Visual cortical activity in Charles Bonnet syndrome: testing the deafferentation hypothesis.

Visual hallucinations in individuals following sight loss (Charles Bonnet syndrome; CBS) have been posited to arise because of spontaneous, compensatory, neural activity in the visual cortex following sensory input loss from the eyes-known as deafferentation. However, neurophysiological investigations of CBS remain limited. We performed a multi-modal investigation comparing visual cortical activity in 19 people with eye disease who experience visual hallucinations (CBS) with 18 people with eye disease without hallucinations (ED-Controls; matched for age and visual acuity) utilising functional

Safety of rTMS to non-motor cortical areas in healthy participants and patients.

OBJECTIVE: rTMS is increasingly being used for stimulation to non-motor areas, but available safety guidelines are derived from experience with motor cortex rTMS. We reviewed the literature and our own data to assess the safety of rTMS to non-motor areas. METHODS: We reviewed for adverse effects all articles published from January 1998 to December 2003 that applied rTMS to non-motor areas, and analyzed data from our own studies from January 1997 to December 2003. RESULTS: Adverse effects were infrequent and generally mild. Headache was the most common, occurring in 23% of the subjects and mo

Simulated prosthetic vision confirms checkerboard as an effective raster pattern for epiretinal implants.

Objective.Spatial scheduling of electrode activation ('rastering') is essential for safely operating high-density retinal implants, yet its perceptual consequences remain poorly understood. This study systematically evaluates the impact of raster patterns, or spatial arrangements of sequential electrode activation, on performance and perceived difficulty in simulated prosthetic vision (SPV). By addressing this gap, we aimed to identify patterns that optimize functional vision in retinal implants.Approach.Sighted participants completed letter recognition and motion discrimination tasks under fo

A Cell-Type Selective Stimulation and Recording System for Retinal Ganglion Cells.

Future retinal implants will require a stimulation selectivity between different sub-types of Retinal Ganglion Cells (RGCs) to evoke natural perceptions rather than phosphenes in patients. To achieve this, a cell-type specific stimulation pipeline is required that identifies target RGC sub-types from recorded input images and extracts the specific stimulation parameters to activate this cell-type selectively. Promising biological experiments showed that ON-/OFF- sustained/transient RGCs could be selectively activated by modulating repetition rate and amplitude of an electrical stimulation curr

Glow in the dark: Using a heat-sensitive camera for blind individuals with prosthetic vision.

To date, retinal implants are the only available treatment for blind individuals with retinal degenerations such as retinitis pigmentosa. Argus II is the only visual implant with FDA approval, with more than 300 users worldwide. Argus II stimulation is based on a grayscale image coming from a head-mounted visible-light camera. Normally, the 11°×19° field of view of the Argus II user is full of objects that may elicit similar phosphenes. The prosthesis cannot meaningfully convey so much visual information, and the percept is reduced to an ambiguous impression of light. This study is aimed at in

Clinical manifestations of hydropic ear disease (Menière's).

INTRODUCTION: Hydropic ear disease, initially described by and named after Prosper Menière, is one of the most frequent vertigo disorders and one of the most frequent inner ear disorders. It is the syndrome of endolymphatic hydrops which until 2007 could be diagnostically confirmed only by post-mortem histology. In the past, various attempts to formulate clinical diagnostic criteria have been undertaken but were hampered by the inability to ascertain the diagnosis in living patients. With the milestone achievement of endolymphatic hydrops imaging, today the pathology can be ascertained. In thi

3D mapping of static magnetic field magnitude and axial-components around a total body 3T MRI clinical scanner.

OBJECTIVE: The technology employed in magnetic resonance imaging (MRI) systems has evolved continuously, resulting in MRI scanners with stronger static magnetic fields (SMF) B0, faster and stronger gradient magnetic fields, and more powerful radiofrequency transmission coils. The most well-known hazard associated with an MRI environment is the projectile effect due to Spatial Field Gradient (SFG). Furthermore, movement through the SFG generates a time-varying magnetic field, which in turn induces a voltage in body tissues. This has the potential to result in a range of physiological symptoms,

Mapping autonomic, mood and cognitive effects of hypothalamic region deep brain stimulation.

Because of its involvement in a wide variety of cardiovascular, metabolic and behavioural functions, the hypothalamus constitutes a potential target for neuromodulation in a number of treatment-refractory conditions. The precise neural substrates and circuitry subserving these responses, however, are poorly characterized to date. We sought to retrospectively explore the acute sequelae of hypothalamic region deep brain stimulation and characterize their neuroanatomical correlates. To this end we studied-at multiple international centres-58 patients (mean age: 68.5 ± 7.9 years, 26 females) suffe

The Efficacy of Transcorneal Electrical Stimulation for the Treatment of Primary Open-angle Glaucoma: A Pilot Study.

The aim of this study was to evaluate the effects of transcorneal electrical stimulation in subjects with primary open-angle glaucoma. Five eyes of four male subjects with primary open-angle glaucoma (average age: 52.25 ± 14.68 years) were enrolled. The subjects underwent transcorneal electrical stimulation every 3 months according to the following procedure. A Dawson-Trick-Litzkow electrode was placed on the cornea, and biphasic electric current pulses (10 ms, 20 Hz) were delivered using a stimulator (BPG-1,BAK Electronics) and a stimulus isolation unit (BSI-2). A current that evoked a phosph

Altered white matter connectivity associated with visual hallucinations following occipital stroke.

INTRODUCTION: Visual hallucinations that arise following vision loss stem from aberrant functional activity in visual cortices and an imbalance of activity across associated cortical and subcortical networks subsequent to visual pathway damage. We sought to determine if structural changes in white matter connectivity play a role in cases of chronic visual hallucinations associated with visual cortical damage. METHODS: We performed diffusion tensor imaging (DTI) and probabilistic fiber tractography to assess white matter connectivity in a patient suffering from continuous and disruptive phosph

Learning to see via epiretinal implant stimulationin silicowith model-based deep reinforcement learning.

OBJECTIVE: Diseases such as age-related macular degeneration and retinitis pigmentosa cause the degradation of the photoreceptor layer. One approach to restore vision is to electrically stimulate the surviving retinal ganglion cells with a microelectrode array such as epiretinal implants. Epiretinal implants are known to generate visible anisotropic shapes elongated along the axon fascicles of neighboring retinal ganglion cells. Recent work has demonstrated that to obtain isotropic pixel-like shapes, it is possible to map axon fascicles and avoid stimulating them by inactivating electrodes or

[Cancer associated retinopathy (CAR). Two clinical cases and review of the literature].

The cancer associated retinopathy (CAR) is a paraneoplasic retinopathy in which an antigen-antibody reaction, due to retinal antigens, also expressed in tumours, leads to degeneration of retinal photoreceptor cells. We observed in CHL-Luxembourg, 2 clinical cases of non-Hodgkin's lymphoma with severe prognosis in whom we described the presence of anti-recoverin antibodies. The CAR is most frequently associated with small cell lung and ovarian carcinomas. Clinical symptoms (phosphenes, progressive loss of eyesight) sometimes, occur before the diagnostics of primary cancer. Retinal degeneration

Hemianopic anosognosia.

In a prospective study of 32 consecutive patients with homonymous visual field defects due to ischemic infarcts we found hemianopic anosognosia (HAN), defined as the unawareness of visual loss in the homonymous hemifield (or hemiquadrant), in 20 patients (62%). HAN, although occurring predominantly in right-side lesions in 16 of 26 patients (62%) was also present in four of six patients (or 67%) with left-side lesions. This group of patients has been presented in a prior report on positive spontaneous visual phenomena. HAN was associated with somatic anosognosia in nine patients and hemineglec

Targeting alpha-band oscillations in a cortical model with amplitude-modulated high-frequency transcranial electric stimulation.

Non-invasive brain stimulation to target specific network activity patterns, e.g. transcranial alternating current stimulation (tACS), has become an essential tool to understand the causal role of neuronal oscillations in cognition and behavior. However, conventional sinusoidal tACS limits the ability to record neuronal activity during stimulation and lacks spatial focality. One particularly promising new tACS stimulation paradigm uses amplitude-modulated (AM) high-frequency waveforms (AM-tACS) with a slow signal envelope that may overcome the limitations. Moreover. AM-tACS using high-frequenc
