradicated by neoadjuvant systemic therapy (NST). Our aim was to determine the percentage of pathologic complete response of the DCIS component in a nationwide cohort and to assess associated clinicopathologic variables. Furthermore, the impact on surgical treatment after NST was investigated. METHODS: Women diagnosed with HER2-positive IBC, treated with NST and surgery, between 2010 and 2020, were s

Association between pre-diagnosis BMI, physical activity, pathologic complete response, and chemotherapy completion in women treated with neoadjuvant chemotherapy for breast cancer.

PURPOSE: Physical activity and lower BMI have shown benefit for breast cancer survival, but the association between these factors, pathologic complete response (pCR), and chemotherapy completion is not clear. We evaluated whether BMI and physical activity are associated with pCR and chemotherapy completion during neoadjuvant breast cancer treatment. METHODS: We conducted a retrospective case-control study of women given neoadjuvant chemotherapy for stage I-III breast cancer between 2010 and 2016. A medical record review provided pCR, chemotherapy completion, and patient characteristics. A tel

Can Radiomics Predict Pathologic Complete Response After Neoadjuvant Chemoradiotherapy for Rectal Cancer? A Systematic Review and Meta-Analysis of Diagnostic-Accuracy Studies.

Background: The rectal cancer treatment paradigm is rapidly changing with the advent of total neoadjuvant therapy and non-operative management approaches in responders. A good clinical response to neoadjuvant treatment documented by magnetic resonance imaging, endoscopy and clinical examination corresponds, to a large extent, to a pathologic complete response, as assessed in surgical specimens. Methods: We undertook a systematic review and meta-analysis on the MRI-based omics approach to predicting pathologic complete responses. Results: A total of 29 studies with relevant data available repor

The first case of primary malignant melanoma of the esophagus to achieve pathologic complete response after preoperative ipilimumab + nivolumab followed by resection.

Primary malignant melanoma of the esophagus is a rare disease with a poor prognosis. Surgical resection is common, but there is no consensus on perioperative treatment. Studies have reported the efficacy of programmed cell death-1 inhibitors (e.g., nivolumab, pembrolizumab) and anti-cytotoxic T-lymphocyte-associated antigen 4 agents (e.g., ipilimumab) in treating malignant melanoma. Here, we present the first case of primary malignant melanoma of the esophagus with lymph node metastases treated with nivolumab and ipilimumab followed by resection, achieving a pathologic complete response.
