PURPOSE: The wide-awake local anesthesia no tourniquet (WALANT) approach has been proposed as a promising alternative to general anesthesia (GA) for hand surgery. However, evidence in the pediatric population remains limited. The objective of this study was to evaluate its impact on clinical outcomes, procedural efficiency, and institutional costs for pediatric trigger thumb (PTT) release.
METHODS: A retrospective cohort study was conducted on all patients (≤18 years) who underwent open PTT release by a single surgeon at a tertiary pediatric hospital between August 2020 and May 2025. Patients

BACKGROUND: Children's surgery is increasingly recognized as a global health priority, yet pediatric anesthesia remains comparatively under-recognized despite being essential for safe surgical and perioperative care.
THE PROBLEM: Children require specialized expertise, age-appropriate equipment, reliable monitoring, oxygen, medicines, recovery, and critical care. In many low- and middle-income countries, deficiencies across these domains limit access and contribute to preventable perioperative morbidity and mortality.
THE CENTRAL CLAIM: We posit that children's surgical systems cannot be str

BACKGROUND: Timely surgical correction of pediatric trigger thumb can be delayed due to the perceived risks of general anesthesia. This study introduces 2 non-general anesthesia protocols, sedation-assisted local anesthesia (SALA), and audiovisual distraction-assisted local anesthesia (ADALA) for pediatric trigger thumb release.
METHODS: Between January 2015 and December 2024, a total of 296 trigger thumb releases were performed in 249 children under 12 years of age. The choice between SALA and ADALA was based on each child's ability to cooperate at the time of surgery. Sedation was achieved

Even experienced anesthesiologists can be challenged when establishing a central venous catheter in children. In addition to the classical indications, a difficult peripheral vein access represents a relevant indication for a central venous catheter to avoid repeated traumatic puncture attempts. The size of the catheter utilized depends on the body weight and the vein diameter to prevent thrombotic complications. Irrespective of the chosen vein, the puncture should be performed under direct sonographic visualization of the needle tip and subsequently the wire. The internal jugular vein is the

Langerhans cell histiocytosis (LCH) is an uncommon condition that often calls for input from several specialists, especially when the mouth is involved or when treatment affects the child's immune system. This report outlines the care of a five-year-old female with multisystem disease who developed fever and eye irritation during chemotherapy. Once her blood counts recovered, she received full dental rehabilitation under general anesthesia because of extensive decay and limited cooperation. Close coordination among oncology, dental, pediatric, and eye-care teams supported her recovery and high

Pediatric anesthesia presents unique challenges due to children's distinct physiological and anatomical characteristics, including variations in drug metabolism, airway structure, and respiratory and circulatory regulation. Despite significant advances in patient safety that have reduced anesthesia-related mortality over recent decades, the declining pediatric population has made specialized training and clinical practice increasingly difficult. This narrative review addresses practical aspects of pediatric anesthesia, emphasizing patient monitoring, airway management, and recent clinical adva

INTRODUCTION: Recently, concerns have been raised regarding the impact of anesthetic choice on outcomes for patients undergoing hypospadias repair. It has been suggested that caudal epidural anesthesia (CEA) may be associated with increased postoperative complications, such as urethrocutaneous fistula and glanular dehiscence. However, subsequent studies have found no association between CEA and increased complications. Spinal anesthesia (SA) is another technique that can provide surgical blockade during hypospadias repair. However, no studies have examined outcomes for pediatric patients under

OBJECTIVES: Emergence agitation (EA) is a common and troublesome problem in pediatric patients recovering from general anesthesia. The incidence of EA is reportedly higher after general anesthesia maintained with sevoflurane, a popular inhalational anesthetic agent for pediatric patients. We conducted this prospective, randomized, double-blind study to test the effect of an intravenous ultra-short-acting barbiturate, thiamylal, administered during induction of general anesthesia on the incidence and severity of EA in pediatric patients recovering from Sevoflurane anesthesia.
METHODS: Fifty-fo
