Tendon injections - upper extremity.

Imaging-guided tendon procedures aim to reduce pain and increase function by controlling inflammation and stimulating healing. Ultrasound is the preferable guiding modality due to its high resolution and real-time demonstration of the tendinous anatomy and needle positioning. The technique includes appropriate patient positioning, which varies depending on the targeted tendon, as well as sterile and proper draping. For most procedures, we prefer the "in-plane" approach, which demonstrates the entire needle as it advances through different tissue layers. Upper limb injections commonly use corti

Stepwise management of temporomandibular joint internal derangement: from conservative therapy to arthrocentesis with intra-articular hyaluronic acid and 12.5% dextrose.

Temporomandibular joint (TMJ) internal derangement frequently leads to orofacial pain, joint dysfunction, and limited mandibular movement. Conservative treatment is first, but arthrocentesis may help refractory cases. This case report describes one year of conservative therapy and arthrocentesis with intra-articular hyaluronic acid (HA) and 12.5% dextrose. A 21-year-old woman had right-sided preauricular pain, joint clicking, and intermittent locking for one year. Clinical and magnetic resonance imaging findings confirmed anterior disc displacement with reduction and intermittent locking with

Partial Cuff Repair in Rotator Cuff Tears: Current Concepts and Clinical Considerations.

BackgroundPartial-thickness rotator cuff tears are a commonly underreported shoulder condition that can lead to pain, functional impairment, and limited range of motion. These tears affect both young, active individuals and the elderly. If left untreated, partial-thickness tears may progress to full-thickness tears over time.Classification and diagnosisPartial-thickness tears can occur on the bursal side, articular side, or within the tendon itself (intratendinous). The severity of pain and the risk of tear progression depend on the tear's location. Diagnosis often involves physical examinatio

Hand Osteoarthritis: Molecular Mechanisms, Randomized Controlled Trials, and the Future of Targeted Treatment.

Hand osteoarthritis (OA) is a prevalent and disabling condition, yet its pathogenesis remains less studied than OA in large weight-bearing joints. Emerging genetic, epigenetic, and microbiome research suggests that hand OA might be biologically distinct, involving joint-specific pathways not shared by knee or hip OA. This review integrates genome-wide association studies specific to hand OA, highlighting key molecular contributors such as inflammatory cytokines. These genetic insights, together with emerging data on epigenetic alterations and gut microbial dysbiosis, point to broader systemic

Beyond Corticosteroids: Ultrasound-Guided Regenerative and Hydrodissection Therapies for Adhesive Capsulitis.

Adhesive capsulitis (AC) is a common fibroinflammatory shoulder disorder, affecting 2-5% of the general population and up to 20% of people with diabetes mellitus. Although historically considered self-limiting, the course frequently exceeds two years and imposes a substantial functional burden. Intra-articular corticosteroids remain the predominant first-line option, but their benefit is often short-lived and is offset by transient hyperglycemia in diabetic patients, potential tissue toxicity with repeated use, and limited durability. These limitations have prompted interest in non-steroidal,

Preliminary Evidence on Intra-Articular Autologous Conditioned Serum (ACS) in Temporomandibular Joint Disorders (TMDs): A Systematic Review with a Focus on Mechanisms and Potential Application in Clinical Practice.

Intra-articular injections form a substantial element of the temporomandibular joint disorder (TMD) therapy. Given the role played by IL-1β in pathology, the use of autologous conditioned serum (ACS) is well-founded. Despite years of effective use in different locations, data regarding the intra-articular administration of ACS in TMD is scarce, and the strategy itself is not routinely applied. This study aims to provide preliminary evidence on the therapeutic efficacy of administering intra-articular ACS in treating TMD. Patients with TMD who received intra-articular ACS were included. Injection therapies for lateral epicondylalgia: a systematic review and Bayesian network meta-analysis.

BACKGROUND: There are many injection therapies for lateral epicondylalgia but there has been no previous comprehensive comparison, based on the Bayesian method. METHODS: The MEDLINE, EMBASE and the Cochrane Central Register of Controlled Trials (CENTRAL) databases were searched for appropriate literature. The outcome measurement was the pain score. Direct comparisons were performed using the pairwise meta-analysis, and network meta-analysis, based on a Bayesian model, was used to calculate the results of all of the potentially possible comparisons and rank probabilities. Ultrasound-Guided Treatment of Peripheral Entrapment Mononeuropathies.

High-resolution neuromuscular ultrasound (US) is a valuable tool for the localization, diagnosis, and provision of conservative treatment to peripheral entrapment mononeuropathies. US-guided interventions require careful coordination of transducer and needle movement along with a detailed understanding of sonoanatomy. Preprocedural planning and positioning are instrumental in performing these interventions and should not be overlooked. Minimally invasive procedures, such as corticosteroid injections, aspiration of ganglia, orthobiologics, and hydrodissection, are useful options for the nonsurg

Temporomandibular Joint Disorder and Airway in Class II Malocclusion: A Review.

In class II malocclusion, there is an anteroposterior disparity between the upper dentition and the lower dentition, which may or may not be accompanied by a skeletal discrepancy. For orthodontists, this is one of the common malocclusions encountered during clinical practice. This might be due to excess maxillary growth or retarded growth of the mandible or a combination of both. In such types of malocclusion, both the upper and lower airways are affected, the lower one most commonly. Characteristic features seen are a narrow maxillary arch, a proclined upper anterior, and mouth breathing as a

Ultrasonographic Diagnosis and Computed Tomographic Confirmation of a Scapular Body Stress Fracture in an Elite Boxer: A Case Report.

Background and Clinical Significance: Scapular stress fractures are exceptionally rare in athletes and are notoriously difficult to diagnose due to their subtle presentation and poor sensitivity on initial radiographs. This case report describes the diagnostic challenge of a scapular body stress fracture in an elite boxer who initially presented with wrist pain. Case Presentation: A 19-year-old right-hand-dominant female elite boxer presented with a three-month history of bilateral wrist pain. Initial examination and MRI were consistent with a triangular fibrocartilage complex (TFCC) injury. D

[Insertional Achilles tendinopathy : Differentiated diagnostics and therapy].

Achilles tendinopathy at the calcaneal insertion is classified into insertional tendinopathy, retrocalcaneal and superficial bursitis. The aim of this study was to present the current evidence on conservative and surgical treatment of insertional tendinopathy of the Achilles tendon. Conservative first-line therapy includes reduction of activity levels, administration of non-steroidal anti-inflammatory drugs (NSAID), adaptation of footwear, heel wedges and orthoses or immobilization. In addition, further conservative therapy options are also available. Eccentric stretching exercises should be i

Peri- and Intraarticular Injections with Isolable Treatment Effects in Recurrent Mandibular Dislocation: A Mapping Review of the Current Evidence.

Background/Objectives: Recurrent temporomandibular joint dislocation is associated with repeated dislocation episodes, pain, impaired jaw function, and psychosocial burden. Injectable intra- and periarticular therapies have been proposed as minimally invasive methods of improving joint stability, but the available evidence is heterogeneous and frequently involves adjunctive procedures. This mapping review aimed to characterize injectable treatments whose effects could be assessed independently from simultaneous non-injectable interventions. Cervicovagopathy: ligamentous cervical instability and dysstructure as a potential etiology for vagus nerve dysfunction in the cause of human symptoms and diseases.

Vagus nerve dysfunction is implicated in the pathophysiology of many different symptoms and diseases that plague humanity. In many cases, the etiology of this condition remains elusive. One potentially implicating factor is cervical spine pathology, as the 2 vagus nerves are located in the carotid sheath just anterior to the cervical vertebrae. We propose that cervicovagopathy occurs primarily by the slow stretching of the posterior cervical ligaments because of a forward head-facedown lifestyle from excessive cell phone and computer usage. While the excessive stretch and compression on the va
