Successful renal transplantation is a highly effective endeavor that improves and prolongs the lives of patients with chronic kidney disease. Transplant surgery and immunosuppression carries risk and the demand for donor kidneys outstrips supply by far. These realities mandate thoughtful allocation and utilization of this limited resource to select candidates. As the criteria for candidates and donor grafts continue to expand, the field must adapt and seek new approaches. The complex process-from evaluation of candidates, transplant surgery, immunosuppression, and follow-up care after transpla

Antiplatelet therapy with P2Y12-receptor inhibitors has become the cornerstone of medical treatment in patients with acute coronary syndrome treated with percutaneous coronary intervention (PCI). With over 100 million prescriptions filled since its approval, clopidogrel is the most widely used P2Y12-receptor inhibitor. Dual antiplatelet therapy with clopidogrel plus aspirin has been associated with a lower rate of major cardiovascular events in patients after PCI than aspirin monotherapy. However, an alarmingly high number of clopidogrel-treated patients experience adverse thrombotic events. I

Opioid use disorder (OUD) is a chronic disease that remains difficult to treat, even with significant improvements in available medications. While current treatments work well for some, they often do not account for the unique needs of individual patients, leading to less-than-ideal results. Precision medicine offers a new path forward by tailoring treatments to fit each person's genetic, psychological, and social needs. This review takes a close look at medications for OUD, including methadone, buprenorphine, and naltrexone, as well as long-acting options that may improve adherence and conven

BACKGROUND AND AIMS: Precision, personalized or stratified medicine, which promises to deliver the right treatment to the right patient, is a topic of international interest in both the lay press and the scientific literature. A key aspect of precision medicine is the identification of biomarkers that predict the response to medications (i.e. pharmacogenetics). We examined why, despite the great strides that have been made in biomarker identification in many areas of medicine, only in oncology has there been substantial progress in their clinical implementation. We also considered why progress

BACKGROUND: Currently available pharmacotherapies for interstitial lung disease (ILD) are limited to systemic glucocorticoids, steroid-sparing immunosuppressive medications and antifibrotic medications. Each of these treatments target major components of ILD biology.
OBJECTIVE: This review presents the relevant evidence to the pharmacological management of ILD that is based on a treatable traits approach. We further highlight major knowledge gaps that should be the focus of future research that moves patient care toward a more precision-medicine approach.
SUMMARY OF REVIEW: The previous trea

Protein carbamylation is a nonenzymatic post-translational protein modification driven by urea-derived cyanate and its reactive form isocyanic acid. Because CKD is characterized in part by sustained urea retention and prolonged protein exposure to cyanate, it represents the prototypical human model of the systemic consequences of excess carbamylation. Advances in analytical chemistry and translational nephrology research have reshaped our understanding of carbamylation, transforming it from a biochemical curiosity into a central mechanism linking azotemia to clinical pathology across multiple

Precision medicine has revolutionized oncology by leveraging biomarkers for individualized therapy selection, early diagnosis, and real-time disease monitoring. However, the application of biomarkers in non-oncology therapeutic areas remains comparatively underdeveloped despite significant potential. This narrative review explores the evolving role of biomarkers across cardiovascular, renal, neurological, metabolic, and respiratory diseases, focusing on their clinical and regulatory contexts of use-diagnostic, prognostic, predictive, surrogate, safety, and pharmacodynamic/response. Key example

Thrombotic thrombocytopenic purpura (TTP) is a life-threatening thrombotic microangiopathy driven by severe ADAMTS13 deficiency with consequent ultra-large von Willebrand factor-mediated microthrombosis. Over the last two decades, understanding of the ADAMTS13-vWF axis has transformed management and made TTP a model for precision medicine. This narrative review (adult focus) synthesizes contemporary evidence on diagnosis, treatment, genetics, and emerging therapies in immune-mediated TTP. We summarize a pragmatic, biomarker-guided algorithm integrating early therapeutic plasma exchange and cor

Fecal microbiota transplantation (FMT) has evolved from an empirical remedy for recurrent Clostridioides difficile infection (rCDI) into a foundational platform for precision microbiome-based therapeutics. This comprehensive review details FMT's journey, analyzing its multifaceted mechanisms of action-including restoration of colonization resistance, metabolic reprogramming via short-chain fatty acids and bile acids, and profound immunomodulation-which extend far beyond simple microbial replacement. We critically evaluate its established, high efficacy in rCDI and its expanding, albeit more va

Pheochromocytomas and paragangliomas (PPGLs) are rare neuroendocrine tumors derived from chromaffin cells of the adrenal medulla and extra-adrenal paraganglia. Over the past two decades, the genomic characterization of PPGLs has profoundly transformed their diagnosis, classification, risk stratification, and therapeutic management. Up to 40% of PPGLs harbor germline pathogenic variants, the highest proportion among human neoplasms, and somatic driver events are identified in a substantial fraction of the remaining cases. Integrative multi-omic studies have established three main molecular clus

The poor solubility and bioavailability of antiretroviral drugs complicate the manage-ment of Human Immunodeficiency virus. The efficacy of these medications is diminished due to restricted absorption in the gastrointestinal tract. Patients often exhibit a wide range of reactions attributable to fluctuations in blood drug concentrations. Achieving the target plasma concentra-tions is challenging and often necessitates higher dosages, which increases the risk of adverse ef-fects. The formulation of pharmaceuticals with poor solubility is a complex and costly process that hinders overall drug de

Pancreatic ductal adenocarcinoma (PDAC) remains one of the most lethal malignancies with dismal 5-year survival rates. Characterized by its complex tumor microenvironment and resistance to conventional therapies, PDAC has historically been difficult to treat effectively. Recent advances in molecular profiling have revolutionized our understanding of the genetic landscape of pancreatic cancer. This review examines how molecular genetics is driving precision medicine approaches in pancreatic cancer, with particular focus on KRAS-targeted therapies. Understanding these developments is crucial for

Pharmacogenomics (PGx) has the potential to revolutionize hypertension management by tailoring antihypertensive therapy based on genetic profiles. Despite significant advances in genomic research, the clinical translation of PGx in hypertension remains challenging due to genetic complexity, variability in drug response, and implementation barriers. This review explores the genetic basis of hypertension, highlighting key pharmacogenomic markers that influence antihypertensive metabolism and efficacy, including CYP2D6, CYP3A4, UMOD, and ACE polymorphisms. We also examine the role of Mendelian ra

The complement cascade is an evolutionarily ancient system of soluble and cell-bound proteins. Activation of the system triggers sequential proteolytic steps that rapidly generate downstream protein fragments, several of which have biologic activity. The complement system can directly lyse invasive pathogens, and it engages other cellular and molecular components of the immune system. In some contexts, complement also has counterregulatory immune effects and can dampen inflammation. Beyond its role in immunity, the complement cascade facilitates the clearance of injured cells and debris and ca

The management of valvular heart disease has undergone a remarkable transformation over the past two decades, which was driven by the advent of catheter-based treatment methods. Whereas medical therapy was the only available treatment option for many older patients deemed unsuitable for conventional surgery until the early 2000s, a wide range of interventional therapies is now available. Transcatheter aortic valve replacement and mitral valve transcatheter edge-to-edge repair evidently provide prognostic advantages over medical therapy for inoperable patients with severe aortic stenosis and se
