Novel Ureteral Stent Catheterization Technique for Treating Hyperchloremic Metabolic Acidosis After Total Pelvic Exenteration.

BACKGROUND/AIM: Hyperchloremic metabolic acidosis after total pelvic exenteration (TPE) is relatively rare. Urinary diversion of the ileal conduit during TPE can result in increased urine reabsorption leading to hyperchloremic metabolic acidosis. We developed a new technique for the retrograde catheterization of a ureteral stent into an ileal conduit to treat hyperchloremic metabolic acidosis. CASE REPORT: A 70-year-old man underwent TPE for locally recurrent rectal cancer. Multiple episodes of complications, such as hyperchloremia and metabolic acidosis, occurred. Effective drainage of urine

Factors Predictive of 90-Day Morbidity, Readmission, and Costs in Patients Undergoing Pelvic Exenteration.

OBJECTIVE: Pelvic exenteration for recurrent gynecological malignancies is characterized by a high rate of severe complications. Factors predictive of morbidity, readmission, and cost were analyzed. METHODS: Data of consecutive patients undergoing pelvic exenteration between January 2007 and December 2016 were prospectively evaluated. RESULTS: Fifty-eight patients were included in the analysis. Anterior, posterior, and total exenterations were executed in 39 (67%), 9 (16%), and 10 (17%) patients, respectively. Ten (15.5%) severe complications occurred: 8 (20.5%), 0 (0%), and 1 (10%) after an

Pubic Bone Excision and Perineal Urethrectomy for Radical Anterior Compartment Excision During Pelvic Exenteration.

BACKGROUND: Malignant infiltration of the pubic bone traditionally is considered inoperable. Consequently, there is little published on surgical approaches to resection of the anterior pelvic bone. En bloc partial or complete pubic bone excision can be performed depending on the degree of involvement. OBJECTIVE: This article describes our surgical approach of pelvic exenteration with en bloc composite pubic bone excision. DESIGN: The surgical technique describes 2 distinct aspects of the surgery, first, a perineal as opposed to abdominal transection of the urethra, and, second, varying exten

Use of Inflated Foley Catheters to Prevent Early Empty Pelvis Complications Following Pelvic Exenteration.

For most patients with bulky pelvic tumors, pelvic exenteration remains the only curative option. Although initially reported as a palliative procedure, nowadays it is rather performed with curative intent. Once the resectional phase is ended, a large defect will remain at the level of the pelvic diaphragm, predisposing to severe complications which are generically included under the name of empty pelvis syndrome. It has been widely demonstrated that this type of complication is associated with severe mortality, even if the patient is free of any pelvic recurrence. We present the case of a 56-

Reconstruction of total pelvic exenteration defects with rectus abdominus myocutaneous flaps versus primary closure.

BACKGROUND: Total pelvic exenteration (TPE) is reserved for patients with locally invasive and recurrent pelvic malignancies. Complications such as wound infections, dehiscence, hernias, abscesses, and fistulas are common after this procedure. The purpose of this study was to determine whether tissue transfer to the pelvis after TPE decreases wound complications. METHODS: Fifty-three patients who underwent TPE between 2004 and 2010 were reviewed. Two groups were identified, those who underwent pelvic reconstruction with a vertical rectus abdominus myocutaneous flap (n = 17) and those who unde

Preoperative Radiotherapy as a Major Risk Factor for Pelvic Abscess after Abdominoperineal Resection and Total Pelvic Exenteration: A Retrospective Study.

OBJECTIVES: Pelvic abscess, alongside perineal wound infection, is a major postoperative complication after abdominoperineal resection (APR) and total pelvic exenteration (TPE). Because the underlying mechanisms of these two complications differ, separate evaluation is warranted. This study aimed to identify risk factors specifically associated with pelvic abscess. METHODS: We retrospectively analyzed patients who underwent APR or TPE at our institution between December 2004 and August 2025. Patients were divided into abscess and non-abscess groups, and patient characteristics and surgical va

Novel technique with bladder peritoneum to prevent empty pelvic syndrome after laparoscopic pelvic exenteration for gynecologic malignancies: Three case reports.

RATIONALE: Pelvic exenteration (PE) is a radical surgical procedure for treating locally recurrent or uncontrolled pelvic malignancies. The consequent postoperative pelvic dead space presents a challenge to extirpative surgeons. Many methods have been utilized for pelvic floor reconstruction to reduce related postoperative complications, however, none of them have been widely accepted. PATIENT CONCERNS: Here, we report 3 cases of patients who underwent PE. Case 1 was a 36-year-old woman who presented to our hospital with abnormal vaginal bleeding. Case 2 was a 50-year-old woman with recurrenc

Neoadjuvant treatment response and oncologic outcomes after pelvic exenteration for locally advanced rectal cancer.

BACKGROUND: A lack of response to neoadjuvant chemoradiotherapy (nCRT) has been described as a surrogate of aggressive tumor biology leading to poor oncologic outcomes in locally advanced rectal cancer (LARC). However, there is a paucity of data on this association for patients with LARC undergoing pelvic exenteration (PE). METHODS: The National Cancer Database was queried for adults with cT4 rectal adenocarcinoma between 2006 and 2021. The following 3 groups were identified: upfront PE, nCRT with neoadjuvant rectal (NAR) score of <8 followed by PE, and nCRT with an NAR score of ≥8 followed b

Management of enteroperineal fistulas following pelvic exenteration: Insights from a high-volume referral centre.

AIM: Total pelvic exenteration (TPE) has emerged as the standard treatment for advanced anorectal, genitourinary and soft-tissue pelvic tumours. Long-term complications, particularly empty pelvis syndrome (EPS) leading to enteroperineal fistula (EPF), remain inadequately addressed. The surgical management of EPF poses significant challenges, with limited published data and no official guidelines. This study aimed to investigate the incidence and outcomes of EPFs after TPE and to describe the management of reoperative surgery in this patient cohort. METHOD: Patients who underwent TPE at Royal

Combined laparoscopic and transperineal endoscopic total pelvic exenteration for local recurrence of rectal cancer.

BACKGROUND: Transanal minimally invasive surgery is a combination of single-port surgery and transanal surgery and was initially developed as a treatment for rectal tumors. Recently, this approach has also been used for more advanced or extended pelvic surgery. METHODS: We present a surgical video of combined laparoscopic and transperineal endoscopic total pelvic exenteration performed in a male patient with recurrent rectal cancer and discuss the pros and cons of this approach. RESULTS: The operating time was 775 min and the operative blood loss was 485 ml. The pathology was recurrent adeno

Prediction of short-term surgical complications in women undergoing pelvic exenteration for gynecological malignancies.

OBJECTIVE: Pelvic exenteration (PE) is an extensive surgery associated with high rates of postoperative morbidity and mortality. The absence of well-defined preoperative selection criteria to identify patients eligible for PE prompted the assessment of pre-operative predictors of 30-day major surgical complications. METHODS: Demographics and surgical characteristics of patients undergoing PE for gynecologic cancer in a single institution between 01/2004-12/2016 were reviewed. Postoperative complications within 30 days following surgery were graded using the Accordion grading system. Logistic

Does histologic type correlate to outcome after pelvic exenteration for cervical and vaginal cancer?

BACKGROUND: Adenocarcinoma (AC) of the cervix comprises 15-20 % of all cervical carcinomas, and data regarding the prognostic value of histologic type after pelvic exenteration (PE) are lacking. Our aim was to analyze the prognostic value of histologic type in overall survival (OS) and disease-specific survival (DSS) after PE and correlate it to clinical and pathologic variables. METHODS: We reviewed a series of 77 individuals who underwent PE for cervical or vaginal cancer from January 1980 to December 2010. RESULTS: Mean age was 54.5 years. Fifty-three patients (68.9 %) had cervical and 24
