Culture positivity and distribution of the conjunctival sac bacteria in the perioperative period of corneal refractive surgery.

The objective of this study was to investigate the culture positivity and distribution of the conjunctival sac bacteria in the perioperative period of corneal refractive surgery. The selected time points of the perioperative period included before the use of antibiotic eye drops, before eye wash (after the use of antibiotic eye drops), after eye wash, and immediately after surgery. Conjunctival specimens obtained at the four time points were cultured to detect the positivity and distribution of bacteria. Before prophylactic antibiotic eye drops were administered, 49 eyes (50%) had positive bac

Kratom and opioid use disorder in the perioperative period: A case report on the expanding role of buprenorphine.

Kratom (Mitragyna speciosa), an herbal plant gaining popularity as a treatment for chronic pain and opioid withdrawal symptoms, continues to be marketed and sold in the United States in an unregulated fashion. We present a patient with a history of opioid use disorder and taking kratom for chronic pain, who presented for an elective total knee replacement. We experienced challenges as well as successes with using buprenorphine as the primary medication to treat his post-operative pain, adding to the growing body of evidence to consider the use of buprenorphine both for kratom withdrawal and in

Importance of assessing biomarkers and physiological parameters of anemia-induced tissue hypoxia in the perioperative period.

Anemia is associated with increased risk of Acute Kidney Injury (AKI), stroke and mortality in perioperative patients. We sought to understand the mechanism(s) by assessing the integrative physiological responses to anemia (kidney, brain), the degrees of anemia-induced tissue hypoxia, and associated biomarkers and physiological parameters. Experimental measurements demonstrate a linear relationship between blood Oxygen Content (CaO2) and renal microvascular PO2 (y = 0.30x + 6.9, r2 = 0.75), demonstrating that renal hypoxia is proportional to the degree of anemia. This defines the kidney as a p

Chinese guideline on the application of anti-seizure medications in the perioperative period of supratentorial craniocerebral surgery.

Seizures are a common symptom of craniocerebral diseases, and epilepsy is one of the comorbidities of craniocerebral diseases. However, how to rationally use anti-seizure medications (ASMs) in the perioperative period of craniocerebral surgery to control or avoid seizures and reduce their associated harm is a problem. The China Association Against Epilepsy (CAAE) united with the Trauma Group of the Chinese Neurosurgery Society, Glioma Professional Committee of the Chinese Anti-Cancer Association, Neuro-Oncology Branch of the Chinese Neuroscience Society, and Neurotraumatic Group of Chinese Tra

Choice of fluid therapy in patients of craniopharyngioma in the perioperative period: A hospital-based preliminary study.

BACKGROUND: Electrolyte imbalance and acute diabetes insipidus (DI) are the most common complications in patients undergoing craniopharyngioma surgery. Improper management of water and electrolyte imbalance is common cause of morbidity and mortality. Data is sparse and controversial regarding the choice of fluid therapy in this population during perioperative period. METHODS: In this retrospective-prospective study involving 73 patients (58 retrospective), the type of fluid therapy was correlated with occurrence of hypernatremia, hyponatremia, DI, morbidity, and mortality. In the retrospectiv

[State of the kinin-regulating function of the lungs in the perioperative period in patients undergoing thoracic surgery].

Kallikrein-kinin system (prekallikrein and kallikrein) has been studied in arterial and mixed venous blood of 22 patients operated on the lungs and mediastinum for suppurative processes and tumors and in 15 patients operated on for ischemic heart disease (aortocoronary bypass surgery). The study was conducted before surgery, immediately after the operation was over, and 8 to 10 hours after surgery. It has been observed that the lungs do not play an essential part in the regulation of background plasma kallikrein activity. Metabolic function as regards kallikrein manifests only with the activat

Management of Patients With AIDS and Retroperitoneal Calcified Fibrosis During the Perioperative Period: A Case Report.

This case report details the perioperative management of a 27-year-old male patient with AIDS and a rare giant retroperitoneal calcifying fibrous tumor. The tumor's surgical removal posed high risk due to its proximity to vital structures and the patient's human immunodeficiency virus infection. The report highlights the importance of multidisciplinary collaboration in managing such complex cases. Key nursing considerations included providing psychological support to mitigate the stigma associated with AIDS, adhering strictly to infection control and disinfection protocols, closely monitoring

No cost difference between single or dual implants for distal femur fractures in the perioperative period.

PURPOSE: This study uses a value-driven outcome model to assess the cost-effectiveness of single versus dual implants used in the fixation of distal femur fractures (DFF). METHODS: A retrospective review identified all DFF treated at a level I trauma center between 2013 and 2023. Patients with open fractures, bilateral fractures, multiple injuries, and less than six months of cost-data or follow-up were excluded. DFF were divided into fixation by an intramedullary nail (IMN), lateral plate (LP), or dual construct (DC) including IMN and plate or dual plating. Cost data included all costs, incl

Improving Urothelial Carcinoma Outcomes: The Powerful Combination of Neoadjuvant and Adjuvant Chemotherapy in the Perioperative Period.

INTRODUCTION: To enhance urothelial carcinoma (UC) prognosis, clinicians combine surgery with intraoperative (ICT), neoadjuvant (NACT), or adjuvant chemotherapy (ACT); however, studies on their individual and combined effects vary. Furthermore, studies on the combined use of ACT and NACT are scarce. OBJECTIVE: This study aimed to assess the impact of these chemotherapy regimens on UC prognosis, particularly the effectiveness of ACT + NACT, using the Surveillance, Epidemiology, and End Results (SEER) database. METHODS: We analyzed 45,211 UC cases from 2019 to 2021, focusing on renal, ureter,

Anesthetic considerations for inadvertent severe hypertension in a patient with osteogenesis imperfecta during the perioperative period.

Osteogenesis imperfecta is a rare genetic syndrome caused by mutations of bone matrix protein type I collagen coding genes, which is characterized by increased bone fragility and recurrent fractures. We report a patient undergoing general anesthesia with additional nerve block for wrist and hip fracture repair. Preoperatively, we were confronted with an inadvertent severe hypertension without significant triggers and history, which prompted us to be vigilant for hypertensive emergencies due to the fragile vessel regulation capacity in osteogenesis imperfecta patients.

Administration of tamoxifen in the perioperative period to patients with breast carcinoma prolongs axillary fluid drainage.

Compelling evidence points to an estrogen receptor independent mechanism of action of tamoxifen in the extracellular matrix, in addition to its action via the estrogen receptors. We retrospectively studied 380 patients who underwent curative resection for primary breast cancer in our institution from January 1994 to December 1998, of which 227 received tamoxifen in the perioperative period and the remaining 153 never received tamoxifen or delayed the initiation of treatment for at least two weeks following the operation. The administration of tamoxifen in the perioperative period resulted in t

Place of High-Flow Oxygen Therapy (HFOT) in the Perioperative Period: A Thoracic Surgery Case Report.

High-flow oxygen therapy (HFOT) has emerged as a promising approach in perioperative respiratory management, particularly in preventing postoperative respiratory complications, although its application in thoracic surgery remains underexplored. We report the case of a 58-year-old female with a history of pulmonary tumor and metastatic bone disease who was admitted for thoracic surgery due to dyspnea (NYHA class III). Preoperative evaluation revealed poor general condition with stable hemodynamics. Given the elevated risk of respiratory complications, HFOT was initiated for both preoxygenation

Unexpected pheochromocytoma leading to cardiac arrest during the perioperative period: a case report and literature review.

BACKGROUND: Pheochromocytomas (PCCs) are rare neuroendocrine catecholamine (CA)-secreting tumours that originate from chromaffin tissue and can produce and store CAs. Unexpected PCCs pose a serious threat to the perioperative safety of patients and a considerable challenge to anaesthesiologists because of the risks of fatal hypertensive crises and other stresses. CASE PRESENTATION: A 37-year-old woman who was scheduled for tonsillectomy and palatopharyngoplasty under general anaesthesia experienced a malignant cardiovascular event after induction, which was characterized mainly by a sharp inc

Coagulation parameters correlate to venous thromboembolism occurrence during the perioperative period in patients with spinal fractures.

BACKGROUND: Venous thromboembolism (VTE) is one of the leading causes of mortality in hospitalized patients. However, whether the coagulation-related parameters of the hospitalized patients could be used to predict the occurrence of VTE in patients with spinal injury surgery remained unclear. METHOD: The patients with spinal fractures who met the inclusion and exclusion criteria were enrolled to be analyzed using a retrospective analysis approach. The association of risk factors of enrolled patients and operations to VTE occurrence were analyzed. The activated partial thromboplastin time, pro

Myocardial Infarction in Non-obstructive Coronary Arteries (MINOCA) in the Perioperative Period can Epinephrine be Responsible?

Ephedrine, metaraminol, epinephrine and maneuvers like carotid sinus stimulation used during intraoperative period have been postulated to cause temporary spasm of the coronary vessels leading to decrease supply to the myocardium and precipitating myocardial infraction in non-obstructive coronary arteries (MINOCA). As an anaesthesiologists, we should be aware that even a dose as small as 25 mcg epinephrine infiltrated along with local anaesthetic in the subcutaneous plane may be responsible for coronary vessel spasm and thus myocardial infraction in nonobstructive coronary arteries. We report
