Hydroxyurea (HU) was approved to be used in the treatment of sickle cell disease (SCD) because of its anti-sickling potential. However, there is variability in HU response among SCD patients and this can be due to physiological, socioeconomic, environmental, metabolic and/or genetic factors. The present review focuses on the latter two. Three quantitative trait loci, HBG2, BCL11A and HMIP, have been suggested as important markers for HU response. Other genes (ASS1, KLF10, HAO2, MAP3K5, PDE7B, TOX, NOS1, NOS2A, FLT1, ARG1, ARG2, UGT1A1, OR51B5/6, SIN3A, SALL2, SAR1A, UTB, OCTN1, CYP2C9, AQP9, M

INTRODUCTION: Carriers of variant alleles of genes that encode liver CYP450 and UGT enzymes may experience abnormal plasma levels of antipsychotics and, consequently, worse efficacy or tolerability. Although pharmacogenomics is a rapidly developing field, current guidelines often rely on limited, underpowered evidence. We have previously demonstrated that meta-analysis is a viable strategy for overcoming this problem. Here, we propose a project that will expand our previous work and create a living systematic review and meta-analysis of drug plasma level differences between carriers and non-ca

PURPOSE OF REVIEW: Therapeutic drug monitoring (TDM) and pharmacogenomic testing (PGx) are increasingly important in optimizing psychopharmacological treatments. This review highlights the significance of integrating these tools to enhance pharmacological care in psychiatry.
RECENT FINDINGS: TDM has a longstanding tradition in managing psychiatric disorders, showcasing interindividual variability in drug metabolism and response. Recent studies demonstrate that TDM improves symptom control, reduces relapse rates, and decreases hospitalization for patients on psychotropic medications. The intro

Most pharmacogenetic research is conducted in adult, non-pregnant populations of European ancestry. Study of more diverse and special populations is necessary to validate findings and improve health equity. However, there are significant barriers to recruitment of diverse populations for genetic studies, such as mistrust of researchers due to a history of unethical research and ongoing social inequities. Engaging communities and understanding community members' perspectives may help to overcome these barriers and improve research quality. Here, we highlight one method for engaging communities,

Alcohol use disorder (AUD) is highly prevalent and among the leading causes of morbidity and mortality in the United States. Pharmacotherapies for AUD are limited, thus making identification of patient subgroups that are most likely to respond favorably crucial. In this article, pharmacogenetic research on US FDA-approved and commonly prescribed off-label medications for the treatment of AUD is comprehensively reviewed. While the field has advanced in understanding pharmacotherapies for AUD and potential genetic moderators of treatment responses, the pharmacogenetic data to guide the prescribi

Flecainide is a class IC antiarrhythmic utilized in prophylaxis of refractory paroxysmal supraventricular tachycardias in pediatric populations. Despite being a highly effective agent, its narrow therapeutic index increases the risk of toxicity and proarrhythmic events, including wide-complex tachycardia. In the absence of direct plasma sampling in the fetus to quantitate flecainide systemic concentrations, clinicians typically make drug dosing decisions from maternal plasma concentrations and QRS duration on maternal ECGs. There remains a paucity of standard guidelines and data to inform the

PURPOSE OF REVIEW: NSAIDs are the drugs most frequently involved in hypersensitivity reactions (HSR). These are frequently prescribed at all ages. HSR are of great concern and can affect people at any age. These drugs can induce reactions by stimulating the adaptive immune system (IgE or T cell), known as selective responders or more frequently by abnormalities in biochemical pathways related with prostaglandin metabolism. These are known as cross-intolerant. With some exceptions, skin testing and in-vitro studies are of little value in selective responders.
RECENT FINDINGS: In the last years

Testing thiopurine methyltransferase (TPMT) enzyme activity or genotype prior to thiopurine prescribing is recommended to reduce the risk of moderate to severe-and potentially fatal-myelosuppression in poor or intermediate TPMT metabolizers. Despite this, only about one-third of individuals prescribed thiopurines in Australia currently receive TPMT testing. The budgetary implications of expanding testing to align with guidelines remain unclear. We conducted a budget impact analysis from the Australian healthcare system perspective, comparing costs under current versus increased TPMT testing up

BACKGROUND: People living with life limiting conditions such as incurable cancer often have problems with pain, vomiting and other symptoms which impact quality of life. Supportive and Palliative Care aims to improve people's symptoms, particularly towards the very end of life. Many medications exist that can be used to help with these symptoms, but often they do not work, or they cause side effects for the people taking them. "Pharmacogenomics" is a way of predicting who is more likely to have a good response to drugs, and who is more likely to get side effects, based on variation in their ge

BACKGROUND: Although pharmacogenomics can effectively guide antihypertensive treatment, relevant evidence from low‑resource primary care settings is lacking. The aim of this study is to evaluate a pharmacogenomic‑guided antihypertensive strategy in real‑world, low‑resource primary care settings.
METHODS: We randomly assigned 94 villages (in 1:1:1 ratio, with stratification by villages) to receive a multifaceted intervention that included follow-up, health education, and medication adjustment guided by pharmacogenomics (treatment) or physician's experience (control), or routine care (observati

Aminoglycosides are widely used antibiotics with notable side effects, such as nephrotoxicity, vestibulotoxicity, and sensorineural hearing loss (cochleotoxicity). MT-RNR1 is a gene that encodes the 12s rRNA subunit and is the mitochondrial homologue of the prokaryotic 16s rRNA. Some MT-RNR1 variants (i.e., m.1095T>C; m.1494C>T; m.1555A>G) more closely resemble the bacterial 16s rRNA subunit and result in increased risk of aminoglycoside-induced hearing loss. Use of aminoglycosides should be avoided in individuals with an MT-RNR1 variant associated with an increased risk of aminoglycoside-indu

Adverse drug reactions (ADRs) are a major problem in modern medicine, representing up to the fourth-highest cause of mortality. Pharmacogenomic tests are 1 of the most promising methods to tackle the challenge of ADRs. The objective of this study was to analyze the clinical and demographic information of the pan-Canadian active surveillance network, Canadian Pharmacogenomics Network for Drug Safety (CPNDS). Information entered into the database by trained active surveillors between May 15, 2005 and May 9, 2017 was collected and analyzed. Specific data included for analysis were number of ADR r

Opioids serve as the cornerstone for anesthesia, postoperative analgesia, and chronic pain management. However, significant interindividual variations exist in their efficacy and safety, posing substantial challenges to clinical practice. Genetic polymorphisms of Cytochrome P450 (CYP) enzymes, particularly CYP2D6 and CYP3A4/5, represent one of the primary contributors to these differences. This review aims to elucidate the mechanisms by which CYP enzyme gene polymorphisms influence opioid metabolism and discuss their clinical implications. Key conclusions indicate that the CYP2D6 genotype sign

Glucocorticoids remain among the most widely prescribed immunosuppressive and anti-inflammatory agents worldwide, yet their clinical use is complicated by substantial inter-individual variability in both therapeutic response and adverse effects. Despite decades of clinical experience, glucocorticoid prescribing remains largely empirical, with limited integration of precision medicine approaches that could optimize therapeutic outcomes while minimizing toxicity. This review synthesizes current evidence on emerging precision approaches to glucocorticoid therapy across three key domains: pharmaco

CYP3A5 genotype is a known predictor of tacrolimus dose requirements. This single-center retrospective study evaluated the impact of CYP3A5 genotype-guided dosing, implemented at our institution in 10/2023, on early tacrolimus exposure. Patients were included if they were adult kidney transplant recipients between 7/2014 and 10/2024; patients in the weight-based cohort had research CYP3A5 genotypes in the institutional research biorepository and the genotype-guided cohort had a clinical CYP3A5 genotype available at transplant. The primary outcome was time to therapeutic steady-state (TSS) and

Effective pain management is a key component in the continuum of perioperative care to ensure optimal outcomes for surgical patients. The overutilization of opioids in the past few decades for postoperative pain control has been a major contributor to the current opioid epidemic. Multimodal analgesia (MMA) and enhanced recovery after surgery (ERAS) pathways have been repeatedly shown to significantly improve postoperative outcomes such as pain, function and satisfaction. The current review aims to examine the history of perioperative MMA strategies in ERAS and provide an update with recent evi

CYP2C19 loss of function (LOF) carriers undergoing percutaneous coronary intervention (PCI) have an increased risk of ischemic events when treated with clopidogrel. PCI patients in TAILOR-PCI were randomized to clopidogrel or genotype-guided (GG) therapy in which LOF carriers received ticagrelor and non-carriers clopidogrel. Direct medical costs associated with a GG approach have not been described before. TAILOR-PCI participants for whom direct medical costs were available for the duration from the date of PCI to one-year post PCI were included. Primary cost estimates were obtained from the M
