Bridging the gap between knowledge and action for health: Case studies.

Biomedical discoveries could improve people's health only if they are suited to the diverse political and social contexts, health systems and population groups. Knowledge generated through evidence-informed health policy and practice when applied to the local situation enhances the quality and efficiency of health care. This article describes four case studies on bridging the gap between knowledge and action for health in a tertiary care hospital in Bangkok, Thailand. Gaps between knowledge and action for health are classified into "know-do" and "do-know" gaps with knowledge implementation and

Institutionalising public health emergency preparedness and responses in Africa: lessons learned during the 2022-2025 outbreaks with crossborder spread potential.

Lessons and best practices from outbreaks during 2022-25 in Africa were not comprehensively documented or shared to inform future outbreak responses. We conducted a narrative review of published articles and outbreak response reports of mpox, cholera, Ebola virus disease, and Marburg virus disease and captured experts' perspectives and lessons. We analysed and presented the data in themes. Evidence indicates that effective responses are built on routine investments maintained between outbreaks, particularly in decentralised laboratories, digital surveillance systems, community structures, and

IS THERE CONVERGENCE ACROSS COUNTRIES? A SPATIAL APPROACH.

We analyze convergence across countries over the last half century as a result of globalizing forces. Drawing on theories of modernization, dependency, the world-system, political trade blocs, and the world-society, we consider economic, demographic, knowledge, financial, and political dimensions of convergence. Using a new methodology, we calculate the minimum volume ellipsoid encompassing different groupings of countries, finding that during the 1960-2009 period, countries have not evolved significantly closer or similar to one another, although groups of countries based on their core-periph

Who Counts, Who's Accountable: Independent Scholarship and the Future of Carceral Mortality Oversight.

Deaths in jails and prisons are rising, yet the federal government lacks a reliable system to track or prevent them. This essay examines the Department of Justice's failure to implement the Death in Custody Reporting Act and produce accurate and timely data on deaths behind bars. We call for transferring responsibility for carceral mortality monitoring to public health systems with capacity to identify multilevel drivers of death and support interventions that promote prevention and accountability. Shifting this responsibility affirms the value of lives lost and moves surveillance from error-p

Measles as tracer and tugboat: Why eradication can strengthen immunization systems.

As measles resurges globally, we face an important strategic choice. Large outbreaks reflect declining vaccination coverage, inequities in health services access, conflict, population displacement, vaccine hesitancy, and inadequate investment in immunization programs. However, measles is more than a vaccine-preventable disease. Because of its extraordinary transmissibility, it functions both as a tracer of weaknesses in immunization systems and public health infrastructure, and as a potential tugboat for strengthening them. The Immunization Agenda 2030 highlights strong immunization systems, eq

From emergency relief to system strengthening: Stakeholder perspectives on official development assistance for reproductive, maternal, neonatal and child health in Guinea-Bissau.

Guinea-Bissau, a politically fragile West African nation, relies heavily on official development assistance (ODA) to sustain reproductive, maternal, neonatal and child health (RMNCH) services. This study explored how national and international stakeholders perceive the role, evolution and effectiveness of RMNCH-related ODA and how these perspectives compare with donor strategies. We conducted semi-structured interviews with 10 key informants and applied an AI-assisted, systematically validated framework to analyse 50 donor documents, integrating findings through comparative coding and converge

Hollow by Design: Why Evaluation Systems Decouple in the MENA Region.

Institutional theory predicts that formal governance structures often develop without corresponding operational substance, a phenomenon variously described as decoupling or isomorphic mimicry. The evaluation governance literature has acknowledged this possibility but has not subjected it to rigorous set-theoretic analysis. The present study applies fuzzy-set Qualitative Comparative Analysis to data from 15 countries in the Middle East and North Africa to examine which institutional conditions are necessary and which are sufficient for substantive evaluation practice. Four conditions drawn from

The cholera outbreak in Syria: a call for urgent actions.

The ongoing cholera outbreak in Syria poses a significant public health threat that requires immediate and comprehensive attention. The spread of the outbreak is attributed to a combination of factors, including displacement due to armed conflict, chronic water insecurity, inadequate water, sanitation, and hygiene infrastructure, climate change-induced droughts, weakened health system capacity, and political instability. The recent earthquake in the region has further complicated the situation, potentially leading to a surge in cholera cases. The limited capacity of the Syrian health system to

Climate change drives a decline in global grazing systems.

Grazing systems represent the most extensive production systems in the world and are highly sensitive to climate change. However, their global-scale sensitivity and vulnerability to climate impacts remain poorly understood. Here, we apply the safe climatic space framework to assess how changes in core climatic drivers of grazing suitability, including temperature, precipitation, humidity, and wind speed, will reshape global grassland-based grazing systems. Our analysis projects a net decline of 36 to 50% of areas in climate suitability for grazing by 2100, accompanied by inter- and intracontin

Implementation of integrated disease surveillance and response systems in West Africa: lessons learned and future directions.

The Integrated Disease Surveillance and Response (IDSR) framework, introduced by the WHO in 1998, aimed to unify disease surveillance across West Africa, replacing fragmented systems. However, challenges such as limited real-time reporting, inadequate data collection and workforce shortages continue to impede disease control and outbreak response. The resurgence of infectious diseases like Ebola, cholera, COVID-19 and monkeypox highlights the need to strengthen IDSR systems for effective public health management. This article reviews IDSR implementation in West Africa, identifying persistent g

Contribution and influence of social capital on corruption in the health sector: a view through the lens of service users.

BACKGROUND: The interplay between social capital and health system corruption in healthcare systems is underexplored, yet vital, a major barrier to advancing universal health coverage is underexplored. While social capital can support anticorruption by enabling collective action, it may also perpetuate corruption when networks are used to bypass procedures or access care unfairly. This study explores how social capital contributes to corrupt practices complex relationships play out in practice in Nigerian health facilities. [Monkeypox and men who have sex with men: we need to treat it with a destigmatizing perspective.].

Monkeypox is a zoonosis, which is transmitted by direct and close contact, and has recently produced a large epidemic outbreak in non-endemic areas, classified as a Public Health Emergency of international concern by the World Health Organization. The global doubting and delayed response, and the stigmatizing approach to men who have sex with men promoted by public opinion, some scientists, socio-political agents, and the media, could be some of the reasons why the epidemic has not yet been contained. The stigma around the disease produces severe injuries to its victims and obstructs actions a

The cycle of distrust in health policy and behavior: Lessons learned from the Negev Bedouin.

BACKGROUND: Over the last decades, health systems worldwide have faced a decline in public trust. For marginalized minority populations, who generally suffer from poverty and political exclusion, the roots of this trend go much deeper, establishing a state of bi-directional distrust between them and health institutions. Although studied to a lesser extent compared to trust, distrust does impede health initiatives, such as infectious diseases prevention programs, mostly of so-called Neglected Zoonotic Diseases (NZDs). Where distrust prevails, even trust building actions such as defining rights

Public health intervention amidst conflict: Violence, politics, and knowledge frames in the 2018-20 Ebola epidemic in Democratic Republic of the Congo.

Research, policy, and donor interest in health systems in conflict environments has grown rapidly in recent years. The 2018-20 Ebola outbreak in Democratic Republic of the Congo is a critical case of healthcare militarization. The first-ever such outbreak in an active conflict zone, it grew notorious for violence against response teams, with attacks aggravating the spread of disease. However, while medical responders observed physical attacks, the causes of the violence remained largely unknown. Drawing on interviews and participant observation, we contribute civilian vantages of the way healt

Disseminated strongyloidiasis in an immunocompetent male: A Case Report.

BACKGROUND: In October 2023, Bangladesh introduced a free, single-dose human papillomavirus (HPV) vaccine for girls aged 9-14 through its national vaccination program to prevent cervical cancer, the second most common cancer among Bangladeshi females, caused by the HPV. Although vaccine hesitancy was not a significant issue before the COVID-19 pandemic, experiences from that pandemic and global literature suggest that the population's uptake of this vaccine may face barriers due to concerns related to reproductive health, fertility, and cultural and religious beliefs. This is particularly rele

Factors shaping political priorities for violence against women-mitigation policies in Sri Lanka.

BACKGROUND: Although violence against women (VAW) is a global public health issue, its importance as a health issue is often unrecognized in legal and health policy documents. This paper uses Sri Lanka as a case study to explore the factors influencing the national policy response to VAW, particularly by the health sector. METHODS: A document based health policy analysis was conducted to examine current policy responses to VAW in Sri Lanka using the Shiffman and Smith (2007) policy analysis framework. RESULTS: The findings suggest that the networks and influences of various actors in Sri Lan

Alabama's LePage decision, the future of ex vivo embryos, and more.

In February 2024 the Alabama Supreme Court held that the destruction of frozen embryos stored at an in vitro fertilization (IVF) center could be the basis of a wrongful death suit by the prospective parents under Alabama law. The opinion caused a bipartisan uproar that led to its effective overturning by the Alabama legislature and governor 19 days after it was issued. The Alabama Supreme Court's unexpected intervention in IVF shows the surprising ways a mishap in a clinic can trigger a collision of originalist jurisprudence, judicial rhetoric, political mobilization, and media amplification i

Firearm violence and racial capitalism: A conceptual framework for understanding the persistence and prevalence of gun violence in the United States.

Firearm violence is a leading cause of death in the United States and is marked by profound racial and socioeconomic disparities. Existing research has identified many of the conditions that make interpersonal firearm violence more likely, and intervention research shows that firearm regulation, targeted policing, and community-based programs can reduce harm. Yet firearm violence remains high, persistent, and unequally distributed. This article develops a racial capitalism framework to explain that persistence. Building on structural racism approaches, it asks how the conditions associated wit
