An assessment of US physicians' training in religion, spirituality, and medicine.

This study examined US physicians' training in religion and medicine and its association with addressing religious and spiritual issues in clinical encounters. Reports of receiving training were higher for highly spiritual physicians, psychiatrists, and physicians with high numbers of critically ill patients. Discussing religion or spirituality with patients was associated with having received training through a book or CME literature or during Grand Rounds, through one's religious tradition and from other unspecified sources but not with having received such training in medical school.

A roadmap for education to improve the quality of care in gout.

PURPOSE OF REVIEW: To describe obstacles to optimum management of gout by primary care physicians and to propose educational interventions to improve care. RECENT FINDINGS: In the past, gout education has been hampered by infrequency of continuing medical education courses, loss of excitement for a disease in which therapies have not changed (until recently), insufficient evidence-based medicine, and the lack of motivation by physicians to re-learn this disease once in active practice. We identify 10 common myths that impede appropriate treatment of gout, identify gaps in evidence-based medic

Medication adherence interventions for older adults: literature review.

This literature review explores the range and nature of medication adherence interventions tested with older adults. The unique needs of older adults require specifically designed and tailored interventions. Low medication adherence rates among some elderly contribute to inadequate pharmacological management of illnesses. Searches were conducted to identify randomized controlled trials of medication adherence; computerized databases, journal hand searches, and ancestry searches yielded 63 studies published between 1977 and 2005 where participants' mean age was > 60 years. Interventions were ca

Playing patient, playing doctor: Munchausen syndrome, clinical S/M, and ruptures of medical power.

This article deploys sadomasochism as a framework for understanding medical practice on an institutional level. By examining the case of the factitious illness Munchausen syndrome, this article analyzes the operations of power in the doctor-patient relationship through the trope of role-playing. Because Munchausen syndrome causes a disruption to the dyadic relationship between physicians and patients, a lens of sadomasochism highlights dynamics of power in medical practice that are often obscured in everyday practice. Specifically, this article illustrates how classification and diagnosis are

Can there be a 'cosmetic' psychopharmacology? Prozac unplugged: the search for an ontologically distinct cosmetic psychopharmacology.

'Cosmetic psychopharmacology' is a term coined by Peter Kramer in his 1993 best-seller, Listening to Prozac. It has come to refer to the use of psychoactive substances to effect changes in function for conditions that are either normal or subclinical variants. In this paper, I ask: What distinguishes an existential ailment from clinical depression, or either of those from normal depressed mood, melancholic temperament, dysthymia or other depressive disorders? Can we reliably distinguish one from the other? Are the boundaries of illness and disorder really so distinct? If not, how can we know t

Spina bifida clinic--organisational aspects.

Spina bifida is a complicated problem which affects many systems and demands multidisciplinary attention. The psychosocial aspects of a condition like spina bifida also need to be carefully handled. The ultimate goal is prevention, requiring co-ordinated efforts to establish antenatal diagnosis and screening tests for this anomaly. In our country, the task of solving the difficulties faced by these children at home and in school also falls on our shoulders. All the above responsibilities can only be undertaken if a group of doctors, nurses, social workers, administrators and philanthropists ar

Shared Decision-Making for Patients Hospitalized with Acute Myocardial Infarction: A Randomized Trial.

OBJECTIVE: Adherence to guideline-recommended medications after acute myocardial infarction (AMI) is suboptimal. Patient fidelity to treatment regimens may be related to their knowledge of the risk of death following AMI, the pros and cons of medications, and to their involvement in treatment decisions. Shared decision-making may improve both patients' knowledge and involvement in treatment decisions. METHODS: In a pilot trial, patients hospitalized with AMI were randomized to the use of the AMI Choice conversation tool or to usual care. AMI Choice includes a pictogram of the patient's estima
