Ultrasound-guided stellate ganglion blockade - patient positioning is everything: a case report demonstrating the efficacy of a modified out-of-plane approach.

BACKGROUND: Insomnia has become increasingly prevalent in modern society and is notoriously difficult to treat. Many patients exhibit a poor response to pharmacological interventions. Stellate ganglion block (SGB) has emerged as an effective method for managing insomnia; however, its efficacy may be compromised in some patients, primarily due to a variant vertebral artery anatomy. CASE PRESENTATION: This case report describes a patient with severe insomnia accompanied by anxiety. Through cervical ultrasound scanning, we identified richly branched cervical arteries at the C6-C7 segment of the

Mal-Positioning of Dialysis Catheter in Anomalous Left Superior Pulmonary Vein in a Patient with Acute Type A Dissection, a Case Report.

The partial anomalous pulmonary vein drainage is a rare congenital defect. The pulmonary vein drains in to a systemic vein instead of draining in to the left atrium. In this rare birth defect, the right sided pulmonary vein involvement is more prevalent than the left sided pulmonary veins. We present a case where the anomalous left superior pulmonary vein was diagnosed when a renal dialysis catheter (size = 12F x 16cm) was mal-positioned in to the Anomalous left superior pulmonary vein, demonstrating confusing blood results. We describe how a systematic multidisciplinary approach and use of ad

Inter-fraction target and OARs reproducibility in cervical cancer: Influence of immobilization systems and patient positioning verified by daily fan beam CT.

INTRODUCTION: To assess radiotherapy plans quality, the reproducibility of the targets and organs at risk (OARs) in cervical cancer radiotherapy with different immobilizations and positioning. METHODS: Sixty definitive cervical cancer patients were enrolled and immobilized with the Orfit Rack (prone), the Personalized Prone Plate (PPP) (prone), and the vacuum bag (VB) (supine), respectively. Dosimetric metrics and daily reproducibility of the targets and OARs on fan beam CT (FBCT) were analyzed. RESULTS: The Orfit Rack and PPP (prone position) provided slight advantage in OARs protection. Th

[Radiotherapy in HIV positive patients].

HIV-positive Patients often suffer from the symptoms of accompanying diseases. Palliative radiation therapy of associated tumors leads to an improvement of the patient's condition. Particularly skin tumors, which give rise to pronounced itching and ulcerating, are eliminated fast and safe by radiation therapy. Between 1984 and 1988, 6 HIV patients with Kaposi's sarcoma at different sites, and one HIV-patient with non-Hodgkin's lymphoma were treated by radiation therapy. Depending on tumor site, photons or fast electrons were used. Cosmetic results were satisfying or even excellent in all patie

Spirochaetemia in a HIV positive patient.

BACKGROUND: Borreliosis, caused by Borrelia recurrentis and several other Borrelia species is not a commonly reported case in our environment, but the search for the cause of recurrent pyrexia in this patient made it possible to discover the spirochete as the cause of the disease. METHOD: A 38 year old married HIV positive woman presented with recurrent fever in a private clinic. Six thin smears were made out of the patient serum and dried in the air. Three slides each were stained with 0.12% Leishman and 20% Giemsa stains and examined under the light microscope. RESULT: Three of the Giemsa

[Atherosclerosis in HIV-positive patients].

Worldwide, human immunodeficiency virus (HIV) infection is one of the main health subjects. Even, the human immunodeficiency virus primarily effects the immune system, HIV infection also has an impact on other organs. Cardiovascular manifestations in HIV-infected patients could occur by the HI-virus itself or by opportunistic infections. Reports of myocardial infarction in young HIV-infected patients, who received protease inhibitors, have raised concerns about premature arteriosclerosis and coronary artery disease in this population. In the pre-protease inhibitor era, autopsy reports were the

[Cryptosporidiosis in HIV-positive patients].

Cryptosporidium parvum is a coccidian parasite that affects millions of people worldwide. Clinical outcome of human cryptosporidiosis differs between immunocompetent and immnunodeficient individuals. C. parvum is responsible for causing protracted and life-threatening diarrhea, biliary, and pulmonary infections in immunocompromised persons, especially in patients with AIDS. Though no effective treatment has been found so far, early diagnosis may be useful in controlling the infection. Thirty-eight stool specimens obtained from 35 HIV-positive patients admitted to the Clinic of Infectious Disea

Tuberculosis in the HIV-positive patient.

HIV infection is the most potent risk factor known for tuberculosis, which results both from reactivation and progression from recent infection. We review the epidemiology, clinical features, differential diagnosis and management of HIV-associated tuberculosis, probably the most important opportunistic infection associated with HIV worldwide, and one that is communicable to other HIV-negative persons.

Reliability of ultrasonography measurement of the anterior talofibular ligament (ATFL) length in healthy subjects (in vivo), based on examiner experience and patient positioning.

BACKGROUND: The most common cause of ankle injury is the supination trauma, inflicting a partial or complete rupture of the anterior talofibular ligament (ATFL). Among conventional diagnostic tools and procedures of sports injuries, the method of stress-ultrasonography is reportedly a promising diagnostic tool for examining injuries of the lateral ligaments of the ankle. Preceding studies predominantly examined the comparability of stress-ultrasonography and other established diagnostic tools in terms of efficacy, viability and quality. The purpose of this study was to assess the reliability o

Positioning patients for spine surgery: Avoiding uncommon position-related complications.

Positioning patients for spine surgery is pivotal for optimal operating conditions and operative-site exposure. During spine surgery, patients are placed in positions that are not physiologic and may lead to complications. Perioperative peripheral nerve injury (PPNI) and postoperative visual loss (POVL) are rare complications related to patient positioning during spine surgery that result in significant patient disability and functional loss. PPNI is usually due to stretch or compression of the peripheral nerve. PPNI may present as a brachial plexus injury or as an isolated injury of single ne

Clinical significance of antiproteinase 3 antibody positivity in cANCA-positive patients.

We addressed the clinical significance of antiproteinase 3 (anti-PR3) antibody (Ab) positivity by reviewing the files of 79 patients whose serum contained antineutrophil cytoplasmic antibodies with a cytoplasmic staining pattern (cANCA) and had been tested for anti-PR3 reactivity. Vasculitis was present in most (22/35) cANCA+ PR3+ patients but in only a few (5/44) cANCA+ PR3- patients, thereby suggesting that anti-PR3 Ab positivity in cANCA+ patients is more indicative of vasculitis than cANCA positivity alone. Noteworthy, one-third of cANCA+ PR3+ patients -- those with anti-PR3 Ab titres lowe
