Live sequence charts to model medical information.

BACKGROUND: Medical records accumulate data concerning patient health and the natural history of disease progression. However, methods to mine information systematically in a form other than an electronic health record are not yet available. The purpose of this study was to develop an object modeling technique as a first step towards a formal database of medical records. METHOD: Live Sequence Charts (LSC) were used to formalize the narrative text obtained during a patient interview. LSCs utilize a visual scenario-based programming language to build object models. LSC extends the classical lan

Using Large Language Models to Detect Depression From User-Generated Diary Text Data as a Novel Approach in Digital Mental Health Screening: Instrument Validation Study.

BACKGROUND: Depressive disorders have substantial global implications, leading to various social consequences, including decreased occupational productivity and a high disability burden. Early detection and intervention for clinically significant depression have gained attention; however, the existing depression screening tools, such as the Center for Epidemiologic Studies Depression Scale, have limitations in objectivity and accuracy. Therefore, researchers are identifying objective indicators of depression, including image analysis, blood biomarkers, and ecological momentary assessments (EMA

Cutting Waste in Bronchoscopy: A Multicenter Observational Study in the German Health Care System.

BACKGROUND: Pulmonary endoscopy contributes substantially to medical waste generation. Previous studies have estimated the average waste per bronchoscopy at approximately 1.03 kg. Scalable data are lacking, however. RESEARCH QUESTION: How much waste is generated by pulmonary procedures with reusable endoscopes (bronchoscopy and endobronchial ultrasound [EBUS]) at 2 German tertiary care centers, what is the estimated national impact, and which strategies can be identified for waste reduction? STUDY DESIGN AND METHODS: In a prospective, multicenter observational study conducted over 4 weeks pe

From participatory design to clinical routine: new concepts for visualizing patient-reported outcomes in breast cancer care.

BACKGROUND: Patient-reported outcomes (PROs) improve cancer care by enabling structured self-assessment of disease-related symptoms, overall functioning, and health-related quality of life. Despite proven benefits, routine use is still limited, in part due to suboptimal data visualization. METHODOLOGY: In a sub-study of the multicenter PRO B trial (52 centers, 924 patients with metastatic breast cancer), an interdisciplinary team of clinicians, researchers, patient representatives, and service designers co-developed user-centered PRO visualizations. Drawing on a literature review, an analysis

Privatized Health Care System in Times of Crisis: South Korea's Health Care System Response to the COVID-19 Pandemic.

The COVID-19 pandemic was a global public health crisis that demanded a "whole-of-society" response. In many countries, coordinating efforts across public and private health care sectors proved challenging. Yet South Korea maintained one of the world's lowest excess mortality rates despite having a predominantly privatized health care system, with 90.3% of hospital beds privately owned. This outcome was enabled in part by the government's strategy of disproportionately mobilizing and repurposing public hospitals as dedicated COVID-19 treatment facilities. These hospitals, historically few in n

Patterns of care for people presenting to Australian general practice with musculoskeletal complaints based on routinely collected data: protocol for an observational cohort study using the Population Level Analysis and Reporting (POLAR) database.

INTRODUCTION: General practice is integral to the Australian healthcare system. Outcome Health's POpulation Level Analysis and Reporting (POLAR) database uses de-identified electronic health records to analyse general practice data in Australia. Previous studies using routinely collected health data for research have not consistently reported the codes and algorithms used to describe the population, exposures, interventions and outcomes in sufficient detail to allow replication. This paper reports a study protocol investigating patterns of care for people presenting with musculoskeletal condit

Evaluating YouTube videos for Hodgkin lymphoma patient education.

Hodgkin lymphoma (HL) is a highly treatable malignancy. As patients and families seek information online, educational resources are essential. YouTube is widely used for health information, but the quality of HL-specific content is unclear. We evaluated the characteristics, content, and reliability of YouTube videos for HL patient education. Search terms generated a list of 50 relevant videos. Data were extracted and analyzed using validated video assessment tools. Two independent reviewers evaluated reliability; accuracy was assessed using a structured consensus document across five domains.

Magnetic resonance imaging in patients with implantable cardiac devices.

The use of magnetic resonance imaging (MRI) in patients with implantable cardiac devices, such as pacemakers, cardioverter defibrillators, and loop recorders, has been contraindicated based on concerns regarding the powerful magnetic field generated by MRI. Due to the widespread application and powerful diagnostic capability of MRI, there are instances in which denying a patient with an implantable cardiac device an MRI evaluation may influence the quality of health care received. There are data to suggest that MRI might be considered a relative contraindication instead of an absolute contrain

General Practice Statistics in Australia: Pushing a Round Peg into a Square Hole.

In Australia, general practice forms a core part of the health system, with general practitioners (GPs) having a gatekeeper role for patients to receive care from other health services. GPs manage the care of patients across their lifespan and have roles in preventive health care, chronic condition management, multimorbidity and population health. Most people in Australia see a GP once in any given year. Draft reforms have been released by the Australian Government that may change the model of general practice currently implemented in Australia. In order to quantify the impact and effectivenes

Social Acceptance of Smart Glasses in Health Care: Model Evaluation Study of Anticipated Adoption and Social Interaction.

BACKGROUND: Despite the growing interest in smart glasses, it is striking that they are not widespread among health care professionals. Previous research has identified issues related to social interactions involving the use of smart glasses in public settings, which may differ from those associated with their application in health care contexts. OBJECTIVE: Assuming that smart glasses mediate contact between the health care provider and patient, the objectives of this research are two-fold: (1) to develop an instrument that combines the adoption and mediation perspectives, and (2) to gain ins

Principles of Large-scale Evidence Generation and Evaluation across a Network of Databases (LEGEND).

Evidence derived from existing health-care data, such as administrative claims and electronic health records, can fill evidence gaps in medicine. However, many claim such data cannot be used to estimate causal treatment effects because of the potential for observational study bias; for example, due to residual confounding. Other concerns include P hacking and publication bias. In response, the Observational Health Data Sciences and Informatics international collaborative launched the Large-scale Evidence Generation and Evaluation across a Network of Databases (LEGEND) research initiative. Its

Cost-effectiveness of technology-assisted buprenorphine for opioid use disorder: Analysis of three randomized trials.

INTRODUCTION: Despite the well-documented efficacy of opioid agonist therapies (e.g., buprenorphine), many individuals with opioid use disorder (OUD) do not receive adequate treatment. Therefore, efforts to expand OUD treatment availability are needed. Prior randomized controlled trials (RCTs) have demonstrated the feasibility and efficacy of technology-assisted buprenorphine (TAB), which utilizes a portable computerized device for medication storage, nightly check-in calls, computer-generated random call-backs and iPad-delivered education for overdose and infectious disease prevention. Allergy alerting and overrides for opioid analogues across two health systems.

OBJECTIVES: Despite opioids comprising a majority of drug allergy alert (DAA) overrides, few studies have designed recommendations for clinical decision support (CDS) systems. We determined the frequency of opioid analogue DAA overrides and assessed DAAs overridden by patient and provider type, the most common allergic reactions documented in electronic health records (EHR), reaction severity, and associated hypersensitivity. METHODS: We conducted an observational cross-sectional study of DAAs in two geographically remote health systems. Patients were included if they were 18 years or older a
