Timing of operation for chronic aortic regurgitation.

Left ventricular systolic function is an important determinant of long-term prognosis in patients with chronic aortic regurgitation. Data from several centers, using invasive and noninvasive assessment of left ventricular function, indicate that long-term postoperative survival is excellent, even in symptomatic patients, if preoperative left ventricular systolic function is normal. The long-term postoperative results are significantly worse in symptomatic patients with preoperative left ventricular systolic dysfunction, many of whom appear to have irreversible left ventricular failure before t

Tongue pressure, respiratory muscle and limb strength and functional exercise capacity in oesophageal cancer.

OBJECTIVES: Preoperative management of oesophageal cancer requires effective prehabilitation. This study investigates the relationship between tongue pressure, respiratory muscle, handgrip and lower limb muscle strengths, and functional exercise capacity in patients with oesophageal cancer to develop a novel prehabilitation strategy. METHODS: This prospective, single-centre, observational study analysed data from 29 pretreatment patients with oesophageal cancer. We evaluated tongue pressure, maximal inspiratory and expiratory pressures, handgrip and lower limb muscle strengths, functional exe

Effects of exercise training in people with non-small cell lung cancer before lung resection: a systematic review and meta-analysis.

INTRODUCTION: Exercise training before lung resection for non-small cell lung cancer is believed to decrease postoperative complications (POC) by improving cardiorespiratory fitness. However, this intervention lacks a strong evidence base. AIM: To assess the effectiveness of preoperative exercise training compared with usual care on POC and other secondary outcomes in patients with scheduled lung resection. METHODS: A systematic search of randomised trials was conducted by two authors. Meta-analysis was performed, and the effect of exercise training was estimated by risk ratios (RR) and mean

Multimodal prehabilitation before lung resection surgery: a multicentre randomised controlled trial.

BACKGROUND: Respiratory muscle training may improve ventilatory efficiency (VE/VCO2 slope), a strong predictor of postoperative pulmonary complications. We hypothesised that multimodal prehabilitation, incorporating high-intensity respiratory muscle training, before lung resection would reduce postoperative complications and length of hospital stay. METHODS: We conducted a prospective multicentre, randomised controlled trial (NCT04826575) to examine the effect of prehabilitation in individuals undergoing lung resection. Participants were defined as high-risk for postoperative pulmonary compli

Multimodal prehabilitation and perioperative immune function in patients undergoing abdominal cancer surgery.

BACKGROUND: Prehabilitation is an emerging preoperative strategy designed to optimize patients' functional capacity before surgery to improve postoperative outcomes. Previous studies have demonstrated its clinical benefit, including enhanced recovery and reduced postoperative complications. However, the mechanisms underlying these benefits remain poorly understood. The aim of this exploratory study was to investigate the potential effects of prehabilitation on preoperative and postoperative immune function. METHODS: This prospective study utilized data and material from a subgroup of patients

Gynecologic ERAS Preoperative Interventions.

Enhanced recovery after surgery (ERAS) protocols have been widely adopted within gynecologic surgery to optimize perioperative outcomes. This chapter discusses the evidence behind preoperative gynecologic ERAS elements, including preadmission counseling, comorbidity optimization, fasting guidelines, preoperative medications, and prehabilitation. Much of the evidence is extrapolated from colorectal and other surgical populations but has been supported within gynecology. Prehabilitation is the newest element, aimed at improving preoperative functional status through exercise, nutrition, and psyc

Swimming Exercise Pretreatment Attenuates Postoperative Delirium-Like Behavior in Type 2 Diabetic Rats by Enhancing Mitochondrial Biogenesis Through Activation of SIRT2 Deacetylation.

Postoperative delirium (POD) is a common postsurgical complication that seriously affects patients' prognosis and imposes a heavy burden on families and society. Type 2 diabetes mellitus (T2DM) is a major risk factor for POD. The susceptibility mechanisms of POD in T2DM individuals and the role of exercise preconditioning remain unclear. Adult rats with and without T2DM were used to assess the promotive effect of diabetes on postoperative delirium-like behavior. The diabetic rats were also subjected to a swimming exercise program before surgery. The potential beneficial effect of exercise prec

Multimodal Prehabilitation for Gynecologic Cancer Surgery.

Surgical treatment is commonly employed to treat patients with gynecologic cancer, although surgery itself may function as a stressor, reducing the patients' functional capacity and recovery. Prehabilitation programs attempt to improve patients' overall health and baseline function prior to surgery, thereby enhancing recovery and lowering morbidity. In recent years, prehabilitation has come to primarily refer to multimodal programs that combine physical activity, nutritional support, psychological well-being, and other medical interventions. However, the specific methods of implementing prehab

The Wessex Fit-4-Cancer Surgery Trial (WesFit): a protocol for a factorial-design, pragmatic randomised-controlled trial investigating the effects of a multi-modal prehabilitation programme in patients undergoing elective major intra-cavity cancer surgery.

Background: Surgical resection remains the primary curative treatment for intra-cavity cancer. Low physical fitness and psychological factors such as depression are predictive of post-operative morbidity, mortality and length of hospital stay. Prolonged post-operative morbidity is associated with persistently elevated risk of premature death. We aim to investigate whether a structured, responsive exercise training programme, a psychological support programme or combined exercise and psychological support, delivered between treatment decision and major intra-cavity surgery for cancer, can reduc

Effects of perioperative exercise interventions on lung cancer patients: An overview of systematic reviews.

AIMS AND OBJECTIVES: To identify, appraise and summarise systematic reviews of exercise interventions for surgical lung cancer patients. BACKGROUND: Low exercise capacity, reduced pulmonary function, impaired health-related quality of life and postoperative pulmonary complications are common in surgical lung cancer patients. Numerous systematic reviews address these health problems and examine the effects of exercise intervention. However, differences in the quality and scope of the systematic reviews and discordant findings from the reviews make it difficult for decisions-makers to interpret

Improvement of severely reduced left ventricular function after surgical revascularization in patients with preoperative myocardial infarction.

In recent years, two pathophysiological conditions--stunned and hibernating myocardium--have been described showing how function may be depressed in myocardium that remains viable. The aims of the present study were postoperative assessment of LV function at rest and during exercise after CABG in patients with established previous myocardial infarction and impaired preoperative LV function and evaluation of preliminary experience with positron emission tomography (PET) in the preoperative identification of reversible ischaemic myocardium and its predictivity in postoperative functional improve

Computed tomography-derived muscle quality and density and the risk of postoperative decline in exercise capacity in adults who received cardiac surgery: A retrospective cohort study.

BACKGROUND: Cardiovascular surgery significantly improves outcomes, yet many, particularly older or frail individuals, face substantial postoperative declines in exercise capacity, prolonging recovery and worsening long-term prognosis. Early, precise risk identification is critical. Routinely performed preoperative computed tomography (CT) offers a pragmatic opportunity to assess muscle quality like psoas muscle density (PMD), reflecting physiological resilience. This study aimed to examine the predictive capabilities of CT-derived muscle mass and quality for postoperative exercise capacity de

Left ventricular ischemia due to coronary stenosis as an unexpected treatable cause of paroxysmal atrial fibrillation.

We present a patient with exercise-induced paroxysmal atrial fibrillation who was eventually scheduled for a Cox-maze operation due to persistence of his complaints of fatigue, impaired exercise tolerance, and predominantly exercise-related irregular palpitations despite treatment with several antiarrhythmic drugs. A preoperative exercise stress test without antiarrhythmic or negative chronotropic drugs, however, showed clear evidence of myocardial ischemia. After coronary angioplasty of a significant stenosis in the left anterior descending artery, there was no recurrence of atrial fibrillati

Prehabilitation during oesophageal cancer neoadjuvant chemotherapy and postoperative functional exercise capacity.

OBJECTIVE: To investigate whether initiating prehabilitation during neoadjuvant chemotherapy (NAC) is associated with postoperative recovery of functional exercise capacity in patients with oesophageal cancer. METHODS: In this multicentre observational study, we classified patients with oesophageal cancer into two groups based on the timing of prehabilitation: the early prehab (initiated during NAC) and late prehab (initiated after NAC) groups. Prehabilitation consisted mainly of an exercise programme combining supervised sessions with prescribed self-exercise. Functional exercise capacity wa

Feasibility, tolerability, and effects of exercise-based prehabilitation after neoadjuvant therapy in esophagogastric cancer patients undergoing surgery: an interventional pilot study.

Patients requiring surgery for locally advanced esophagogastric cancer often require neoadjuvant therapy (NAT), which may have a detrimental impact on cardiorespiratory reserve. The aims of this study were to investigate the feasibility and tolerability of a 5-week preoperative high-intensity interval training program after NAT, and to assess the potential effects of the training protocol on exercise capacity, muscle function, and health-related quality of life (HRQL). We prospectively studied consecutive patients with resectable locally advanced esophageal and gastric cancer in whom NAT was p
