Observer Agreement on Computed Tomography Perfusion Imaging in Acute Ischemic Stroke.

Background and Purpose- Computed tomography (CT) perfusion (CTP) provides potentially valuable information to guide treatment decisions in acute stroke. Assessment of interobserver reliability of CTP has, however, been limited to small, mostly single center studies. We performed a large, internet-based study to assess observer reliability of CTP interpretation in acute stroke. Methods- We selected 24 cases from the IST-3 (Third International Stroke Trial), ATTEST (Alteplase Versus Tenecteplase for Thrombolysis After Ischaemic Stroke), and POSH (Post Stroke Hyperglycaemia) studies to illustrate

Indocyanine green fluorescence imaging to evaluate graft perfusion during liver transplantation.

BACKGROUND: Primary graft dysfunction (PGD) is a leading cause of graft loss after liver transplantation. There is no reliable method to anticipate this complication intraoperatively. Indocyanine green (ICG) fluorescence imaging is a technique used in hepatobiliary surgery for detection of liver malignancies, but has never been reported in the setting of liver transplantation (LT) for function assessment. We hypothesized that there could be an association between the type of fluorescence and the occurrence of PGD. METHODS: We retrospectively analyzed 72 patients who underwent LT at our center

Results of Serial Myocardial Perfusion Imaging in End-Stage Renal Disease.

For patients awaiting renal transplantation, there is guideline consensus on the need for ischemia testing but no agreement on the frequency of repeat testing. Moreover, there are no data in this population evaluating changes in ischemia assessed with serial myocardial perfusion imaging. Consecutive patients (n = 649) with end-stage renal disease (ESRD) were referred for cardiovascular risk stratification before renal transplantation between 2007 and 2013. Of these, 151 patients (54 ± 9 years) underwent 2 stress-rest technetium-99m single-photon emission computed tomographic (SPECT) studies wi

Cerebral Misery Perfusion on Susceptibility Weighted Imaging in Acute Carotid Dissection.

The cerebral vasculature incorporates several fail-safes that must be breached before an irreversible ischemic event takes place. In particular, when autoregulatory vasodilatation fails secondary to falling cerebral perfusion pressure (CPP; stage I hemodynamic failure), increases in the oxygen extraction fraction work to maintain the cerebral metabolic rate of oxygen. Previously, failure of this mechanism, stage II hemodynamic failure, or misery perfusion, has been imaged via positron emission tomography/computed tomography (PET/CT). Current susceptibility-weighted sequences (SWI) allow for mo

Contrast-Enhanced Ultrasound Imaging in Detection of Changes in Cerebral Perfusion.

Contrast-enhanced ultrasonography (CEU) is a non-invasive imaging technique that provides real-time, bedside information on changes in global and segmental organ perfusion. Currently, there is a lack of data concerning changes in the distribution of segmental brain perfusion in acute ischemic stroke treated by decompressive craniectomy. The aim of our case series was to assess the role of CEU after decompressive craniectomy in patients with acute ischemic stroke. CEU was performed in 12 patients at baseline and after any one of the following interventions was performed as dictated by the patie

Brain perfusion SPECT imaging and acetazolamide challenge in vascular cognitive impairment.

Cerebrovascular disease is recognized as a common cause of cognitive impairment and dementia, alone or coexisting with other neurodegenerative diseases, mostly Alzheimer's disease. Vascular cognitive impairment (VCI) is a part of the heterogenous disorders group related to cerebral vessel disease. Although age is one of the most important risk factors for VCI, other common cardiovascular risk factors are also involved. By investigating these risk factors, a high proportion of these cognitive disorders can be prevented and/or delayed. Until now, only treatment of midlife arterial hypertension h

Success of arterial revascularization determined by contrast ultrasound muscle perfusion imaging.

BACKGROUND: In the early postoperative evaluation of the success of arterial revascularization, ankle-brachial index (ABI) and other noninvasive tests lack reliability, especially in patients with incompressible arteries or local edema. Contrast-enhanced ultrasound (CEUS) imaging of limb muscle perfusion may be an alternative to standard tests if it detects treatment success reliably. METHODS: We compared a simplified CEUS method with clinical staging, pulse volume recording (PVR), and ABI in patients with lifestyle-limiting peripheral arterial disease undergoing revascularization by percutan

Kana Pick-out Test and brain perfusion imaging in Alzheimer's disease.

BACKGROUND: The Kana Pick-out Test (KPT), which was developed in Japan, is suitable for evaluating frontal lobe function and screening for mild dementia. However, the neural substrates involved remain to be elucidated. The aim of the present study was to identify the regional perfusion patterns in the brain associated with performance scores on the KPT in patients with mild Alzheimer's disease (AD), using brain perfusion assessed by single photon emission computed tomography (SPECT). METHODS: Twenty AD patients with high scores on the KPT and 20 age- and sex-matched AD patients with low score

Collateral circulation imaging: MR perfusion territory arterial spin-labeling at 3T.

BACKGROUND AND PURPOSE: Current knowledge of the collateral circulation remains sparse, and a noninvasive method to better characterize the role of collaterals is desirable. The aim of our study was to investigate the presence and distal flow of collaterals by using a new MR perfusion territory imaging, vessel-encoded arterial spin-labeling (VE-ASL). MATERIALS AND METHODS: Fifty-six patients with internal carotid artery (ICA) or middle cerebral artery (MCA) stenosis were identified by sonography. VE-ASL was performed to assess the presence and function of collateral flow. The perfusion inform

Yield of combined perfusion and diffusion MR imaging in hemispheric TIA.

OBJECTIVE: Transient ischemic attacks (TIA) predict future stroke. However, there are no sensitive and specific diagnostic criteria for TIA and interobserver agreement regarding the diagnosis is poor. Diffusion-weighted MRI (DWI) demonstrates acute ischemic lesions in approximately 30% of TIA patients; the yield of perfusion-weighted MRI (PWI) is unclear. METHODS: We prospectively performed both DWI and PWI within 48 hours of symptom onset in consecutive patients admitted with suspected hemispheric TIAs of <24 hours symptom duration. Two independent raters, blinded to clinical features, asses

Hemodynamic studies on brain CT perfusion imaging with varied injection rates.

OBJECTIVE: This study aimed to determine a contrast medium injection rate that ensures both accuracy for data and safety for operation by comparing hemodynamic parameters of brain CT perfusion imaging with varied injection rates. METHODS: Twenty-four healthy volunteers were divided into three groups based on contrast medium injection rates (4.5, 6.0, and 7.5 ml/s). For all subjects, CT perfusion scanning was started at 4 s after antecubital venous bolus of contrast media injection. A perfusion-analyzing software package was used to produce a time-density curve in the anterior cerebral artery

Diffusion and perfusion-weighted magnetic resonance imaging techniques in stroke recovery.

Most of the functional recovery after stroke takes place during the first three months after the insult. The neuronal mechanisms underlying this recovery are presently mostly unknown. However, in order to create efficient rehabilitation programs, it is of great importance to uncover these mechanisms. Multiple imaging techniques have been employed for the detection and characterization of ischemic lesions in the brain as well as monitoring of processes associated with stroke recovery. Diffusion and perfusion-weighted magnetic resonance imaging techniques are easy and fast to perform and provide

Attenuation correction for single photon emission computed tomography myocardial perfusion imaging.

The specificity of cardiac single photon emission computed tomography (SPECT) perfusion imaging is significantly affected by internal photon absorption. Commonly referred to as anterior wall breast and inferior wall diaphragm attenuation artifacts, even when following characteristic patterns in women and men, the reduced activity produced can be difficult to differentiate from real perfusion defects. Unfortunately, wide variations in body habitus result in unpredictable variations in tissue attenuation and the specificity of uncorrected SPECT is unacceptably low in many laboratories. This manu

Assessment of transient ischemic attack with diffusion- and perfusion-weighted imaging.

BACKGROUND: Diagnosing TIA can be difficult, since evidence of brain ischemia is habitually lacking on CT and conventional MR imaging. It has been suggested that patients with acute brain infarction on neuroimaging should be considered stroke cases instead of TIA, regardless of duration of symptoms, implying that optimal diagnostic methods need to be utilized. We therefore postulated that perfusion-weighted MR imaging (PW imaging) would be useful in the diagnosis of TIA. METHODS: Retrospective analysis of 22 patients with reversible neurologic symptoms lasting less than 24 hours, assessed wit

Perfusion-weighted magnetic resonance imaging thresholds identifying core, irreversibly infarcted tissue.

BACKGROUND AND PURPOSE: Identifying core, irreversibly infarcted tissue and salvageable penumbral tissue is crucial to informed, physiologically guided decision making regarding thrombolytic and other interventional therapies in acute ischemic stroke. Pretreatment perfusion MRI offers promise as a means to differentiate core from penumbral tissues. METHODS: Diffusion-perfusion MRIs were performed before treatment and on day 7 in patients undergoing successful vessel recanalization with intra-arterial thrombolytic therapy. Perfusion maps of the time to peak of the residue function (Tmax) were

Tc-99m HMPAO brain perfusion imaging in young Down's syndrome patients.

Down's syndrome (DS) is characterized by moderate mental retardation and a variety of abnormalities involving multiple organ systems. There is a high incidence of Alzheimer's disease (AD) type dementia beyond the age of 35. In this study, single photon emission computed tomography (SPECT) brain perfusion imaging of young Down's syndrome patients was performed to define the perfusion pattern. Tc-99m HMPAO brain perfusion SPECT was performed on 17 young DS patients, aged 3-24 years (mean: 10.9+/-5.9 years). None of the patients had dementia symptoms. Brain perfusion scans were acquired 15 min af

Cerebral perfusion imaging in asymptomatic carotid artery occlusion following gun shot.

Recognition of brain injury in asymptomatic carotid artery injuries with conventional methods can be difficult. We present a case of angiographically proven asymptomatic left internal carotid artery occlusion with normal CT after a gunshot wound. The SPECT brain perfusion imaging showed mild generalized supratentorial hypoperfusion of the bilateral cerebral cortices on the left side and severe left temporal lobe hypoperfusion.
