Ethical, Legal, and Administrative Considerations for Preparticipation Evaluation for Wilderness Sports and Adventures.

Preparticipation evaluations (PPEs) are common in team, organized, or traditional sports but not common in wilderness sports or adventures. Regarding ethical, legal, and administrative considerations, the same principles can be used as in traditional sports. Clinicians should be trained to perform such a PPE to avoid missing essential components and to maximize the quality of the PPE. In general, participants' privacy should be observed; office-based settings may be best for professional and billing purposes, and adequate documentation of a complete evaluation, including clearance issues, shou

Stimulating ambulance specialist nurse students' ethical reflections by high-fidelity simulation.

Introduction: Ethical competence in professional practice can be considered essential among nurses and nurses in ambulance care encounter ethical dilemmas frequently. To enhance ethical competence among students in the ambulance specialist nursing program, high-fidelity simulation scenarios including ethical dilemmas were introduced as a learning activity. Research aim: The research aim was to investigate the usefulness of high-fidelity simulation in ambulance specialist nurse education to teach ethical reasoning when caring for children. Research design: This study was conducted as a qualitat

Counterfactual harm: a counter-argument.

As artificial intelligence systems are increasingly used to guide decisions, it is essential that they follow ethical principles. A core principle in medicine is nonmaleficence, often equated with "do no harm". A formal definition of harm based on counterfactual reasoning has been proposed and popularized. This notion of harm has been promoted in simple settings with binary treatments and outcomes. Here, we highlight a problem with this definition in settings involving multiple treatment options. Illustrated by an example with 3 tuberculosis treatments (say, A, B, and C), we demonstrate that t

Ethical harms for migrant 24h caregivers in home care arrangements.

The glaring lack of formal and informal caregivers in Germany has not only become apparent in hospitals and nursing homes but also in home care arrangements. One tension is particularly pertinent in such arrangements: a 'family-oriented' logic of the long-term care insurance and the individual wishes of those in need of care meet the actual possibilities of family carers. This care gap has been compensated for by 24-hour care workers, so-called 'live-ins', from Eastern Europe for some years. This contribution maps the 'live-ins' situation comprehensively from an ethical perspective. Based on d

Partnering With Generative Artificial Intelligence for Quantitative Data Analysis in Nursing Research: A Practical Workflow for Ethical Collaboration.

Reproducibility and replicability are our scientific and ethical mandate, and the foundation of trust in nursing science and research. Both rest on transparent documentation of how data were actually analyzed. The rising use of generative artificial intelligence (GenAI) in research intersects with this mandate, with the potential to either support or undermine it, yet practical guidance for conducting GenAI-assisted quantitative analysis remains limited. This gap is particularly relevant for nurse scientists, who increasingly work with complex, biologically-informed datasets spanning genomics,

Uterus transplantation: an ethical analysis.

Uterus transplantation (UTx) represents an emerging therapeutic option for women with absolute uterine-factor infertility (AUFI) and raises complex ethical questions as a non-life-saving intervention. This study presents an ethical analysis based on the four principles of Beauchamp and Childress-autonomy, non-maleficence, beneficence, and social justice-and is grounded in a structured narrative literature review of current medical and ethical publications, drawing on the German legal context as an illustrative example. Uterus transplantation demonstrates potential benefits, particularly in ter

Uterus transplant and controversial assisted reproductive technologies: can the right to parenthood clash with ethical precepts?

Fast-developing assisted reproductive techniques based on innova- tive technologies have given rise to incredibly consequential ethical challenges of unprecedented scope. The development of innovative assisted reproductive technologies (ARTs) has steadily risen since the late 1970s. Until just over 20 years ago, those who sought to become parents had no way to overcome their infertility or sterility issues, but can now achieve parenthood and have children with at least a partial genetic and/or biological relationship. Treatments aimed at ovarian stimulation, artificial insemination, either by

The ethics of neurologically complicated pregnancies.

Bioethical conflicts in pregnancy are distinguished from those in other areas of medicine due to competing interests between mother and fetus because of their shared biology. Historically, prior to the advent of fetal therapy and advances in medical technology, the maternal-fetal complex was considered to be a single entity. With advances in medicine, treatment options can now be directed at both the mother and the fetus, and a duality has evolved in the maternal-fetal unit. Thus at some point during pregnancy, two individuals rather than just one are the responsibility of the physician. In de

Convergences and divergences in nursing ethics across the European Union: A qualitative comparative analysis of national codes.

BACKGROUND: Nursing ethics guides practice, but European Union national codes were developed in diverse socio-legal contexts, generating heterogeneous emphases. PURPOSE: To compare European Union nursing codes of ethics, mapping convergences, divergences, and emerging ethical dimensions. METHODS: Qualitative documentary analysis with thematic content analysis examined 22 European Union member-state codes plus the European Nursing Council and International Council of Nurses (ICN) codes, with input from nursing, qualitative, and health-law experts. DISCUSSION: Fourteen themes were identified:

Ethical Considerations in Critically Ill Neonatal and Pediatric Patients.

The care of critically ill neonates and pediatric patients can be particularly emotionally and ethically challenging. Emerging evidence suggests that we can improve the patient, family, and care team experience in the critical care setting through a better understanding and application of ethical frameworks and communication strategies. We conducted a multidisciplinary panel session at the American Academy of Pediatrics National Conference and Exhibition in the fall of 2022 wherein we explored a myriad of ethical and communication considerations in this unique patient population, with congenit

Recommendations for collaborative paediatric research including biobanking in Europe: a Single Hub and Access point for paediatric Rheumatology in Europe (SHARE) initiative.

Innovative research in childhood rheumatic diseases mandates international collaborations. However, researchers struggle with significant regulatory heterogeneity; an enabling European Union (EU)-wide framework is missing. The aims of the study were to systematically review the evidence for best practice and to establish recommendations for collaborative research. The Paediatric Rheumatology European Single Hub and Access point for paediatric Rheumatology in Europe (SHARE) project enabled a scoping review and expert discussion, which then informed the systematic literature review. The ethical options in transplanting fetal tissue.

Fetal tissue transplants have now been successful in primates, raising the possibility of treatment for Parkinson's disease and other chronic illnesses. Whether or not abortion is morally justified, use of human fetal tissue for research or therapy is justified in certain circumstances. The rationale, both for permitting transplantation of fetal tissue and for limitations in exercising the technology, is based on the same set of ethical principles that supported restrictive legislation in the past: respect for autonomy and a balancing of harms and benefits that gives priority to those most aff

Performance-based payment models and value-based health care: a semi-systematic review through theoretical lenses.

OBJECTIVES: To identify and critically examine the theoretical lenses applied in the literature to conceptualize performance-based and value-based payment models, and to analyze how these frameworks explain their behavioral, organizational, and system-level effects. METHODS: A semi-systematic review was conducted in July 2025 across Web of Science, PubMed/MEDLINE, Embase, LILACS, and CINAHL, using a structured search strategy and transparent selection procedures following PRISMA 2020 reporting principles. Eligible empirical and theoretical studies on value- or performance-based payment were e

From Principles to Practice: Implementing Housing First in the Primer la Llar Program (Barcelona).

PURPOSE: The Housing First model has established itself as a benchmark in addressing chronic homelessness, yet little attention has been paid to how its principles are being applied in everyday practices. This study analyzes its implementation in the Primer la Llar program in Barcelona, focusing on how teams understand and bring these principles into their daily work. MATERIALS AND METHODS: A longitudinal qualitative study was conducted between 2016 and 2020. Data were collected in three phases through interviews with professionals involved in the program, including local government staff, as

Combining value of information analysis and ethical argumentation in decisions on participation of vulnerable patients in clinical research.

BACKGROUND: The participation of vulnerable patients in clinical research poses apparent ethical dilemmas. Depending on the nature of the vulnerability, their participation may challenge the ethical principles of autonomy, non-maleficence, or justice. On the other hand, non-participation may preclude the building of a knowledge base that is a prerequisite for defining the optimal clinical management of vulnerable patients. Such clinical uncertainty may also incur substantial economic costs. MAIN TEXT: We present the participation of pre-menopausal women with atrial fibrillation in trials of n

[Problems of Doctor Judym's peers].

This paper is a work on the health conditions of the inhabitants of Rzeszów and the surrounding area in the second half of the 19th century as based on the mortality analysis. A comparison and contrast has been made between the daily work of a doctor nowadays and a hundred years ago. The issues have been presented at various levels, with reference to the political, socio-economic and cultural relations existing at that time. The work is enhanced with additional comments on the doctor's profession and the problem of diagnostic difficulties as well as the possibility of making a mistaken judgeme

Rethinking restraint in psychiatric services: A Kantian ethical and legal analysis.

The use of restraint in psychiatric care remains one of the most ethically contested practices in mental health services, reflecting an enduring tension between the duty to ensure safety and the obligation to respect patient autonomy. Although coercive interventions are often justified as necessary responses to risk, their ethical legitimacy remains uncertain, particularly when examined through deontological frameworks. This theoretical paper provides a philosophical and legal analysis of restraint practices, drawing on the ethical theory of Immanuel Kant to critically evaluate the moral bound
