OBJECTIVES: To assess the predictive value of various risk factors for pharyngocutaneous fistula (PCF) after total laryngectomy.
METHODS: The characteristics of each study were collected from six databases up to January of 2022. Risk for bias was assessed using the QUADAS-2 tool.
RESULTS: A total of 58 studies in 9845 patients were included in the analysis. The incidence of PCF was 21.69%, 95% confidence intervals (CI) [0.20; 0.24] in the included studies. Age (OR = 1.33, 95% CI [1.12; 1.58]), postoperative anemia (OR = 2.29, 95% CI [1.47; 3.57]), diabetes mellitus (OR = 1.81, 95% CI [1.20;

Objective Carcinosarcoma consists of carcinomatous and sarcomatous tissues and is an aggressive malignant tumor. It is rarely reported in the hypopharynx. Methods A 72-year-old man presented with dysphagia and dyspnea. Laryngoscopy, computed tomography (CT), and 18F-fluorodeoxyglucose positron emission tomography/CT (18F-FDG PET/CT) showed a neoplasm on the left posterior hypopharyngeal wall. The patient underwent bilateral neck dissection and excision of the hypopharyngeal cancer followed by postoperative radiation therapy. Results Immunohistochemistry revealed carcinomatous cells with membra

BACKGROUND: Madelung's disease is a rare disorder characterised by the presence of multiple, symmetric, non-capsulated fat masses in the face, neck and other areas of upper extremities. In some cases, severe clinical complications such as upper airway compression can occur.
CASE REPORT: A 56-year-old man affected by Madelung's disease complained of snoring and severe daytime sleepiness. Polysomnography revealed severe obstructive sleep apnoea. An attempt to treat sleep apnoea by continuous positive airway pressure failed because of poor compliance. Functional expansion pharyngoplasty was carr

Seven patients with advanced lateral oropharyngeal cancer (T3N2bM0, or T4N2bM0) underwent transoral lateral oropharyngectomy (TLO) with reconstruction performed through set-back tongue flap and polyglycolic acid (PGA) sheet. TLO was performed following en bloc resection of tumors using endoscopy. To cover the resulting defect in the lateral oropharyngeal wall, the set-back tongue flap was moved posteriorly and laterally to the area of the tongue base and lateral pharyngeal wall. The tip of the set-back tongue flap was sutured to the lateral pharynx to reconstruct the elevated tongue base and a

Combined latissimus dorsi musculocutaneous and scapular fasciocutaneous free flap were successfully used to reconstruct two complex pharyngolaryngectomy defects with no anterior neck skin. The cervical esophagus and hypopharynx were reconstructed with the scapular portion, and the latissimus segment restored the anterior neck skin. One defect was reconstructed immediately, and the other followed an earlier ablation.

One-stage laryngopharyngeal free microvascular jejunal transfer after total laryngopharyngectomy was performed in 8 patients with advanced laryngopharyngeal cancer. The study showed that this was a relatively safe and time-saving method for hypopharyngeal reconstruction after radical cancer surgery. It improved the quality of life of the patients, allowing normal alimentation, though in some cases only temporarily due to the highly malignant nature of the disease.

In patients with freedom from invasion to anterior half of the larynx in postcricoid and cervical esophageal cancer, reconstruction of vocal tract with preservation of the anterior half of the larynx is applied with oncologic rationality and we performed the procedure in three cases of T2N1. At neck dissection bilateral superior thyroid neurovascular pedicles and unilateral inferior thyroid vessels were preserved. The larynx and upper part of the trachea were devided on the frontal plane and the posterior half with the hypopharynx and cervical esophagus was removed. The anterior half of the pa

From 379 cases of laryngeal and pharyngeal carcinomas we picked out the T3-4 NO-3 tumors and proved whether the sandwich technique or the postoperative irradiation will give the better 5 year survival rates. For supraglottic carcinomas the 5 year survival rate is 59.9% after sandwich therapy and 53.0% after post-operative irradiation. In advanced glottic and hypopharyngeal cancers the prognosis does not depend on one of the both therapeutic schedules: the 5 year survival rate is 55.5% resp. 51.3% and 10.0% resp. 8.3%. Summarizing the carcinomas of all regions of the larynx, that is without the

Seventeen patients with carcinoma of the hypopharynx and/or cervical esophagus who require total pharyngolaryngectomy-esophagectomy are presented; Twenty-one different reconstructive methods were applied in the seventeen patients. An overall two year survival rate of 35 per cent was attained with effective palliation in 65 per cent of the patients. The Wookey and free jejunal transplantation procedures were unsuccessful as primary means of pharyngoesophageal reconstruction, although the Wookey principle was occasionally useful in salvaging failures of other procedures. The reversed deltopector

OBJECTIVE: Pharyngocutaneous fistula (PCF) is the most frequent complication after total laryngectomy. However, delayed oral feeding and inserting a nasogastric tube has been considered as a safe practice among head and neck surgeons, and there is no general agreement on the initiation of the oral intake. The aim of the study is to determine the incidence of PCF after primary total laryngectomy in a large homogenous case series and to evaluate whether fistula and early oral feeding are related.
METHODS: Patients who underwent total laryngectomy/laryngopharyngectomy with primary closure of pha

Squamous cell carcinoma of the hypopharynx represents a distinct clinical entity. Most patients present with significant comorbidities and advanced-stage disease. The overall survival is relatively poor because of high rates of regional and distant metastasis at presentation or early in the course of the disease. A multidisciplinary approach is crucial in the overall management of these patients to achieve the best results and maintain or improve functional results. Traditionally, operable hypopharyngeal cancer has been treated by total (occasionally partial) laryngectomy and partial or circum

We conducted a retrospective chart review of 85 patients (95% male) with advanced but resectable hypopharyngeal carcinoma to evaluate the effectiveness of an intensified multimodal treatment protocol. This protocol involved concomitant chemo- and radiotherapy before and after total laryngectomy, partial pharyngectomy, and radical neck dissection. Our study population was divided into two groups on the basis of extrasurgical therapy. Group A was made up of 19 patients who had received only postlaryngectomy radiotherapy; 6 of these patients had a stage III tumor and 13 had a stage IV tumor. Grou

The purpose of conservation surgery is to eradicate the cancerous lesion while preserving the morphology and function of the affected organ as much as possible. Many important functions including swallowing, phonation and articulation are carried out by the organs in the head and neck. This paper presents the short-term (2 years) results of conservation surgery for carcinomas of the tongue, oropharynx, hypopharynx and larynx. Long-term results await further follow-up because of the fact that we started full use of conservation surgery rather recently. Three types of conservation surgery were a
