Childhood sexual, physical, and psychological abuse in bulimia nervosa.

OBJECTIVE: This study sought to determine if rates of childhood sexual, physical, psychological, and multiple abuse (i.e., abuse in more than one form) differed between women with a lifetime history of bulimia nervosa and women with no history of eating disorders. METHOD: Subjects were 80 women, aged 18-35, with a lifetime history of bulimia nervosa (40 women who had recovered for a year or more and 40 women currently suffering from bulimia nervosa) and 40 women who had never had an eating disorder or related difficulties. The cohort was obtained primarily by newspaper advertisement. Subjects

Routine Emergency Department Screening to Decrease Subsequent Physical Abuse.

STUDY OBJECTIVE: Emergency department (ED) screening for child physical abuse has been widely implemented, with uncertain effects on child abuse identification. Our goal was to determine the effect of screening on referrals to child protective services (CPS) identifying abuse. METHODS: We performed a retrospective cohort study of children younger than 6 years old with an ED encounter at 1 of 2 large health care systems, one of which implemented routine child abuse screening. The main outcome was initial (<2 days) or subsequent (3 to 180 days) referral to CPS identifying child abuse using link

Glimpsing the Iceberg: Parent-Child Physical Aggression and Abuse.

Despite evidence that parents' physical aggression abuse has long-lasting negative consequences, information about the true population prevalence of aggression and physical abuse is limited. We have even less information about how parental aggression and abuse vary by child age, parent gender, and how that aggression and abuse might be clustered within families. To address these gaps, an anonymous, computer-based assessment was administered to nearly 40,000 parents of more than 60,000 children in the United States Air Force, which included a detailed assessment on up to four minor children of

Cutaneous mimickers of physical child abuse: A brief overview.

Child abuse is one of the medico-legal issues a physician may face during his/her clinical practice. It has devastating effects on both the child and family, especially psychological. If falsely identified as a child abuse case, it could result in detrimental consequences. Therefore, physicians must recognise and be able to rule out child abuse mimickers, which are often conditions that are mistakenly confused with true physical child abuse. Injuries like bruises and burns are common presentations and therefore it is important to consider cutaneous abuse mimics to avoid incorrect diagnosis of

Radiation doses in diagnostic imaging for suspected physical abuse.

OBJECTIVE: To measure the actual radiation dose delivered by imaging techniques commonly used in the radiography of suspected physical abuse and to make this information available to health professionals and families. METHODS: Data were collected retrospectively on children under 3 years referred for skeletal surveys for suspected physical abuse, non-contrast CT head scan or radionuclide imaging of the bones in Starship Children's Hospital, Auckland, New Zealand from January to December 2015. Patient size-specific conversion coefficients were derived from International Commission on Radiologi

Impact of Physical Abuse on Internalizing Behavior Across Generations.

This study investigated the multigenerational impact of mothers' own exposure to physical maltreatment on internalizing symptoms in her child after accounting for her parenting practices, depression, and the child's own exposure to stressful life events. Children (n = 101, ages 5-16), predominantly African American, were recruited into this cross sectional study using ethnographic mapping and targeted sampling for high-risk neighborhoods. Mothers reported retrospectively on their own exposure to physical maltreatment in childhood, their parenting practices, as well as current depressive sympto

Childhood physical abuse and aggression: Shame and narcissistic vulnerability.

This study examined narcissistic vulnerability and shame-proneness as potential mediators between childhood physical abuse (CPA) and adult anger and aggression. Participants were 400 undergraduate students, 134 of whom had a history of CPA. All participants completed self-report questionnaires assessing history of CPA, shame-proneness, narcissistic vulnerability, physical aggression, trait anger, and hostility. Results indicated abused participants were more angry and aggressive and experienced higher levels of shame-proneness and narcissistic vulnerability than nonabused participants. Multipl

Physical abuse is associated with HIV-related drug risk.

Those who have experienced abuse may be prone to engaging in risky sexual behavior and risky drug use. The relationship between sexual abuse and risky behavior has been well established in the literature, but the association between physical abuse and risky drug use has been equivocal. We hypothesize that the experience of PTSD symptoms following physical abuse leads to risky drug use. Therefore, we examined the associations among physical abuse history, PTSD symptoms, and HIV-related drug risk in a sample of 121 opioid-dependent persons to determine whether PTSD symptoms mediated the relation

Sentinel injuries in infants evaluated for child physical abuse.

OBJECTIVE: Relatively minor abusive injuries can precede severe physical abuse in infants. Our objective was to determine how often abused infants have a previous history of "sentinel" injuries, compared with infants who were not abused. METHODS: Case-control, retrospective study of 401, <12-month-old infants evaluated for abuse in a hospital-based setting and found to have definite, intermediate concern for, or no abuse after evaluation by the hospital-based Child Protection Team. A sentinel injury was defined as a previous injury reported in the medical history that was suspicious for abuse

Body image among victims of sexual and physical abuse.

This study tries to understand the differences in body experience between victims of sexual abuse and physical abuse. Ninety-eight women completed questionnaires that measured personal information, body-image aberration, body sensitivity and control, and body investment. Findings indicated that victims of sexual abuse demonstrate less body maintenance and protection in addition to greater injury to body sensitivity and control than victims of physical abuse. Moreover, comparing victims of sexual abuse to physical abuse, findings revealed that only victims of sexual abuse report body-image aber

Investigating the association between childhood physical abuse and migraine.

BACKGROUND: Recent clinical and population-based studies suggest that adults who were physically abused as children are more likely to experience migraine than those who were not abused. OBJECTIVES: To investigate the relationship between childhood physical abuse and migraine while controlling for age, race, and gender, in addition to the following potential confounders: adverse childhood conditions; adult socioeconomic indicators; current health behaviors; current stressors; history of physical health conditions, and history of mood and/or anxiety disorders. METHODS: Secondary analysis of t

Effect of childhood physical abuse on cortisol stress response.

RATIONALE: Abuse and neglect are highly prevalent in children and have enduring neurobiological effects. Stressful early life environments perturb the hypothalamic-pituitary-adrenal (HPA) axis, which in turn may predispose to psychiatric disorders in adulthood. However, studies of childhood maltreatment and adult HPA function have not yet rigorously investigated the differential effects of maltreatment subtypes, including physical abuse. OBJECTIVE: In this study, we sought to replicate our previous finding that childhood maltreatment was associated with attenuated cortisol responses to stress

Social learning, sexual and physical abuse, and adult crime.

This research examines the relationship between childhood physical and sexual abuse and the types of crimes committed by male adult offenders. We use the method of discriminant prediction to determine whether independent and dependent variables are related in ways that theories predict. Our analyses of data from the Survey of Inmates in State and Federal Correctional Facilities suggest that offenders model specific behaviors to which they have been exposed. Male offenders who were sexually abused as a child are more likely to commit sexual offenses, particularly sexual offenses against childre

Bruising characteristics discriminating physical child abuse from accidental trauma.

OBJECTIVE: Our objective was to conduct a pilot study to identify discriminating bruising characteristics and to model those findings into a decision tool for screening children at high risk for abuse. METHODS: A case-control study of children 0 to 48 months of age who were admitted to a PICU because of trauma was performed. Case subjects (N = 42) were victims of physical abuse, and control subjects (N = 53) were children admitted because of accidental trauma during the same time period. Bruising characteristics (total number and body region) and patient age were compared for children with ab

Variability in expert assessments of child physical abuse likelihood.

OBJECTIVES: In the absence of a gold standard, clinicians and researchers often categorize their opinions of the likelihood of inflicted injury using several ordinal scales. The objective of this protocol was to determine the reliability of expert ratings using several of these scales. METHODS: Participants were pediatricians with substantial academic and clinical activity in the evaluation of children with concerns for physical abuse. The facts from several cases that were referred to 1 hospital's child abuse team were abstracted and recorded as in a multidisciplinary team conference. Partic

Child physical abuse: health professionals' perceptions, diagnosis and responses.

The objectives of this cross-sectional survey were to assess primary health professionals' perceptions of and ability to recognize child physical abuse in their practice. A random sample was taken of 979 nurses, doctors, and dentists working in primary care in Northern Ireland. The response rate was 44%, and the results showed that 59% of respondents had seen a suspicious case of child physical abuse, of which 47% (n = 201) had reported it. Ability and willingness to recognize and report abuse varied across the three professions. The findings suggest a professional reluctance to engage in reco

Attributions and adjustment following child sexual and physical abuse.

Individuals who experience child physical orsexual abuse are at greater risk for adjustment problems. However, the heterogeneity of symptomatology observed across individuals following child abuse suggests that outcome is determined by multiplefactors. The authors review literature examining the relationships among childhood physical or sexual abuse, attributions, and adjustment. Implications for application and future research are discussed.
