Upper limb tremor induced by peripheral nerve injury.

We report a patient with proximal right upper limb tremor, secondary to direct peripheral nerve lesion caused by prior thoracic surgery. Electromyography demonstrated neurogenic abnormalities and tremor in muscles innervated by the thoracodorsal and long thoracic nerves. Somatosensory evoked potentials, transcranial magnetic stimulation, and MRI of the cervical and thoracic spine were normal. Tremor persisted in REM and non-REM sleep. These findings suggest a peripheral generator.

[Recent progress in diagnosis and treatment of the injury to the peripheral nerve].

OBJECTIVE: To investigate the latest development in diagnosis and treatment of the injury to the peripheral nerve. METHODS: The literature at home and abroad was reviewed, and the research findings with clinical experience in diagnosis and treatment for the injury to the peripheral nerve were summarized. RESULTS: The treatment for the total brachial plexus avulsion injury was successfully performed by the extra-plexus nerve transfer. The avulsion of the brachial plexus could be directly repaired by the healthy C7 nerve root transfer through the anterior spinal approach. The forearm flexors c

Effects of trapidil after crush injury in peripheral nerve.

In this study, we evaluated the effects of trapidil on crush injury by monitoring nitric oxide, malondialdehyde and transforming growth factor-Beta2 levels and by transmission electron microscopy in the rat sciatic nerve. The sciatic nerve was compressed for 20 sec by using a jewelers forceps. Trapidil treatment groups were administrated a single dose of trapidil (8 mg/kg) intraperitoneally just after the injury. The crush and crush + trapidil treatment groups were evaluated on the 2nd, 7th, 15th, 30th and 45th days of the post-crush period. On the 7th and 15th days, damage in thin and thick m

Peripheral nerve ischemia: reperfusion injury and fiber regeneration.

We continued our studies of ischemia-reperfusion (IR) injury, extending the reperfusion duration to 42 days to capture the fiber regeneration process. We used a rat model for IR injury produced by ligation and release of nooses around supplying vessels to the sciatic nerve. Fifty-six rats were used. One group (control N = 8) underwent sham ischemia; the other six groups (N = 8 each) underwent complete hind limb ischemia for 4 h followed by reperfusion durations of 0 h (ischemia alone), 3 h, 7 days, 14 days, 28 days, and 42 days. Behavioral and electrophysiological data were obtained immediatel

[Peripheral nerve injury as a complication from orthopedic operation].

Nerve injury following operation is one of the main causes of the iatrogenic peripheral nerve injury. In order to learn lessons from these cases, one hundred and seven cases of peripheral nerve injury complicated with the orthopedic operations were analyzed. Forty-four cases were cutting injury during operation, made up 41% of all cases and 27 cases were stretch and compression injury, made up 25%. The involved nerves included 41 radial nerves and 24 common peroneal nerves, composing 60.7% of all nerve injury. The operations responsible were mainly the bone and joint operations, which made up

Differentially expressed genes after peripheral nerve injury.

In an attempt to identify genes associated with Wallerian degeneration and peripheral nerve regeneration we have performed differential hybridization screening of a cDNA library from crushed rat sciatic nerve (7 days postlesion) using radioactively labeled cDNA prepared from poly(A)+ RNA of normal vs. crushed nerve. Screening of 5,000 randomly selected colonies yielded 24 distinct clones that were regulated following nerve injury. Fifteen of the differentially expressed sequences could be classified as induced, whereas 9 sequences appeared to be repressed at 1 week postcrush. Sequencing and co

Peripheral nerve injury and causalgia secondary to routine venipuncture.

I examined 11 patients with upper-extremity causalgia secondary to peripheral nerve injury occurring during routine venipuncture. The nerves affected were the medial (n = 5) and lateral (n = 2) antebrachial cutaneous in the antecubital fossa, the superficial radial at the wrist (n = 2), and the dorsal sensory branches in the hand (n = 2). Anatomically, nerves lie on a plane just beneath and in close proximity to veins, making them vulnerable to injury during the procedure.

[Peripheral nerves injection injury: the clinical and experimental study].

The results of operative management of peripheral nerve postinjection injuries in 58 patients are reported. Neurolysis is one effective method in treating nerve postinjection injuries with excellent and good results observed in 64.6 percent of patients in this series. We also investigated the early changes in neurophysiological, histological and ultrastructural studies in the peroneal nerve of the rabbit. The results from early exploration and 0.9% NaCl irrigation were compared with those from late extraneural and intraneural neurolysis. The results showed that the injury occurs early in the n

Modern MRI Diagnostics of Upper-Extremity-Related Nerve Injuries-A Prospective Multi-Center Study Protocol for Diagnostics and Follow Up of Peripheral Nerve Injuries.

(1) Background: Peripheral nerve injuries are severe injuries with potentially devastating impairment of extremity function. Correct and early diagnosis as well as regular regeneration observation is of utmost importance for individualized reconstruction and the best possible results. Currently, diagnoses and follow-up examinations are based on clinical examinations supported with electroneurography, which often causes delays in treatment and can result in impaired healing. However, there is currently no diagnostic device that can reliably correlate the anatomic-pathological parameters with th

The therapeutic potential of ex vivo expanded CD133+ cells derived from human peripheral blood for peripheral nerve injuries.

OBJECT: CD133(+) cells have the potential to enhance histological and functional recovery from peripheral nerve injury. However, the number of CD133(+) cells safely obtained from human peripheral blood is extremely limited. To address this issue, the authors expanded CD133(+) cells derived from human peripheral blood using the serum-free expansion culture method and transplanted these ex vivo expanded cells into a model of sciatic nerve defect in rats. The purpose of this study was to determine the potential of ex vivo expanded CD133(+) cells to induce or enhance the repair of injured peripher

Peripheral nerve injury repair using a nerve connector as an alternative to direct repair.

OBJECTIVE: Nerve injuries can have devastating effects on patients' quality of life. However, the clinical results of direct nerve repair, which is commonly performed for peripheral nerve injury, are often unsatisfactory because of undesirable tension at the repair site. Previous studies have suggested that using a nerve conduit as a connector could provide outcomes that are equivalent to, or potentially better than, direct suture repair. Therefore, the aim of this study was to investigate the underlying mechanisms of peripheral nerve regeneration through a short gap encapsulated in a conduit

[Primary nerve transplantation: does it make sense in defect injury of peripheral nerves?].

Regeneration of damaged nerves limits the outcome of complex injuries of the upper extremities. We report the follow-up results in 7 of 10 patients over 5 years who underwent a transplantation of n. suralis in 6 cases and n. cutaneus antebrachii ulnaris in 4 cases. The average DASH-score of 30 points, no neurinoma and a protective sensibility in 5 cases are comparable with the literature.

Nerve transfers for peripheral nerve injury in the upper limb: a case-based review.

Nerve transfer has become a common and often effective reconstructive strategy for proximal and complex peripheral nerve injuries of the upper limb. This case-based discussion explores the principles and potential benefits of nerve transfer surgery and offers in-depth discussion of several established and valuable techniques including: motor transfer for elbow flexion after musculocutaneous nerve injury, deltoid reanimation for axillary nerve palsy, intrinsic re-innervation following proximal ulnar nerve repair, and critical sensory recovery despite non-reconstructable median nerve lesions.

Transection and Crush Models of Nerve Injury to Measure Repair and Remyelination in Peripheral Nerve.

Injury to the peripheral nervous system begins a well-characterized process within both neurons and Schwann cells to allow axonal regrowth, remyelination, and functional repair. Models of peripheral nerve injury have been widely used to study the behavior of Schwann cells, neurons, and other cell types such as macrophages as the events of Wallerian degeneration and regeneration take place. The most commonly used approaches in rodent models to model nerve injury in human patients are sciatic nerve transection and nerve crush, and both have well established time courses of demyelination, immune

The promotion of peripheral nerve regeneration by chitooligosaccharides in the rat nerve crush injury model.

Chitooligosaccharides (COSs), the biodegradation product of chitosan, have shown many biological functions. In this study, we examined the possible benefits of treatment with COSs (M.W. 800) on regeneration of rat crushed sciatic nerves. The rats with sciatic nerve crush injury were administered intraperitoneally daily with 3 or 6mg/kg body weight of COSs over a 3-week period. During and at the end of COSs treatment, a series of functional and histological examinations, including the measurement of withdrawal reflex latency (WRL) values, walking track analysis, electrophysiological assessments

Different expressions of BDNF, NT3, and NT4 in muscle and nerve after various types of peripheral nerve injuries.

The changes in the expression of brain-derived neurotrophic factor (BDNF), neurotrophin-3 (NT-3), and neurotrophin-4 (NT-4) in the rat neuromuscular system as a result of three different types of sciatic nerve injuries have been evaluated. The changes in mRNA and protein levels for BDNF, NT-3, and NT-4 in the soleus muscle and sciatic nerve were assessed 4-28 days after sciatic nerve transection (neurotmesis), sciatic nerve crush (axonotmesis), and mild acute compression (neurapraxia). BDNF mRNA levels increased dramatically with nerve transection in the soleus muscle and the sciatic nerve 7-1
