Real-world management and in-hospital outcomes of foreign body-associated esophageal perforations in the United States: a national retrospective cohort study.

BACKGROUND/AIMS: Esophageal perforations can be managed conservatively, endoscopically, or surgically. We aimed to characterize the outcomes of these approaches and assess the impact of intervention timing. METHODS: We retrospectively analyzed hospitalizations in the National Inpatient Sample from 2016 to 2022 for foreign body-associated esophageal perforations. Patients were grouped into endoscopic, surgical, or conservative management. Outcomes included mortality, respiratory failure, sepsis, hemorrhage, parenteral nutrition, and gastrostomy. RESULTS: Among the 2,045 patients, 145 (7.1%) u

Hippocampal subregional texture features associated with Alzheimer's disease severity and cognition.

Alzheimer's disease (AD) exhibits selective vulnerability in hippocampal subfields, where microstructural alterations precede overt atrophy. Conventional morphometric measures, such as volume or thickness, are limited in their ability to capture subtle textural heterogeneity that reflects underlying cytoarchitectural disorganization, particularly perforant path fibres within the subiculum. The objective of this study is to determine whether hippocampal subregional texture-based radiomic features can serve as sensitive markers of disease severity and cognitive impairment along the AD continuum.

Gradual angioplasty and stent implantation to treat complete superior vena cava occlusion after Mustard procedure.

A 16-year-old male was admitted with complete occlusion of the superior vena cava pathway 14 years after Mustard procedure for transposition of the great arteries. From a left subclavian vein approach, the atretic vein segment was perforated using a straight guidewire, and was followed by sequential balloon dilation. Implantation of a 30 mm Palmaz-stent through a femoral vein approach resulted in a widely patent channel of the vena cava superior into the systemic venous atrium.

Pneumomediastinum: a diagnostic problem.

Pneumomediastinum has numerous etiologies and its pathways of spread are multiple and well defined. Knowledge of these anatomic pathways and possible etiologies is important in order to avoid extensive and unnecessary evaluations. For example, if there is a known reason for pneumomediastinum, and pneumoperitoneum is present without associated abdominal findings, further evaluation for perforated viscus is unnecessary. In a patient who is an asthmatic or a diabetic, the presence of pneumomediastinum should not lead to work-up with contrast studies unless there are specific clinical reasons to d

Prenatal morphine exposure suppresses mineralocorticoid receptor-dependent basal synaptic transmission and synaptic plasticity in the lateral perforant path in adult male rats.

The effects of prenatal morphine exposure (E11-18) on mineralocorticoid receptor (MR) modulation of synaptic plasticity were investigated in the lateral perforant path (LPP)-dentate gyrus granule cell synaptic system. Hippocampal slices were prepared from adult, prenatally saline- or morphine-exposed male rats. One hour prior to decapitation, some adult male rats were injected subcutaneously with saline or the MR antagonist, canrenoic acid (50 mg/kg). LPP was stimulated with high-frequency (2x100 Hz/0.5 s) and short-term plasticity (STP) and long-term potentiation (LTP) were evaluated at 5 and

Impaired spatial learning after saturation of long-term potentiation.

If information is stored as activity-driven increases in synaptic weights in the hippocampal formation, saturation of hippocampal long-term potentiation (LTP) should impair learning. Here, rats in which one hippocampus had been lesioned were implanted with a multielectrode stimulating array across and into the angular bundle afferent to the other hippocampus. Repeated cross-bundle tetanization caused cumulative potentiation. Residual synaptic plasticity was assessed by tetanizing a naïve test electrode in the center of the bundle. Spatial learning was disrupted in animals with no residual LTP

Chronic Electroconvulsive Shock Disrupts Hippocampal Physiology and Impairs Associative Memory in Mice.

Electroconvulsive therapy (ECT) is a highly effective treatment for depression but it has undesirable side effects on various aspects of cognition, including memory. Patients may display retrograde amnesia for autobiographical events and anterograde amnesia in tests of recall and recognition memory. These amnestic effects are likely to depend, at least in part, on changes in the hippocampus. ECT-induced volume increases in the hippocampus and dentate gyrus subregion in particular correlate with the cognitive deficits. However, the precise cellular and behavioral effects of ECT remain poorly un

Doxycycline mitigates corneal fibrosis by suppressing the focal adhesion pathway.

The cornea, as a key barrier of intraocular tissues, is vulnerable to various injury factors. Acute alkali burns, as an ophthalmic emergency, can cause serious pathological changes such as persistent corneal epithelial defects, ulcers and even perforations. Doxycycline has been proven to have inhibitory effects on matrix metalloproteinases and anti-inflammatory effects, etc. This study focuses on the therapeutic value of doxycycline in corneal alkali burns. In vitro experiments have shown that it can significantly inhibit the expression of Fibronectin and Collagen III by regulating the Focal a

Varicose veins due to reflux in a tortuous sartorius muscle vein.

In daily phlebology practice, duplex ultrasound may sometimes reveal unusual pathways of reflux. A 46-year-old woman presented with heaviness and pain at the medial side of the left thigh after exercise. A tortuous intramuscular course of a large refluxing vein was seen in the sartorius muscle in connection with a femoral vein perforator cranially and the below-knee great saphenous vein distally. Magnetic resonance imaging confirmed these findings. Treatment consisted of a combination of foam sclerotherapy, radiofrequency ablation, and phlebectomies.

Hip sepsis from retroperitoneal rupture of diverticular disease.

Retroperitoneal perforation of diverticular disease is very uncommon and can be difficult to diagnose because of possible pathways of communication between the retroperitoneal space and the thigh. An iliopsoas abscess from a ruptured diverticulum may drain into the hip joint if the capsule has been violated. Cases of abscesses or gas in the thigh have been reported in which thigh pain was the predominant symptom, overshadowing any abdominal signs. We report a case in which hip sepsis due to a ruptured diverticulum was the presenting feature.

The possible mechanism of Hippophae fructus oil applied in tympanic membrane repair identified based on network pharmacology and molecular docking.

OBJECTIVE: This study aimed to explore the mechanisms of Hippophae fructus oil (HFO) in the treatment of tympanic membrane (TM) perforation through network pharmacology-based identification. METHODS: The compounds and related targets of HFO were extracted from the TCMSP database, and disease information was obtained from the OMIM, GeneCards, PharmGkb, TTD, and DrugBank databases. A Venn diagram was generated to show the common targets of HFO and TM, and GO and KEGG analyses were performed to explore the potential biological processes and signaling pathways. Pathways for the rehabilitation of resection defects in the maxilla.

Oral rehabilitation of head and neck cancer patients is an integral component of the care pathway. Maxillectomy procedures can cause significant defects, such as oronasal fistulas, loss of support for the cheek and lip, aesthetic defects in the middle third of the face and functional impairments. Orofacial rehabilitation plays a fundamental role in restoring aesthetics and functional capabilities, such as speech, mastication and deglutition.Rehabilitation of maxillectomy patients poses a challenge for both clinicians and patients. This paper utilises case examples to demonstrate the treatment

Novel Presentation of Terminal Ileitis Associated with Secukinumab Therapy.

Inflammatory bowel disease (IBD) and psoriasis are chronic inflammatory immune-mediated diseases. The interleukin-23- (IL23-) T helper (Th)17 pathway has been implicated in their pathogenesis, with multiple biologic therapies targeting this pathway. IL-17, the main proinflammatory cytokine produced by (TH)17, has been targeted by antibodies and IL-17 receptor blockers with favorable outcomes in treating psoriasis and psoriatic arthritis. However, their role in IBD is unpredictable as studies reported worsening of IBD with agents targeting IL-17 and rare case reports with new-onset IBD. We pres

Microsurgical treatment of lens capsule perforations--Part II: Clinical applications and results.

A microsurgical technique using human fibrinogen concentrate for the closure of anterior lens capsule wounds and through-and-through perforations was developed, experimentally tested and clinically applied. I consider here the first consecutive 31 patients treated. All these patients had perforating injuries of the lens and still localized lens damage; they showed no spontaneous healing tendencies. Successful treatment of lens capsule wounds, where an otherwise clear lens is locally scarred, has been rare. The technique described here was effective in more than 50% of the patients with anterio

Hedgehog activity controls opening of the primary mouth.

To feed or breathe, the oral opening must connect with the gut. The foregut and oral tissues converge at the primary mouth, forming the buccopharyngeal membrane (BPM), a bilayer epithelium. Failure to form the opening between gut and mouth has significant ramifications, and many craniofacial disorders have been associated with defects in this process. Oral perforation is characterized by dissolution of the BPM, but little is known about this process. In humans, failure to form a continuous mouth opening is associated with mutations in Hedgehog (Hh) pathway members; however, the role of Hh in p

Unusual causes of varicose veins in the lower extremities: CT venographic and Doppler US findings.

There are various causes of varicose veins in the lower extremities. Among the causes are venous insufficiency of the saphenofemoral junction, saphenopopliteal junction, or usual perforating veins. Traditionally, Doppler ultrasonography (US) has been used for evaluation of varicose veins. Sometimes, varicose veins arise from an unexpected anatomic source; in these cases, computed tomographic (CT) venography can provide an overview of the varicose veins. Doppler US with complementary CT venography is useful for determining the precise cause of varicose veins. Between November 2003 and March 200

TachoSil Dural Reconstruction in Extracranial-Intracranial Bypass Surgeries.

BACKGROUND: Superficial temporal artery to middle cerebral artery (STA-MCA) bypass is a treatment option for hemodynamic insufficiency in the anterior cerebral circulation. Complications associated with extracranial-intracranial bypass surgeries are ischemic strokes caused by bypass failure, wound-healing disorders, and further issues from cerebrospinal fluid (CSF) leakage. CSF leakage can provide pathways for infection. It is well known in general neurosurgery that watertight closure of the dura mater is necessary to prevent such complications. OBJECTIVE: To provide a technical description o
