Constrictive pericarditis: 21 years' experience and review of literature.

To the best of our knowledge there are no publications about Tunisian experience in constrictive pericarditis (CP); the aim of this study was therefore to review our twenty-one years' experience in terms of clinical and surgical outcomes and risk factors of death after pericardiectomy. An analytic bicentric and retrospective study carried out on 25 patients (20 male) with CP underwent pericardiectomy, collected over a 21-years period. The mean age was 40.46±16.74 years [7.5-72]. The commonest comorbid factor was tabagism (52%). The most common etiology was tuberculosis (n = 11, 44%). Dyspnea w

Surgical management of pericarditis in Zaria, Nigeria.

Over eight years, 58 rural Nigerians with pericarditis were treated surgically in Zaria using basic surgical facilities. Eighteen patients had purulent pericarditis, associated with staphylococcal pneumonia in children, or pneumococcal pneumonia in adults. Treatment with antibiotics and prompt pericardiectomy appeared to be superior to drainage, since a quarter of those initially treated with surgical drainage developed early constriction and required pericardiectomy soon after. Thirteen patients had chronic pericardial effusions, of whom one had epicardial constriction and two had cardiomyopa

[Surgical treatment of chronic constrictive pericarditis due to old tuberculosis; report of a case].

We experienced a case of pericardiectomy to treat constrictive pericarditis due to old tuberculosis. A 40-year-old woman was admitted to our hospital with dyspnea on exertion and edema of both legs. The chest computed tomography showed calcification of anterior ventricles. They were tightened up by calcified band. Subtotal pericardiectomy and removal of the calcification were performed without cardiopulmonary bypass. After the operation, symptoms were not disappeared in early phase. However, improvement for the patient was confirmed 3 months later.

A case of recurrent pericardial constriction presenting with severe pulmonary hypertension.

Chronic constrictive pericarditis (CP) is a relatively rare condition in which the pericardium becomes fibrotic and noncompliant, eventually resulting in heart failure due to impaired ventricular filling. The only curative treatment is pericardiectomy. Classically, CP does not usually cause severe pulmonary hypertension. When attempting to differentiate CP from restrictive cardiomyopathy, the presence of severely elevated pulmonary arterial pressure is used as a diagnostic criterion ruling against CP. We present a case of proven recurrent pericardial constriction following pericardiectomy pres

Constrictive pericarditis associated with combined idiopathic retroperitoneal and mediastinal fibrosis.

Idiopathic retroperitoneal fibrosis and mediastinal fibrosis are localized expressions of a systemic sclerosing disease of unknown cause which, on rare occasions, may coexist in the same patient. Although pericardial involvement may occur, recurrent constrictive pericarditis that is unrelieved by pericardiectomy has not previously been reported in association with either idiopathic retroperitoneal or mediastinal fibrosis. Reported herein is a case of recurrent constrictive pericarditis that was unrelieved by two pericardiectomies , and autopsy revealed unsuspected combined idiopathic retroperi

Extracardiac malignancy presenting with cardiac tamponade.

The clinicopathologic features of a case of cardiac tamponade as the initial presentation of extracardiac malignancy is reported and compared with those of previously reported cases. The lung was the site of the primary tumor in 58% of the cases. Irrespective of the sites of the primary tumor, mediastinal involvement or intrapulmonary metastases or both were documented in 88% of the cases. Examinations of the pericardial fluid and pericardiectomy specimen were negative in a considerable number of cases. Therefore, careful examination of the mediastinum and the accessible lung tissue at the tim

Clinical review: intrapericardial fibrinolysis in management of purulent pericarditis.

Purulent pericarditis (PP) is a potentially life-threatening disease. Reported mortality rates are between 20 and 30%. Constrictive pericarditis occurs over the course of PP in at least 3.5% of cases. The frequency of persistent PP (chronic or recurrent purulent pericardial effusion occurring despite drainage and adequate antibiotherapy) is unknown because this entity was not previously classified as a complication of PP. No consensus exists on the optimal management of PP. Nevertheless, the cornerstone of PP management is complete eradication of the focus of infection. In retrospective studie

Pericardial Constriction and Myocardial Restriction in Pediatric Mulibrey Nanism: A Complex Disease With Diastolic Dysfunction.

BACKGROUND: Mulibrey nanism (MUL) is a rare condition with profound growth delay. Congestive heart failure is a major determinant of prognosis. The aim was to delineate pericardial constriction and myocardial functional abnormalities in a pediatric MUL sample. METHODS: A total of 23 MUL patients and 23 individually sex- and age-matched healthy control subjects were prospectively assessed in a cross-sectional study with echocardiography. RESULTS: Clinical signs of heart failure were present in 7 MUL patients, with severe congestive heart failure in 2. Significant diastolic dysfunction, mainly

Hemorrhagic pericardial effusion resulting in constriction in hereditary hemorrhagic telangiectasia.

BACKGROUND: We report the first ante-mortem diagnosis of hemorrhagic pericardial effusion in hereditary hemorrhagic telangiectasia resulting in constriction; the case also demonstrates the unusual but well-described complication of right-sided heart failure requiring extracorporeal membrane oxygenation (ECMO) support after pericardiectomy. CASE PRESENTATION: A previously healthy 48 year old man with a strong family history of Osler-Weber-Rendu disease presented to our institution with signs and symptoms of advance heart failure. His workup demonstrated a thickened pericardium and constrictive

Pericardial Hemangiosarcoma in a 10-Year-Old Papillon.

A 10 yr old papillon was evaluated for lethargy, inappetence, tachypnea, and labored breathing. Physical examination findings included dehydration, tachypnea, tachycardia, and muffled heart sounds. Thoracic radiographs revealed an enlarged cardiac silhouette. Echocardiography revealed a mild volume of pericardial effusion and no evidence of right atrial tamponade or a cardiac-associated mass. Cytological analysis of the pericardial effusion was consistent with blood. There was no evidence of neoplasia. Coagulation parameters were within normal limits. Initial treatment consisted of pericardioc

Surgical management of effusive constrictive pericarditis.

OBJECTIVE: The surgical approach for effusive constrictive pericarditis (ECP) has not been extensively studied. We present our institution's early and long-term results of pericardiectomy in our cohort of patients with ECP. METHODS: Diagnosis was made primarily by echocardiography. Right heart catheterisation was performed in eight patients. Pre-operatively, 10 patients had undergone at least one previous attempt at therapeutic pericardiocentesis. Pericardiectomy was performed where appropriate (thickened or inflamed). RESULTS: Of our 12 patients (50% male, median age 48 years, range 17-72 y

Perioperative Continuation of Interleukin-1 Blockade in Chronic Constrictive Pericarditis: Imaging-Guided Surgical Decision.

BACKGROUND: Interleukin (IL)-1 inhibitors have transformed pericarditis management by controlling inflammation and preventing relapse. Their use around pericardiectomy in patients with fixed constriction remains largely unreported. CASE SUMMARY: A 42-year-old man with a long-standing inflammatory pericardial syndrome, consistent with incessant pericarditis, developed right-sided heart failure despite long-term anakinra, prednisone, and colchicine, suggestive of evolution to constrictive pericarditis. Echocardiography and cardiac magnetic resonance showed constrictive physiology with pericardi

Prognostic nutritional index (PNI) for predicting perioperative complications after tuberculous constrictive pericarditis surgery: a single-center retrospective study.

BACKGROUND: Perioperative complications following pericardiectomy in patients with constrictive pericarditis can significantly affect cardiac function recovery and postoperative outcomes. The prognostic nutritional index (PNI), a well-established nutritional marker, has been shown to predict outcomes in various diseases. However, its role as a predictive factor in patients with tuberculous constrictive pericarditis undergoing pericardiectomy remains unclear. This study aimed to evaluate the association between preoperative PNI and adverse perioperative outcomes in this patient population. MET

Cardiac tamponade secondary to intrapericardial rupture of a hepatic amoebic abscess.

We report the case of a patient who underwent an emergency subxiphoid pericardiectomy with evacuation of 1700 cc of anchovy sauce-like fluid. On pericardial inspection a fistulous communication was seen with a cavity in the left hepatic lobe. No loculi or adhesions were found in the pericardial sac and no further surgical procedure was advised. The hepatic cavity and pericardium were drained: bacteriological investigation of the fluid yielded entamoeba histolytica and chloroquine was administered. Apart from the rarity of the observation in western countries, we discuss surgical approaches in

Neovascularization in pericarditis: a possible etiology of hemopericardium.

Hemorrhagic pericardial effusion is life-threatening and mostly occurs during pericarditis; however, its underlying mechanism is unclear. We report a case of pericardial hematoma with obvious hemorrhage. A 56-year-old man without prior chest trauma presenting with exertional dyspnea and abdominal fullness was treated for idiopathic pericarditis. The echocardiogram showed a thickened pericardium with effusion. Imaging showed extravasation and neovascularization. Following ineffective pharmacotherapy, we performed pericardiectomy. The site of bleeding was undetectable. Because parietal pericardi

Surgical treatment of chronic constrictive pericarditis using cardiopulmonary bypass.

The results of operative treatment of 11 patients with chronic constrictive pericarditis with the aid of cardiopulmonary bypass are described. Ten of the 11 patients had Functional Class III or IV disability preoperatively, and all had evidence of severe constrictive disease on physical examination and cardiac catheterization. Pericardiectomy, performed on cardiopulmonary bypass through a median sternotomy incision, required an average total perfusion time of 80 minutes. There were no intraoperative or hospital deaths, and none of the patients required prolonged postoperative hospitalization,

Surgical experience with chronic constrictive pericarditis.

OBJECTIVE: Constrictive pericarditis is a disease characterized by marked thickening of the pericardium which causes restriction of diastolic cardiac function. The purpose of this report is to review the outcome of pericardiectomy for constrictive pericarditis and to discuss its problems. METHODS: Sixteen consecutive patients who underwent pericardiectomy for constrictive pericarditis in our institution between March 2000 and June 2011 were reviewed. All patients underwent decortication including at least the anterior surface from the right atrium to the left phrenic nerve and the diaphragmat

IgG4-Related Pericarditis: Multimodality Imaging Diagnosis and Surgical Outcomes in a Case Series.

BACKGROUND: IgG4-related disease (IgG4-RD) is a systemic fibroinflammatory condition that rarely affects the pericardium. When present, it may manifest as recurrent or constrictive pericarditis, posing diagnostic and therapeutic challenges. CASE SUMMARY: We present 5 patients with biopsy-proven IgG4-RD involving the pericardium (IgG4-RD pericarditis) who developed constrictive physiology. All patients initially received medical therapy, including nonsteroidal anti-inflammatory drugs, colchicine, corticosteroids, and, in some cases, biologics. Despite these interventions, persistent or progres
