Thyrotropin-releasing hormone: does two hundred micrograms provide effective stimulation to the preterm fetal pituitary gland compared with four hundred micrograms?

OBJECTIVE: Our purpose was to compare the response of the fetal pituitary-thyroid axis to 200 and 400 micrograms of thyrotropin-releasing hormone administered to the mother immediately before delivery with a control group. STUDY DESIGN: A randomized controlled trial was conducted of 26 women at gestational ages between 24 weeks and 33 weeks 6 days who had received one or more doses of betamethasone who were expected to be delivered within 1 to 4 hours. Women received either 200 or 400 micrograms of thyrotropin-releasing hormone or were in the control group. RESULTS: Thyroid-stimulating hormo

Endocrine and reproductive dysfunction following fractionated total body irradiation in adults.

The endocrine and reproductive sequelae of total body irradiation for haematological malignancy have been studied in 21 patients (11 male) who were treated with 10 Gy in five fractions or 12 or 13.2 Gy in six fractions over 3 days. Eighteen patients (eight male) aged 16-49 years underwent dynamic tests of the hypothalamic-pituitary axis with insulin hypoglycaemia, thyrotrophin releasing hormone (TRH) and gonadotrophin releasing hormone stimulation and basal measurement of prolactin, sex steroids and thyroid hormones. Growth hormone responses (mean peak 64 +/- 36 mU/l, range 21-146 mU/l) and co

Pituitary responses to synthetic luteinizing hormone-releasing hormone in thirty-four cases of amenorrhea or oligomenorrhea associated with galactorrhea.

Pituitary responses to 100 mcg. of luteinizing hormone-releasing hormone (LH-RH) administered subcutaneously were studied in 34 cases of amenorrhea or anovulatory oligomenorrhea associated with galactorrhea. Twenty-six patients had pituitary prolactin-secreting tumors (group I); eight patients had a normal sella turcica and remission of the syndrome either spontaneously or after thyroid replacement therapy (group 2). Follicle-stimulating hormone (FSH) and luteinizing hormone (LH) responses to LH-RH were variable in each group of patients, ranging from poor to exaggerated, and no statistically

[Galactorrhea].

Galactorrhea syndromes are mainly caused by hyperprolactinemia, which has been defined by the basal prolactin level more than 15 ng/ml. However, normoprolactinemia can not be proved only by the basal prolactin level less than 15 ng/ml, which required the assessment of prolactin secreting capacity. Occulted hyperprolactinemia has be well known as the same syndrome as hyperprolactinemia, which shows basal prolactin level less than 15 ng/ml and the exceed response of prolactin to prolactin secreting stimulation like as thyroid releasing hormone. Women with occulted hyperprolactinemiais show tempo

Serum prolactin levels in patients with fibrocystic breast disease.

In 193 patients suffering from fibrocystic breast disease, basal serum prolactin concentrations were determined and compared to serum prolactin levels in 193 healthy women. In 45 additional patients and 23 healthy control subjects, a thyrotropin-releasing hormone (TRH) stimulation test was performed. The response to TRH in seven healthy female volunteers and in one patient with fibrocystic breast disease, was correlated with the mean serum prolactin levels over 24 hours. Serum prolactin levels were above normal in 45.6% of the patients and in 21.2% of the control subjects. Mean values of the t

Tests of prolactin secretion in diagnosis of prolactinomas.

Prolactin-secreting tumours of the pituitary were identified and treated by transsphenoidal microsurgery in fourteen infertile females with hyperprolactinaemia. Resting prolactin levels were 590--9000 mU/1 (mean 3400). In seven patients, tomography of the pituitary fossa was normal and resting prolactin levels were 590-6000 mU/1 (mean 3400). In these patients the pre-operative diagnosis prolactinoma in these patients was made by demonstrating loss of the normal circadian prolactin profile and impaired prolactin response to intravenous thyrotrophin-releasing hormone (T.R.H.) and metoclopramide

Ca2+ and hormones interact synergistically to stimulate rapidly both prolactin production and overall protein synthesis in pituitary tumor cells.

Effects of Ca2+ and hormones on short-term protein synthesis were examined utilizing intact Ca2+-depleted and Ca2+-restored GH3 pituitary tumor cells as a model system. Amino acid incorporation by cells in complete growth medium during short incubations was markedly reduced by EGTA concentrations in excess of Ca2+. Thyrotropin-releasing hormone (TRH) rapidly enhanced amino acid incorporation and prolactin production, with both effects being reserved by EGTA in excess of extracellular Ca2+ or prevented by cellular Ca2+ depletion. Epidermal growth factor and phorbol myristate acetate (PMA) also

Circumventricular organs: definition and role in the regulation of endocrine and autonomic function.

1. The circumventricular organs (CVO) are structures that permit polypeptide hypothalamic hormones to leave the brain without disrupting the blood-brain barrier (BBB) and permit substances that do not cross the BBB to trigger changes in brain function. 2. In mammals, CVO include only the median eminence and adjacent neurohypophysis, organum vasculosum lamina terminalis, subfornical organ and the area postrema. 3. The CVO are characterized by their small size, high permeability and fenestrated capillaries. The subcommissural organ is not highly permeable and does not have fenestrated capillarie

Functional Lactotrophs in Induced Adenohypophysis Differentiated From Human iPS Cells.

Prolactin (PRL), a hormone involved in lactation, is mainly produced and secreted by the lactotrophs of the anterior pituitary (AP) gland. We previously reported a method to generate functional adrenocorticotropic hormone-producing cells by differentiating the AP and hypothalamus simultaneously from human induced pluripotent stem cells (iPSCs). However, PRL-producing cells in the induced AP have not been investigated. Here, we confirmed the presence of PRL-producing cells and evaluated their endocrine functions. We differentiated pituitary cells from human iPSCs using serum-free floating cultu

Changes of prolactin secretion following long-term danazol application.

Patients (n = 10) with endometriosis or fibrocystic mammary disease were treated with an oral dose of 4 x 200 mg danazol for 6 months. Prolactin and gonadotropin secretion was evaluated before, and in some of them during 1, 3, and 6 months of therapy, as well as 4 weeks after discontinuation of the steroid. Prolactin, luteinizing hormone (LH), follicle-stimulating hormone (FSH), estradiol, and progesterone were measured before a bolus of 10 mg metoclopramide and 25 micrograms LH-releasing hormone (LH-RH) had been given. The serum concentrations of prolactin and LH were estimated 25 minutes and

Suckling in lactating women stimulates the secretion of insulin and prolactin without concomitant effects on gastrin, growth hormone, calcitonin, vasopressin or catecholamines.

The levels of growth hormone, vasopressin, prolactin, calcitonin, gastrin, insulin, epinephrine, norepinephrine and dopamine were measured in six lactating women during breast feeding. Prolactin levels increased in response to suckling as expected. In addition, insulin levels rose two-fold. No consistent changes were observed in the levels of the other hormones. It is suggested that the suckling related insulin release is either secondary to a reflexly induced activation of the vagal nerves or to the increased circulating levels of prolactin. Furthermore, it is suggested that the insulin relea

Development of pituitary adenoma in women with hyperprolactinaemia: clinical, endocrine, and radiological characteristics.

Sixty eight women referred for treatment of hyperprolactinaemia entered a three year follow up study to determine the clinical and endocrine course of the disease and its association with microadenoma of the pituitary. Details recorded before treatment included medical history, gonadotrophin and ovarian hormonal concentrations, and release of prolactin in response to protirelin (thyrotrophin releasing hormone), benserazide, cimetidine, and nomifensine. Sellar tomography was then performed yearly for three years in all women, 54 of them also undergoing computed coronal and sagittal tomography.

Congenital hypothyroidism in a young man with growth hormone, thyrotropin, and prolactin deficiencies.

A growth-retarded, mentally deficient, young man is described with diminished secretory response of growth hormone, thyrotropin, and prolactin to the pharmacologic stimuli of insulin, arginine, chlorpromazine, and thyrotropin-releasing hormone. Gonadotropin and ACTH functions were normal both basically and upon pharmacologic stimulation. Additionally, the patient was unresponsive to exogenous thyrotropin injections. These data suggest that the hypothyroidism in this patient was due to combined thyroid dysgenesis and pituitary insufficiency, i.e., primary and secondary hypothyroidism.

Lowering prolactin level in a hyperprolactinemic man: responses of luteinizing hormone, follicle-stimulating hormone, and testosterone.

A 25-year-old man with hypogonadotropic hypogonadism and elevated prolactin levels, presumably from a microadenoma of the pituitary gland, was treated with bromocriptine mesylate. After three weeks, the prolactin level had fallen and the gonadotropin levels had shown a substantial rise, reaching supraphysiologic levels by eight weeks, eventually returning to normal by 15 weeks. The serum testosterone level did not rise until after three weeks and reached a maximum by 15 weeks. When the bromocriptine therapy was stopped for one week, all indexes returned to near pretreatment levels. Within two

Plasma prolactin and prolactin release in liver cirrhosis.

A significant increase of basal plasma prolactin levels (radioimmunoassayed) in 75 patients with liver cirrhosis was found in comparison to 50 male controls (8.5+/-4.5 (SD) vs. 5.5+/-1.7 ng/ml p less than 0.001). The extent and incidence of hyperprolactinaemia in 48 patients with alcoholic cirrhosis was more pronounced than in 27 cases of cirrhosis of non-alcoholic aetiologies (mean 9.7+/-4.8 vs. 5.7+/-2.1 ng/ml). No relation to ascites formation as well as to the development of gynaecomastia was apparent. Prolactin release following thyrotropin-releasing hormone was markedly enhanced in alcoh

Impaired prolactin release in Huntington's chorea. Evidence for dopaminergic excess.

Eight individuals with Huntington's chorea had low basal and impaired human prolactin responses to both chlorpromazine and thyrotrophin-releasing hormone. These findings are compatible with enhanced hypothalamic dopaminergic activity in this disease. Two juvenile rigid patients with Huntington's chorea had excessive prolactin responses to chlorpromazine. Approximately half of the twenty-three potentially affected first-degree relatives of patients with Huntington's chorea had normal prolactin responses to chlorpromazine. However, twelve had significantly abnormal responses-seven in one directi

Studies of hypothalamic function in Huntington's chorea.

In eight patients with classical Huntington's chorea hypothalamic function was assessed by the insulin tolerance test, the thyrotrophin releasing hormone test, the gonadotrophin releasing hormone test and water deprivation and the results compared with those of 10 control subjects. All patients ceased to have choreiform movements for approximately 60 minutes during the insulin tolerance test. Four of the patients failed to show clinical features of stress in response to hypoglycaemia. The fasting blood glucose level and blood glucose response to insulin were similar for the two groups.
