[Topical treatment for age-related macular degeneration: Where are we now?].

The therapeutic management of age-related macular degeneration (AMD) is a major public health issue. One of its two late forms, neovascular AMD, is currently treated by intravitreal injections of pharmaceutical active ingredients. Although it is very effective in treating pathologies of the posterior segment of the eye, the intravitreal route is not an ideal option for the long-term management of a chronic disease such as AMD. Indeed, in the literature, some authors even call it a "burden" for the practitioners, the patients and the healthcare system. Thus, consideration should be given to les

[The cases of penetrating eye injuries with large intraocular foreign body].

AIM: Three consecutive cases of penetrating eye injuries complicated with large (size 9-12 mm) intraocular foreign body are presented as such. PATIENTS AND METHODS: The initial findings, management, surgical procedures and final anatomical and functional outcomes of each particular case are given. Corneal entrance laceration was seen in all three patients. Second scleral full-thickness eye wall laceration was found just posterior to the horizontal muscle insertions in two eyes. Full-thickness exit posterior eye wall laceration obturated with foreign body was diagnosed in one case. This metall

The Evolution of Triamcinolone Acetonide Therapeutic Use in Retinal Diseases: From Off-Label Intravitreal Injection to Advanced Nano-Drug Delivery Systems.

Ophthalmic drug delivery to the posterior segment of the eye has been challenging due to the complex ocular anatomy. Intravitreal injection of drugs was introduced to deliver therapeutic doses in the posterior segment. Different posterior segment diseases including age-related macular degeneration, diabetic macular edema, retinal vein occlusions, uveitis, and cystoid macular edema, among others, have been historically treated with intravitreal corticosteroids injections, and more recently with intravitreal corticosteroids drug implants. Triamcinolone acetonide (TA) is the most frequently used

Can (Natural) deep eutectic systems increase the efficacy of ocular therapeutics?

The eye is one of the most complex organs in the human body, with a unique anatomy and physiology, being divided into anterior and posterior segments. Ocular diseases can occur in both segments, but different diseases affect different segments. Glaucoma and cataracts affect the anterior segment, while macular degeneration and diabetic retinopathy occur in the posterior segment. The easiest approach to treat ocular diseases, especially in the anterior segment, is through the administration of topical eye drops, but this route presents many constraints, namely precorneal dynamic and static ocula

[Value of immersion B-scan ultrasonography in vitreo-retinal pathology].

Diagnosis immersion B Scan ultrasonography is very useful to study anterior segment lesions of the eye (angle of iris, anterior synechiae, iris and ciliary body tumors). We studied forty patients with posterior segment lesions of the eye with this technique. We think that it is useful to study anterior vitreous (cyclitic membranes, vitreoretinal relationships in front of ora serrata). It is useful to study the posterior segment of the eye with this immersion technique, in case of high hyperopia, microphthalmos, phtysis bulbi with retinal detachment and to evaluate acute vitreous hemorrhages, v

Corneal Hydrops Secondary to Intrastromal Corneal Ring Intrusion into the Anterior Chamber 7 Years after Implantation: A Case Report.

INTRODUCTION: This report describes the first known case of late-onset intrusion of an intrastromal ring segment into the anterior chamber due to posterior migration of a ring segment. To our knowledge, intrastromal ring segment penetration has only been shown to occur in the intraoperative or immediate postoperative period. We postulate that the perforation in this case is due to progressive thinning of the cornea, possibly in combination with habitual eye rubbing. CASE REPORT: A 44-year-old man presented with acute corneal hydrops related to penetration of Descemet's membrane and endotheliu

Penetration Enhancers for Topical Drug Delivery to the Ocular Posterior Segment-A Systematic Review.

There is an unmet clinical need for eye drop formulations to efficiently treat the diseases of the posterior ocular segment by non-invasive topical administration. Here, we systematically reviewed the literature on ocular penetration enhancers and their ability to transfer drugs to the posterior segment of the eye in experimental studies. Our aim was to assess which penetration enhancer is the most efficient at delivering drugs to the posterior segment of the eye, when topically applied. We conducted a comprehensive search in three electronic databases (Ovid Embase, Ovid MEDLINE, and PubMed) t

[Sympathetic ophthalmia].

The authors give an account of a case of sympathetic ophthalmia which was successfully managed by reoperation of the injured eye, corticoids and immunosuppressive drugs. Sympathetic ophthalmia developed after perforation of the cornea along with traumatic cataract one month following the accident. It was manifested by irritation of the anterior segment of both eyes and marked exudation into the area of the posterior pole of the sympathizing eye. On the injured eye surgical reconstruction of the anterior segment was performed and concurrently broad spectrum antibiotics and corticoids were admin

Ocular leukaemia in acute lymphoblastic leukaemia of childhood.

Modern cytotoxic drugs increase the survival rate of children with Acute Lymphoblastic Leukaemia (ALL): But the eye is a pharmacological sanctuary and after clinical "cure" of the haematological disease the child may be blinded by leukaemic ocular infiltration. This is the history of a child who had had no recurrence of haematological leukaemia for four and a half years but following remission developed central nervous system (CNS) leukaemia followed by leukaemic iridocyclitis in the right eye after 20 months and in the left eye after three years three months from the initial diagnosis of ALL.

Ocular pathology in NeuroAIDS - An autopsy study.

The central nervous system (CNS) and the eye are involved in Human immunodeficiency virus related disease. Although, optic nerve is considered an extension of the CNS, it has not been systematically evaluated to determine if infections of brain can extend into the eye or vice versa. The brain and posterior compartment of eyeball retrieved at autopsy of patients succumbing to NeuroAIDS, were evaluated with Hematoxylin & Eosin, special stains and immunohistochemistry for infective pathogens. Multiplex PCR was performed in vitreous, CSF and serum for simultaneous detection of bacterial, viral, an

Vitrectomy in penetrating ocular trauma: an experimental study using rabbits.

To study the value and the proper timing of vitrectomy in cases of ocular trauma involving the posterior segment, 24 rabbit eyes were traumatized by introducing a steel foreign body and autogenous blood into the vitreous. Twelve eyes underwent vitrectomy, 6 in the first week after trauma ("early") and 6 in the fourth week ("late"). The other 12 eyes, which underwent removal of the foreign body but not vitrectomy, served as controls. Traction retinal detachment did not develop in the 6 early-vitrectomy eyes, but it did occur before vitrectomy in 4 of the 6 eyes of the late-vitrectomy group and

[Practical value of preventive posterior sclerectomy in trabeculectomy].

The results of prophylactic posterior sclerectomy, carried out to prevent ciliochoroid detachment in antiglaucoma fistulization surgery (trabeculectomy in various modifications) are analyzed. A total of 293 patients (303 eyes) were under study. In 51.8 percent of cases (157 eyes) trabeculectomy was combined with prophylactic sclerectomy, carried out both in the upper (121 eyes) and lower (36 eyes) segments of the eye. The results evidence that prophylactic posterior sclerectomy promoted a 2.3 times reduction (from 28.1 to 12.1 percent) in the incidence of ciliochoroid detachments; the site of

Drug delivery across the blood-retinal barrier: Biological gateways for posterior segment therapy.

Posterior-segment retinal diseases remain a leading cause of irreversible vision loss worldwide, yet effective treatment continues to be limited by the challenge of achieving sufficient and durable drug exposure within target ocular tissues. Although intravitreal injection has transformed the management of retinal diseases, therapeutic success is determined not only by drug potency but also by transport across retinal interfaces, intraocular distribution, and long-term tissue exposure. Traditionally, these challenges have been attributed primarily to the restrictive nature of the blood-retinal

[Research progress on biomechanical characteristics of the posterior eye and vitreoretinal diseases and optic neuropathy].

The occurrence and development of biological tissue diseases are closely related to their biomechanical properties. In recent years, significant achievements have been made in the research on the biomechanical properties of the cornea and sclera, and the research on the association between the biomechanics of the posterior eye and eye diseases has also been continuously advanced. This review systematically collates the research on the biomechanical characteristics of the retina, Bruch's membrane-choroid complex (BMCC), and optic nerve head. The retina is anisotropic, with the elastic modulus i

Triamcinolone-loaded self nano-emulsifying drug delivery systems for ocular use: An alternative to invasive ocular surgeries and injections.

Inflammation of the posterior segment of the eye is a severe condition and hard to cure as delivery of drugs to the inflammation site is inefficient. Currently, the primary treatment approach is ocular surgery or invasive ocular injections. Herein, we designed and developed a topically self nano-emulsifying drug delivery system (SNEDDs) to deliver triamcinolone acetonide (TCA) to the posterior segment of the eye. A screening based on TCA solubility was conducted on each excipient followed by preparation of various formulations using different ratios of the selected excipients. Vesicles of opti
