Intravenous Propafenone for Conversion of Atrial Fibrillation or Flutter to Sinus Rhythm: A Randomized, Placebo-controlled, Crossover Study.

BACKGROUND: Propafenone has been claimed to be effective in converting atrial fibrillation and flutter to sinus rhythm; however, controlled clinical trials have reported variable results, and data about the safety of propafenone in the setting of heart failure are lacking. The aim of the present study was to evaluate the efficacy and safety of intravenous propafenone in converting atrial fibrillation and flutter to sinus rhythm. METHODS: Sixty patients with acute (<72 h) or chronic atrial fibrillation or flutter were included in a randomized, placebo-controlled, conditional cross-over study. T

Wide complex tachycardia and brugada electrocardiographic pattern induced by propafenone: A rare and unpredictable pro-arrhythmic effect.

Propafenone is a class IC anti-arrhythmic drug used for the rhythm control strategy in atrial fibrillation worldwide, including in Latin America countries. Propafenone intoxication is a potentially life-threatening condition. Ventricular tachycardia and the Brugada electrocardiographic (ECG) pattern are possible pro-arrhythmic manifestations of propafenone poisoning. We present a rare patient case, where therapeutic range dosage of propafenone resulted in a wide complex tachycardia, followed by the Brugada ECG pattern during atrial fibrillation. We discuss the possible mechanisms related to th

Is there a synergic effect of propafenone associated with atrial overdrive pacing for atrial arrhythmia prevention? A randomised crossover study.

OBJECTIVE: To assess the effect of adding propafenone to atrial overdrive for the prevention of atrial arrhythmia episodes in patients with DDD pacemakers. DESIGN: 22 patients (8 female, 14 male, mean (SD) age 67 (9) years, range 48 to 77) with DDD pacemakers and frequent paroxysmal atrial arrhythmia episodes were evaluated in a randomised crossover study. SETTING: University hospital. METHODS: Atrial overdrive was defined as a paced rate of 10 paced beats/min above the mean ventricular rate stored for the last 24 hours in the pacemaker memory function. The protocol consisted of two phases

A method for evaluating different modes of action of an antiarrhythmic drug in man. The effects of propafenone on sinus nodal functions.

In vitro experiments have shown that the antiarrhythmic effects of propafenone are due to a direct depressant action and to a beta-blocking activity. In this study a method was used to evaluate the direct effect and the autonomically mediated actions of an antiarrhythmic agent in a clinical setting. An electrophysiological study was performed twice, at an interval of 24 hr, in 17 patients (age: 52 +/- 17 years) with normal resting and intrinsic heart rate. In the first study the overall effect of intravenous propafenone (1.5-2 mg/kg) was evaluated by comparing the sinus node parameters obtaine

Usefulness of programmed stimulation in predicting efficacy of propafenone in long-term antiarrhythmic therapy for paroxysmal supraventricular tachycardia.

The electrophysiologic effects of intravenous (i.v.) and oral propafenone were evaluated in 14 patients with Wolff-Parkinson-White syndrome and in 10 patients with atrioventricular (AV) nodal reentrant tachycardia. The effective refractory periods of the right atrium and the AV node increased after both preparations. In patients with Wolff-Parkinson-White syndrome, i.v. propafenone blocked anterograde accessory pathway conduction in 2 patients and retrograde conduction in 1; during oral therapy, accessory pathway conduction block occurred in 2 additional patients. The mean cycle length of the

[The study of acute clinical electrophysiological effects of propafenone on paroxysmal supraventricular tachycardia using transesophageal atrial pacing technique].

Trans-esophagus atrial pacing (TEAP) was employed to evaluate the acute electrophysiological effects of propafenone administrated intravenously in 50 patients with SVT. Forty three of the subjects are patients with atrial ventricular reciprocating tachycardia (AVRT). Seven of them were involved in atrioventricular nodal reentry tachycardia (AVNRT). The results indicate that propafenone exerts an obvious inhibitory effect on both the dual atrioventricular node pathways and the accessory pathways, with the latter one being markedly affected. In addition, propafenone can deter the antegrade and r

Propafenone is associated with fewer recurrences of supraventricular arrhythmias in mechanically ventilated patients with septic shock and right ventricular dysfunction.

INTRODUCTION: Supraventricular arrhythmias (SVA) are common in mechanically ventilated patients with septic shock and may be aggravated by right ventricular (RV) dysfunction. Optimal rhythm-control strategies in this setting remain uncertain. We explored whether propafenone differs from amiodarone in maintaining sinus rhythm in patients with RV dysfunction. METHODS: This exploratory sub-analysis of the randomized double-blind PRASE trial included patients with septic shock who developed new-onset SVA, had preserved to moderately reduced left ventricular systolic function (ejection fraction >3

Conversion of recent onset atrial fibrillation with single loading oral dose of propafenone: is in-hospital admission absolutely necessary?

A population of 283 patients with recent onset (< 72 hours) AF, without heart failure, who received a single 450- or 600-mg oral dose of propafenone, or digoxin 1 mg, or placebo for conversion to sinus rhythm (SR), was studied to determine whether a routine admission to the hospital for drug administration is justified. Previous bradyarrhythmias or sick sinus syndrome (SSS), and concomitant use of antiarrhythmic drugs were exclusion criteria. None of the 283 patients studied experienced VT or VF and none of them needed implantation of a temporary pacemaker. Periods of atrial tachyarrhythmias w

[Anti-arrhythmia efficacy of propafenone per os in the prevention of paroxysmal supraventricular arrhythmia resistant to class Ia agents].

The efficacy of orally administered propafenone in the prevention of paroxysmal supraventricular dysrhythmias (atrial fibrillation, atrial tachysystole, reciprocal tachycardia) resistant to Vaughan-Williams class Ia drugs was investigated in 10 patients. Propafenone controlled dysrhythmia very well in 4 out of the ten patients, and satisfactorily in another one at a dosage ranging from 450 to 900 mg/day in 3 or 4 divided doses; this result was documented using continuous 24 hr. Holter ECG monitoring. Among the 5 clinical failures, 1 patient had atrial fibrillation primarily at night, and anoth

Comparative pharmacokinetics between a microdose and therapeutic dose for clarithromycin, sumatriptan, propafenone, paracetamol (acetaminophen), and phenobarbital in human volunteers.

A clinical study was conducted to assess the ability of a microdose (100 μg) to predict the human pharmacokinetics (PK) following a therapeutic dose of clarithromycin, sumatriptan, propafenone, paracetamol (acetaminophen) and phenobarbital, both within the study and by reference to the existing literature on these compounds and to explore the source of any nonlinearity if seen. For each drug, 6 healthy male volunteers were dosed with 100 μg (14)C-labelled compound. For clarithromycin, sumatriptan, and propafenone this labelled dose was administered alone, i.e. as a microdose, orally and intrav

Flecainide and propafenone induced ST-segment elevation in patients with atrial fibrillation: clue to specificity of Brugada-type electrocardiographic changes.

Potent sodium channel, blockade with type IC antiarrhythmics can provoke characteristic electrocardiographic changes consistent with Brugada's syndrome in unselected patients with atrial fibrillation. In 176 treated patients, the incidence of the characteristic abnormality is small (2.3%), and thus although no ventricular arrhythmia events were observed during follow-up, the long-term clinical significance in a larger patient cohort remains to be determined. These data provide insight into the specificity of the response to type I drug administration in patients suspected of having Brugada's s

Intravenous versus oral initial load of propafenone for conversion of recent-onset atrial fibrillation in the emergency room: a randomized trial.

BACKGROUND: Non-valvular paroxysmal atrial fibrillation is a common clinical condition associated with a high risk of thromboembolism and hemodynamic problems which increase with the duration of arrhythmia. Therefore, even if arrhythmia ceases spontaneously within 24 hours in about half of the patients, a higher early conversion rate is desirable. Propafenone either by intravenous or oral load has been shown effective in conversion to sinus rhythm. METHODS: We consecutively randomized all emergency patients with non-valvular atrial fibrillation lasting no more than 48 hours to either intraven
