Brief Report: Cocaine Use and Pre-exposure Prophylaxis: Adherence, Care Engagement, and Kidney Function.

BACKGROUND: Concomitant use of cocaine and HIV pre-exposure prophylaxis (PrEP) raises important clinical questions around adherence, retention in care, and renal toxicity. METHODS: We assessed the associations of confirmed cocaine use with PrEP adherence (both ascertained through objective measures), care engagement, and renal function in the iPrEx open-label extension. Cocaine use was measured in scalp hair samples and categorized as light (500-3000 pg/mg) and moderate to heavy (>3000 pg/mg). PrEP adherence in the first 3 months was measured through plasma tenofovir concentrations. Disengage

Periconception pre-exposure prophylaxis to prevent HIV transmission: benefits, risks, and challenges to implementation.

HIV-serodiscordant couples face complicated choices between fulfilling reproductive desire and risking HIV transmission to their partners and children. Sexual HIV transmission can be dramatically reduced through artificial insemination and sperm washing; however, most couples cannot access these resources. We propose that periconception pre-exposure prophylaxis (PrEP) could offer an important, complementary therapy to harm reduction counseling programs that aim to decrease HIV transmission for couples who choose to conceive. In this paper, we describe the potential benefits of periconception P

Antiretroviral pre-exposure prophylaxis prevents vaginal transmission of HIV-1 in humanized BLT mice.

BACKGROUND: Worldwide, vaginal transmission now accounts for more than half of newly acquired HIV-1 infections. Despite the urgency to develop and implement novel approaches capable of preventing HIV transmission, this process has been hindered by the lack of adequate small animal models for preclinical efficacy and safety testing. Given the importance of this route of transmission, we investigated the susceptibility of humanized mice to intravaginal HIV-1 infection. METHODS AND FINDINGS: We show that the female reproductive tract of humanized bone marrow-liver-thymus (BLT) mice is reconstitu

Enhanced screening and bacterial sexually-transmitted infection diagnoses after HIV pre-exposure prophylaxis initiation.

BACKGROUND: Recipients of HIV pre-exposure prophylaxis (PrEP) experience higher rates of gonorrhea and chlamydia diagnoses than non-recipients. However, it is unclear if these observations reflect a causal relationship between PrEP initiation and acquisition of sexually transmitted infections (i.e., behavioral "risk compensation") or a diagnostic bias related to PrEP recipients screening more frequently than non-recipients. METHODS: We conducted a self-controlled case series study comparing rates of gonorrhea and chlamydia diagnoses after versus before PrEP initiation among commercially-insur

Pre-Exposure Prophylaxis-Related Messages Across Facebook, Instagram, and Twitter (Rebranded X): Content Analysis.

BACKGROUND: Interventions are needed to address the lack of pre-exposure prophylaxis (PrEP) awareness and mitigate barriers related to PrEP use. One such intervention modality is social media, as PrEP awareness and communication on issues, such as access and cost, are easily addressed via clear social media messages on platforms for PrEP-eligible people, and especially young people, who use social media more frequently. OBJECTIVE: This descriptive study seeks to extend understanding of PrEP awareness and usage by examining PrEP-related communication across 3 popular social media platforms (Fa

Healthcare and service provider perspectives on pre-exposure prophylaxis for HIV among young Australians.

BACKGROUND: The Australian Pharmaceutical Benefits Scheme was changed on 15 January 2021 to provide people aged <18 years access to government-subsidised pre-exposure prophylaxis (PrEP) for HIV. This study investigated healthcare and service providers' attitudes and practices in discussing PrEP and impacts on PrEP prescribing practices, and the differences in the provision of PrEP to young adults aged >18 years compared with people aged <18 years. METHODS: An online cross-sectional survey, conducted February to November 2023, used purposive, convenience and snowball sampling to recruit health

Pre-exposure prophylaxis engagement among sexual minority men in Taiwan: a behavioral health perspective.

In Taiwan, sexual minority men (SMM) account for a significant portion of new HIV cases, highlighting the need for effective prevention measures. However, pre-exposure prophylaxis (PrEP) use remains limited, necessitating a comprehensive examination of barriers. This study examined PrEP engagement and its predictors among 284 SMM in Taiwan. Participants were categorized into five stages, based on the Transtheoretical Model of Health Behavior Change, reflecting distinct levels of PrEP engagement. Ordinal logistic regression, informed by the Behavioral Model of Health Service Use, identified fac

Boosting HIV Pre-exposure Prophylaxis (PrEP) Prescribing Confidence in a Southern Family Medicine Residency.

INTRODUCTION: Pre-exposure prophylaxis (PrEP) is a highly effective pharmacologic strategy for preventing HIV infection, yet it remains underutilized, particularly in the southeastern United States and among adolescent populations. Family medicine physicians, especially those practicing in rural areas, are well-positioned to identify and manage PrEP-eligible patients, including adolescents and members of LGBTQ+ (lesbian, gay, bisexual, transgender, and queer) communities. However, many providers report inadequate training in sexual health, LGBTQ+ care, and HIV prevention. This study aimed to a

Lenacapavir-associated drug resistance: implications for scaling up long-acting HIV pre-exposure prophylaxis.

Although drug resistance could emerge if lenacapavir is initiated during undiagnosed acute infection or if infection occurs during the drug's pharmacokinetic tail, these cases will not compromise the effectiveness of WHO-recommended therapies, as there is no cross-resistance between lenacapavir and other licensed antiretroviral drugs. Lenacapavir pre-exposure prophylaxis (PrEP) is also unlikely to drive population-level lenacapavir resistance given the rarity of breakthrough infections and the reduced replication capacity of most lenacapavir-resistant variants, which most likely reduces their

Pre-Exposure prophylaxis knowledge, use and adherence among a US national sample of women.

Pre-exposure prophylaxis (PrEP) is a proven strategy for HIV prevention, yet uptake among women vulnerable to HIV remains low. This study draws on survey data and urine-based adherence testing from the American Women Assessing Risk Epidemiologically (AWARE) study to assess PrEP knowledge, use, and adherence in a behaviorally vulnerable sample. Despite high awareness of PrEP, actual use and willingness to consider PrEP were minimal. Objective adherence testing showed that most women who self-reported current PrEP use adhered to the daily regimen. Findings suggest that socioeconomic status, insu

Pre-Exposure Prophylaxis Cascade Outcomes Among Black Cisgender Women in the Southern United States.

Black cisgender women in the Southern United States experience disproportionate human immunodeficiency virus (HIV) rates. Pre-exposure prophylaxis (PrEP) is an effective HIV prevention tool but is underutilized in this population. We assessed PrEP cascade outcomes and correlates among Black women in the Southern United States. Between March and June 2022, we administered a cross-sectional online survey to Black cisgender women residing in the Southern United States who reported unprotected sex in the past 6 months and were negative for HIV by self-report. Participants provided information on s

Sexually transmitted infections among pre-exposure prophylaxis users in a Swedish multi-centre cohort.

An additional upsurge in bacterial STIs has been observed in Sweden following HIV pre-exposure prophylaxis (PrEP) implementation. From a prevention perspective, it is of relevance to optimize testing strategies within PrEP programmes to identify those most at risk. An open retrospective longitudinal observational cohort study was performed at three STI clinics in Uppsala, Gothenburg, and Malmö. A questionnaire and journal data regarding STI were collected from a sample of 199 MSM enrolled in the PrEP programmes and providing informed consent. Incident bacterial STIs during follow-up were analy

Persistence With Human Immunodeficiency Virus Pre-exposure Prophylaxis in an Active-Duty Military Population.

There is limited data on Human Immunodeficiency Virus Pre-exposure Prophylaxis (PrEP) use and persistence in the military. Despite universal access to care, there is concern that PrEP persistence may be lower in military populations due to logistical challenges and perceived stigma. This study evaluated the persistence rates as well as reasons for PrEP discontinuation in a military cohort. This study evaluated all active-duty service members who received PrEP between 2020 and 2022 at a large military infectious diseases clinic. All charts were examined to determine patient characteristics. Pat

Assessing Structural Racism and Discrimination Along the Pre-exposure Prophylaxis Continuum: A Systematic Review.

Structural racism and discrimination (SRD) is deeply embedded across U.S. healthcare institutions, but its impact on health outcomes is challenging to assess. The purpose of this systematic literature review is to understand the impact of SRD on pre-exposure prophylaxis (PrEP) care continuum outcomes across U.S. populations who could benefit from HIV prevention. Guided by PRISMA guidelines, we conducted a systematic review of the published literature up to September 2023 using PubMed and PsycInfo and included peer-reviewed articles meeting inclusion criteria. At least two authors independently

Incidence and risk factors associated with acquired syphilis in HIV pre-exposure prophylaxis users.

BACKGROUND: Acquired syphilis continues to affect millions of people around the world. It is crucial to study it in the context of HIV Pre-Exposure Prophylaxis (PrEP) to achieve the goals set out in the 2030 Agenda since the literature suggests increased risk behaviors for sexually transmitted infections. This study aimed to investigate the incidence and factors associated with acquired syphilis among PrEP users. MATERIALS AND METHODS: This retrospective cohort included data on PrEP users from all over Brazil from 2018 to 2020, retrieved from the national antiretroviral logistics system. We c

HIV pre-exposure prophylaxis initiation, persistence, and adherence during pregnancy through the postpartum period.

OBJECTIVE: We evaluated pre-exposure prophylaxis (PrEP) initiation, persistence, and adherence measured via tenofovir-diphosphate (TFV-DP) concentrations in dried blood spots (DBS) among women offered PrEP during pregnancy. METHODS: We prospectively analyzed data from participants in the PrIMA Study (NCT03070600) who were offered PrEP during the second trimester and followed through 9 months postpartum. At follow-up visits (monthly in pregnancy; 6 weeks, 6 months, 9 months postpartum), self-reported PrEP use was assessed, and DBS were collected for quantifying TFV-DP concentrations. RESULTS:

Vaginal microbial shifts are unaffected by oral pre-exposure prophylaxis in South African women.

Vaginal microbiota have been shown to be a modifier of protection offered by topical tenofovir in preventing HIV infection in women, an effect not observed with oral tenofovir-based pre-exposure prophylaxis (PrEP). It remains unclear whether PrEP can influence the vaginal microbiota composition. This study investigated the impact of daily oral tenofovir disoproxil fumarate in combination with emtricitabine for PrEP on the vaginal microbiota in South African women. At baseline, Lactobacillus iners or Gardnerella vaginalis dominant vaginal communities were observed in the majority of participant

Integrating pre-exposure prophylaxis of HIV infection into family planning services: a scoping review.

INTRODUCTION: The aim of this review was to map evidence of integrating pre-exposure prophylaxis (PrEP) for HIV prevention into family planning services. A comprehensive package, using a combination of PrEP and contraceptive delivery, could reduce the number of new HIV infections and/or unintended pregnancies for at-risk women and adolescent girls. METHODS: A scoping literature search was conducted between August 2020 and October 2020. After developing the review question, electronic databases (MEDLINE, Embase, Cochrane Library, Global Health, Web of Science) were systematically reviewed. All
