Ischiofemoral Impingement and Hamstring Syndrome as Causes of Posterior Hip Pain: Where Do We Go Next?

Recent advances in understanding hip joint anatomy and biomechanics have contributed to improvement of diagnosis and treatment decisions for distal causes of deep gluteal syndrome (DGS). Ischiofemoral impingement and hamstrings syndrome are sources of posterior hip pain that can simulate symptoms of DGS. The combination of a comprehensive history and physical examination with imaging and ancillary testing are critical for diagnosis. Six key physical examination tests are described to differentiate distal versus proximal sources of extrapelvic posterior hip pain. Outcomes depend on patient comp

Analysis of the Effectiveness of Injection-Based Minimally Invasive Treatment for Piriformis Syndrome: Clinical Outcomes and Anatomical Correlates.

PURPOSE OF REVIEW: Piriformis syndrome (PS) is an increasingly recognized cause of buttock pain and sciatica-like symptoms within the broader framework of deep gluteal syndrome. Injection-based minimally invasive treatments are commonly used when conservative therapy fails, yet reported outcomes remain heterogeneous. This structured narrative review with a systematic literature search evaluates the clinical effectiveness of local anesthetic, corticosteroid, and botulinum toxin injections for PS and examines the anatomical and procedural factors that may influence treatment response. Extra-spinal sciatica and sciatica mimics: a scoping review.

Not all sciatica-like manifestations are of lumbar spine origin. Some of them are caused at points along the extra-spinal course of the sciatic nerve, making diagnosis difficult for the treating physician and delaying adequate treatment. While evaluating a patient with sciatica, straightforward diagnostic conclusions are impossible without first excluding sciatica mimics. Examples of benign extra-spinal sciatica are: piriformis syndrome, walletosis, quadratus lumborum myofascial pain syndrome, cluneal nerve disorder, and osteitis condensans ilii. In some cases, extra-spinal sciatica may have a

A three-headed piriformis muscle: an anatomical case study and narrative review of literature.

The piriformis muscle (PM) is found in the gluteal region, exiting the pelvis through the greater sciatic foramen and dividing it into the suprapiriform and infrapiriform foramina. The piriformis works as part of the hip external rotator muscle group, and is responsible for rotation of the femur upon hip extension and abduction of the femur during flexion of the hip joint. The aim of the present report is to describe a very rare case of the primary three-headed PM. To the best knowledge of the authors, the said variant has not yet been described in the existing literature. The 71-year-old male

Anatomical Relationships of the Sciatic Nerve and Pudendal Nerve to the Ischial Spine as They Exit the Greater Sciatic Foramen.

OBJECTIVE: Deep gluteal syndrome is a clinical condition in which discomfort may arise due to the pathoanatomy of the subgluteal space. We conducted an anatomical exploration to categorize the relationship of the piriformis muscle, sciatic nerve (SN), and pudendal nerve (PN) to the ischial spine (IS) and sacrospinous ligament. METHODS: We analyzed 22 cadavers. The piriformis muscle, SN, and PN were exposed through either a transgluteal approach or a gluteal flap. The relationship of the neural structures to the IS, sacrospinous ligament, and ischial bone as they exit the greater sciatic foram

Does endoscopic piriformis tenotomy provide safe and complete tendon release? A cadaver study.

BACKGROUND: Endoscopic piriformis release (EPR) is among the available treatments for piriformis syndrome. This procedure typically involves dividing the muscle near the sciatic nerve in the sub-gluteal space, which contains numerous blood vessels and nerves. The objectives of this prospective cadaver study were: 1) to assess the reproducibility and quality of endoscopic piriformis tenotomy near the greater trochanter; 2) to detect iatrogenic injuries to the lateral hip rotators, nerves, and vessels; 3) and to define the surgical safety margins relative to the sciatic nerve and inferior glutea

Piriformis Pyomyositis Presenting as Migratory Hip to Knee Pain: A Case Report.

CASE: The authors report a case of piriformis pyomyositis in a teenage female patient with fever and left hip pain. Her pain migrated to the knee with concurrent near resolution of hip pain. Imaging revealed an abscess in the left piriformis with pus tracking along the sciatic nerve sheath. This was complicated by internal iliac vein thrombosis and an embolus to the lung. Open drainage was performed, followed by outpatient intravenous cloxacillin and oral warfarin, with complete resolution of symptoms. CONCLUSION: Piriformis pyomyositis is a rare condition with varying presentations. The thre

Editorial Commentary: Gluteal Pain Syndrome of the Hip: It's Been There All Along!

The diagnosis and management of deep gluteal syndrome of the hip is currently evolving and the lack of prospective studies makes it even more difficult to discern what is the appropriate treatment. The communication between physicians managing this problem needs to improve to coordinate care better and limit the amount of unnecessary studies that these patients typically undergo.

Behind the Pain: Understanding and Treating Piriformis Syndrome.

Piriformis syndrome (PS) is a neuromuscular condition that occurs when excessive tension or anatomical variations in the piriformis muscle compress the sciatic nerve. This compression can cause pain in the buttock that radiates down the back of the affected lower limb, often resembling sciatica. This article presents a case of a 62-year-old woman with PS, characterized by left buttock pain and paresthesia along the sciatic nerve path, unresponsive to conservative treatments. Diagnosing PS is a difficult task because its symptoms often overlap with those of other conditions, such as lumbar radi

Restoring Functionality: A Case Report on Physiotherapeutic Rehabilitation for L5-S1 Anterolisthesis Management.

Anterolisthesis is a condition where a vertebra in the spine slips forward relative to the vertebra below it. Anterolisthesis is often described in terms of the direction of the slippage and the affected vertebrae, such as L5-S1 anterolisthesis, which indicates the slippage occurring between the fifth lumbar vertebra (L5) and the sacral bone (S1). Anterolisthesis can result from various factors, trauma, or congenital abnormalities. The symptoms associated with anterolisthesis can include lower back pain, stiffness, muscle tightness, and neurological symptoms if the slippage compresses nearby n

Deep gluteal space anatomy and its relationship with deep gluteal pain syndromes.

INTRODUCTION: Increasing interest has been seen in understanding the anatomy and biomechanics involved in the Deep Gluteal Syndrome, therefore the main objective of our paper was to define the anatomy of the deep gluteal space concerning the important osseous, muscular and neurological structures. METHODS: 12 cadaveric models (24 hemipelvises) were used. We proceeded with classical anatomic dissection and evaluated numerous osseous, musculotendinous and neurologic structures and their relationships. We also determined the femoral anteversion and neck-shaft angles. RESULTS: We found that 15.4

The majority of piriformis muscles are innervated by the superior gluteal nerve.

The piriformis muscle is clinically implicated in pain disorders, posterior approaches for total hip arthroplasty, and iatrogenic injury to the muscle and the surrounding nerves. The piriformis muscle has been said to receive innervation from L5 to S3 ventral rami with most sources using S1 and S2 ventral rami as the most common innervation this muscle. However, descriptions of the nerve in the literature are vague. Therefore, the aim of this study was to clarify the anatomy of the nerve supply to the piriformis muscle. Twenty sides from ten fresh-frozen cadavers were studied. Specifically, vi

Endoscopy for sciatic nerve entrapment in deep gluteal syndrome. A systematic review of literature.

BACKGROUND: Deep gluteal syndrome (DGS) involves pain in the buttocks due to entrapment of the sciatic nerve (SN) in the deep gluteal space. This condition can be caused by various structures within this space, such as muscles and fibrous bands, which often cause significant pain and mobility problems. Diagnosis is based on clinical evaluation, diagnostic imaging and nerve conduction studies. At the same time, treatment ranges from physical therapy to surgery, with endoscopic treatment gaining popularity due to its reduced invasiveness and fewer side effects. MATERIAL AND METHODS: The systema

Piriformis syndrome and low back pain: a new classification and review of the literature.

Piriformis syndrome is a common cause of low back pain. It is often not included in the differential diagnosis of back, buttock, and leg pain. Additionally it has received minimal recognition because it is often seen as a diagnosis of exclusion. Familiarity with the common elements of the syndrome should increase its recognition and facilitate the appropriate treatment. These include buttock pain and tenderness with or without electrodiagnostic or neurologic signs. Pain is exacerbated in prolonged sitting. Specific physical findings are tenderness in the sciatic notch and buttock pain in flexi

The sciatic nerve in human cadavers - high division or low formation?

Variations of the sciatic nerve have been extensively studied in the past including its relationship with the piriformis muscle and associated clinical conditions like piriformis syndrome and sciatica. In the present study we noticed some interesting variations of the sciatic nerve, which were slightly different from the cases described earlier. In the previous studies most of the authors described the higher division of sciatic nerve and none of them discussed its formation. In this study we tried to look its formation from the sacral plexus and its divisions in the thigh. We noticed that in

Variant anatomy of sciatic nerve in a black Kenyan population.

Knowledge of variant anatomy of the sciatic nerve is important in avoiding inadvertent injury during operations in the gluteal region and interpreting nondiscogenic sciatica. This variant anatomy may cause piriformis syndrome and failure of sciatic nerve block. The variations differ between populations but data from Africans is scarce. This study, therefore, investigated variations of sciatic nerve in a black Kenyan population. One hundred and sixty-four sciatic nerves from 82 cadavers of black Kenyans were exposed by dissection at the Department of Human Anatomy, University of Nairobi, Kenya.

Effective Stretching Positions of the Piriformis Muscle Evaluated Using Shear Wave Elastography.

CONTEXT: Piriformis syndrome is often associated with muscle spasms and shortening of the piriformis muscle (PM). Physical therapy, including static stretching of the PM, is one of the treatments for this syndrome. However, the effective stretching position of the PM is unclear in vivo. This study aimed to determine the effective stretching positions of the PM using ultrasonic shear wave elastography. DESIGN: Observational study. METHODS: Twenty-one healthy young men (22.7 [2.4] y) participated in this study. The shear elastic modulus of the PM was measured at 12 stretching positions using s

Uses and technique of ultrasound-guided botulinum toxin infiltration.

Botulinum toxin (BT) is a neurotoxin that causes flaccid paralysis by inhibiting the release of acetylcholine at the neuromuscular junction, and it may lead to long-term muscle atrophy. It is used to treat conditions associated with muscle hypertrophy or to enhance muscle flexibility, thereby facilitating surgical procedures. It has been shown that ultrasound-guided administration is superior to the anatomical landmarks technique as it reduces side effects and improves efficacy. Although some applications are not officially approved, evidence supports its efficacy and safety in the treatment o
