Functional dystonia: A case-control study and risk prediction algorithm.

OBJECTIVE: Functional dystonia (FD) is a disabling and diagnostically challenging functional movement disorder (FMD). We sought to identify historical predictors of FD vs. other primary dystonias (ODs) and develop a practical prediction algorithm to guide neurologists. METHODS: 1475 consecutive new patient medical records were reviewed at an adult/pediatric tertiary-referral dystonia clinic from 2005 to 2017. Ninety-nine met criteria for clinically established FD (85 adults and 14 pediatric), paired with 99 age/dystonia distribution-matched OD. Univariate and multivariate regression analyses

Using algorithms to address trade-offs inherent in predicting recidivism.

Although risk assessment has increasingly been used as a tool to help reform the criminal justice system, some stakeholders are adamantly opposed to using algorithms. The principal concern is that any benefits achieved by safely reducing rates of incarceration will be offset by costs to racial justice claimed to be inherent in the algorithms themselves. But fairness trade-offs are inherent to the task of predicting recidivism, whether the prediction is made by an algorithm or human. Based on a matched sample of 67,784 Black and White federal supervisees assessed with the Post Conviction Risk A

A learning algorithm for predicting mental health symptoms and substance use.

Learning health systems use data to generate knowledge that informs clinical care, but few studies have evaluated how to leverage patient-reported mental health symptoms and substance use data to make patient-specific predictions. We developed a general Bayesian prediction algorithm that uses self-reported psychiatric symptoms and substance use within a population to predict future symptoms and substance use for individuals in that population. We validated our approach in 2444 participants from two clinical cohorts - the National Network of Depression Centers and the Johns Hopkins HIV Clinical

The Univariate Flagging Algorithm (UFA): An interpretable approach for predictive modeling.

In many data classification problems, a number of methods will give similar accuracy. However, when working with people who are not experts in data science such as doctors, lawyers, and judges among others, finding interpretable algorithms can be a critical success factor. Practitioners have a deep understanding of the individual input variables but far less insight into how they interact with each other. For example, there may be ranges of an input variable for which the observed outcome is significantly more or less likely. This paper describes an algorithm for automatic detection of such th

Assessment of a Statistical Algorithm for the Prediction of Benign Paroxysmal Positional Vertigo.

IMPORTANCE: Benign paroxysmal positional vertigo (BPPV) is an otologic pathologic condition defined as a sensation of spinning triggered by changes in head position relative to gravity and caused by an entrapment of fragmented endolymph debris most commonly in the posterior semicircular canal. Confirmation of diagnosis requires experience with procedures that are poorly known by those other than practitioners with advanced otologic training. The complexity in the diagnosis of BPPV inspired the design of a questionnaire-based algorithm that would be useful for determining a vestibular diagnosis

Prediction Algorithm for Surgical Intervention in Neonatal Brachial Plexus Palsy.

BACKGROUND: Neonatal brachial plexus palsy (NBPP) results in reduced function of the affected arm with profound ramifications on quality of life. Advances in surgical technique have shown improvements in outcomes for appropriately selected patients. Patient selection, however, remains difficult. OBJECTIVE: To develop a decision algorithm that could be applied at the individual patient level, early in life, to reliably predict persistent NBPP that would benefit from surgery. METHODS: Retrospective review of NBPP patients was undertaken. Maternal and neonatal factors were entered into the C5.0

Validation of a Psychological Screening Algorithm for Predicting Spine Surgery Outcomes.

It is well established that psychological factors can affect spine surgery results. A widely utilized presurgical psychological screening (PPS) algorithm that combines established psychosocial risk factors to determine patients' likelihood of achieving successful spine surgery results is evaluated in this study. Patients (n = 603) underwent a PPS, including a diagnostic interview, psychometric testing, and review of medical records. The evaluator completed a PPS algorithm for patients, placing them into one of five prognosis categories, ranging from excellent to poor. For analysis purposes, th

Evaluation of an Algorithm to Predict Menstrual-Cycle Phase at the Time of Injury.

CONTEXT: Women are 2 to 8 times more likely to sustain an anterior cruciate ligament (ACL) injury than men, and previous studies indicated an increased risk for injury during the preovulatory phase of the menstrual cycle (MC). However, investigations of risk rely on retrospective classification of MC phase, and no tools for this have been validated. OBJECTIVE: To evaluate the accuracy of an algorithm for retrospectively classifying MC phase at the time of a mock injury based on MC history and salivary progesterone (P4) concentration. DESIGN: Descriptive laboratory study. SETTING: Research l

Development of the Tardivo Algorithm to Predict Amputation Risk of Diabetic Foot.

Diabetes is a chronic disease that affects almost 19% of the elderly population in Brazil and similar percentages around the world. Amputation of lower limbs in diabetic patients who present foot complications is a common occurrence with a significant reduction of life quality, and heavy costs on the health system. Unfortunately, there is no easy protocol to define the conditions that should be considered to proceed to amputation. The main objective of the present study is to create a simple prognostic score to evaluate the diabetic foot, which is called Tardivo Algorithm. Calculation of the s

OBJECTIVE: The aim of this study is to develop an on-scene injury severity prediction (OSISP) algorithm for truck occupants using only accident characteristics that are feasible to assess at the scene of the accident. The purpose of developing this algorithm is to use it as a basis for a field triage tool used in traffic accidents involving trucks. In addition, the model can be valuable for recognizing important factors for improving triage protocols used in Sweden and possibly in other countries with similar traffic environments and prehospital procedures. METHODS: The scope is adult truck o

A computationally efficient algorithm for genomic prediction using a Bayesian model.

BACKGROUND: Genomic prediction of breeding values from dense single nucleotide polymorphisms (SNP) genotypes is used for livestock and crop breeding, and can also be used to predict disease risk in humans. For some traits, the most accurate genomic predictions are achieved with non-linear estimates of SNP effects from Bayesian methods that treat SNP effects as random effects from a heavy tailed prior distribution. These Bayesian methods are usually implemented via Markov chain Monte Carlo (MCMC) schemes to sample from the posterior distribution of SNP effects, which is computationally expensiv

Prediction of epitopes in closely related proteins using a new algorithm.

Latrophilin 1 (presynaptic receptor) binds α-latrotoxin from black widow spider venom and regulates neurotransmitter release from nerve endings. The study of the mechanism of action of this receptor is impeded by the existence of closely related latrophilins 2 and 3. A profile of differences detecting the most differing and identical sites in several proteins was developed in order to obtain highly specific antibodies for differentiation between isoforms of related proteins. In addition, we used an algorithm for prediction of immunogenic sites of the protein, based on the basic vector method.

A general sample size framework for developing or updating a predictive algorithm: with application to clinical prediction models.

BACKGROUND: When developing or updating a predictive algorithm to make predictions (e.g., risk estimates) in individuals, the sample size of the development (training) dataset is an important consideration. Small datasets reduce a model's predictive performance, generalisability and fairness, and may lead to harm. To help identify (minimum) sample sizes required for model development or updating, various sample size calculations exist but their underlying theory is based on standard (unpenalised) regression and focus on minimising overfitting. To address this, we propose a more general approac

Clinical applicability of an artificial intelligence prediction algorithm for early prediction of non-persistent atrial fibrillation.

BACKGROUND AND AIMS: It is difficult to document atrial fibrillation (AF) on ECG in patients with non-persistent atrial fibrillation (non-PeAF). There is limited understanding of whether an AI prediction algorithm could predict the occurrence of non-PeAF from the information of normal sinus rhythm (SR) of a 12-lead ECG. This study aimed to derive a precise predictive AI model for screening non-PeAF using SR ECG within 4 weeks. METHODS: This retrospective cohort study included patients aged 18 to 99 with SR ECG on 12-lead standard ECG (10 seconds) in Ewha Womans University Medical Center for 3

The Ottawa SAH search algorithms: protocol for a multi- centre validation study of primary subarachnoid hemorrhage prediction models using health administrative data (the SAHepi prediction study protocol).

BACKGROUND: Conducting prospective epidemiological studies of hospitalized patients with rare diseases like primary subarachnoid hemorrhage (pSAH) are difficult due to time and budgetary constraints. Routinely collected administrative data could remove these barriers. We derived and validated 3 algorithms to identify hospitalized patients with a high probability of pSAH using administrative data. We aim to externally validate their performance in four hospitals across Canada. METHODS: Eligible patients include those ≥18 years of age admitted to these centres from January 1, 2012 to December 3

Prediction of Ligand Binding to Transthyretin Using Machine Learning Algorithms and Low-Dimensional Molecular Descriptors: A Tox24 Challenge Study.

This study presents a comprehensive machine learning approach for predicting the percent displacement of ANSA from human transthyretin (TTR) at fixed assay conditions as defined in the Tox24 Challenge. TTR is a critical serum transport protein for thyroid hormones, and its disruption by environmental chemicals can lead to endocrine system dysregulation with serious developmental and metabolic consequences. However, the scarcity of large, chemically diverse data sets and the lack of standardized experimental protocols have limited the computational prediction of TTR binding, restricting the dev

How to use learning curves to evaluate the sample size for malaria prediction models developed using machine learning algorithms.

BACKGROUND: Machine learning algorithms have been used to predict malaria risk and severity, identify immunity biomarkers for malaria vaccine candidates, and determine molecular biomarkers of antimalarial drug resistance. Developing these prediction models requires large training datasets to ensure prediction accuracy when applied to new individuals in the target population. Learning curves can be used to assess the sample size required for the training dataset by evaluating the predictive performance of a model trained using different dataset sizes. These curves are agnostic to the specific p

An individualized risk prediction tool for ectopic pregnancy within the first 10 weeks of gestation based on machine learning algorithms.

BACKGROUND: As the main cause of maternal deaths in early pregnancy, delayed diagnosis of ectopic pregnancy (EP) may lead to severe consequences. Patients with pregnancy of unknown location (PUL) exhibit a significantly higher incidence of EP and associated risks compared to the general population. Therefore, this study aims to construct an early prediction model to identify EP risk among patients with PUL and provide a valuable direction for clinical intervention. METHODS: Retrospectively recruited 1896 patients with PUL within 10 weeks of gestation. Feature selection was done using the leas
