Chronic Kidney Disease and Disproportionally Increased Cardiovascular Damage: Does Oxidative Stress Explain the Burden?

Chronic kidney disease (CKD) patients are among the groups at the highest risk for cardiovascular disease and significantly shortened remaining lifespan. CKD enhances oxidative stress in the organism with ensuing cardiovascular damage. Oxidative stress in uremia is the consequence of higher reactive oxygen species (ROS) production, whereas attenuated clearance of pro-oxidant substances and impaired antioxidant defenses play a complementary role. The pathophysiological mechanism underlying the increased ROS production in CKD is at least partly mediated by upregulation of the intrarenal angioten

Adaptive immunity in rheumatoid arthritis: anticitrulline and other antibodies in the pathogenesis of rheumatoid arthritis.

PURPOSE OF REVIEW: To describe recent progress concerning rheumatoid arthritis (RA)-associated autoantibodies, in particular antibodies to citrullinated proteins antigens. RECENT FINDINGS: An increasingly diverse and RA-associated repertoire of antibodies has been defined over the last few years. These antibodies are preferentially, but not exclusively, reactive with posttranslationally modified antigens. Citrullinated antigens are the most common targets, but also other modifications including homocitrullination (carbamylation) are recognized. These antibodies display varying degrees of cros

Access to novel potent pleiotropic prodrugs, targeting both butyrylcholinesterase and serotonin reuptake, with anti-amnesic activities in Alzheimer's disease model.

The treatment of Alzheimer's disease should undoubtedly be multifactorial and require a polypharmacological approach to be effective. Pleiotropic prodrugs can release, upon inhibition of a primary target, a drug that targets a second protein of therapeutic interest. Here, we describe the design of prodrugs that release a potent serotonin reuptake inhibitor, 7-hydroxysertraline, upon the inhibition of butyrylcholinesterase. Indeed, 7-hydroxysertraline is not only a ligand of the serotonin transporter but also possesses the structural requirements to yield carbamates, able to bind to the catalyt

Arterial Remodelling in Chronic Kidney Disease: Impact of Uraemic Toxins and New Pharmacological Approaches.

Chronic kidney disease (CKD) is a major public health concern that affects around 10 percent of the world's population. The severity of CKD is mainly due to the high prevalence of cardiovascular (CV) complications in this population. The aim of this review is to describe the arterial remodelling associated with CKD, to provide a quick overview of the mechanisms involved and to review the recent pharmacological approaches aimed at improving vascular health in CKD. CKD patients are exposed to metabolic and haemodynamic disorders that may affect the CV system. Large artery functional and geometri

Peptides Bearing Multiple Post-Translational Modifications as Antigenic Targets for Severe Rheumatoid Arthritis Patients.

Rheumatoid arthritis (RA) is characterized by the presence of autoantibodies that are of paramount importance for the diagnosis and prognosis of the disease and have been implicated in its pathogenesis. Proteins resulting from post-translational modifications (PTMs) are capable of triggering autoimmune responses important for the development of RA. In this work, we investigate serum antibody reactivity in patients with an established RA against a panel of chimeric peptides derived from fibrin and filaggrin proteins and bearing from one to three PTMs (citrullination, carbamylation and acetylati

[Early reumatoid arthritis].

Rheumatoid arthritis (RA) is a systemic, autoimmune disease characterized by synovitis and irreversible joint destruction. RA development is a multi-stage process conditioned by the presence of genetic and environmental risk factors on the basis of which systemic immunization develops. The best known genetic factor predisposing to RA development is the polymorphism of HLA-DRB1 region of the major histocompatibility complex. Among environmental factors, importance in the pathogenesis of RA is attributed to the development of inflammation of the respiratory tract and chronic periodontitis caused

Transiently reduced activity of carbamyl phosphate synthetase and ornithine transcarbamylase in liver of children with Reye's syndrome.

Since Reye's syndrome is associated with hyperammonemia, we measured the urea-cycle enzymes in hepatic tissue of 13 patients. Expressed as nanomoles of citrulline per milligram of hepatic protein per minute, mean activity of carbamyl phosphate synthetase (6.27 +/- 2.45 S.D.) and ornithine transcarbamylase (136.19 +/- 41.83) in Reye's syndrome was reduced significantly (P less than 0.005) when compared with that of 25 "normal" controls (11.54 +/- 4.24 and 307.49 +/- 94.15, respectively). Activity was maximally reduced during the first days of clinical symptoms; it returned toward normal during

Citrulline in health and disease. Review on human studies.

The amino acid L-citrulline (CIT) is safely used from the neonatal period onwards in those with urea cycle defects and carbamyl phosphate synthetase or ornithine transcarbamylase deficiencies, but several lines of enquiry indicate that it might have a much wider therapeutic role. When protein intake is low and there is a catabolic state, endogenous arginine (ARG) synthesis cannot fully be met and its supplementation can prove challenging, particularly in patients with critical and multisystem illness. Supplementary CIT could constitute a safer but still focused means of delivering ARG to endot

Long-term management of a case of carbamyl phosphate synthetase deficiency using ketanalogues and hydroxyanalogues of essential amino acids.

A 13-year-old girl with congenital deficiency of carbamyl phosphate synthetase has been treated intermittently for one year with a restricted protein diet supplemented by various mixtures of the alpha-ketoanalogues of valine, leucine, isoleucine, and phenylalanine, the D,L-alpha-hydroxyanalogue of methionine, and five amino acids (lysine, arginine, histidine, threonine and tryptophan). Numerous adjustments in the composition of this mixture were made. Eventually normal levels of plasma ammonia and most amino acids were achieved, with three exceptions: slightly increased glutamine, pronounced a

Kale anaemia in ruminants. I. Survey of the literature and experimental induction of kale anaemia in lactating cows.

After a review of the literature the report of a trial with 30 dairy cows is presented. The cows were divided into 3 groups of 10; 1 group functioned as control (A), 1 had free access to marrow-stem kale (B), and the animals of the 3rd received 10 kg marrow-stem kale daily (C). The trial period lasted about 6 weeks. Blood samples from all animals were taken before the trial began, twice in the course of the experiment, and 3 weeks after its termination. Estimations were made for haemoglobin, haematocrit, the erythrocyte in vitro uptake of I131-triiodothyronine, total protein and protein fracti

Conformational drift of long-lived proteins in the human eye: Insights into post-translational modifications and analytical techniques for ocular protein ageing.

Long-lived proteins provide a uniquely informative substrate for studying the molecular chemistry of human ageing, and the eye offers one of the most accessible and spatially resolved systems for examining this process. The lens remains the principal model: its central crystallins are synthesized during embryonic and early postnatal life and are retained throughout the lifespan, thereby preserving a cumulative record of irreversible chemical damage. We then extend this framework to other ocular compartments that contain long-lived or slowly turned-over proteins, including Bruch's membrane, the

The role of autoantibodies in the pathophysiology of rheumatoid arthritis.

Rheumatoid arthritis (RA) is an autoimmune disease characterized by joint inflammation. The presence of autoantibodies in the sera of RA patients has provided many clues to the underlying disease pathophysiology. Based on the presence of several autoantibodies like rheumatoid factor (RF), anti-citrullinated protein antibodies (ACPA), anti-carbamylated protein antibodies (anti-CarP), and more recently anti-acetylated protein antibodies RA can be subdivided into seropositive and seronegative disease. The formation of these autoantibodies is associated with both genetic and environmental risk fac

New biomarkers in rheumatoid arthritis.

Rheumatoid arthritis (RA) is a common autoimmune disease affecting around 1% of the population. Although major advances have been made in the treatment of RA, still relatively little is known on disease pathogenesis and aetiology. From treatment studies it has become clear that treating patients early in their disease course will provide the best results. However, especially in the early phase of arthritis, in particular when the patients do not yet fulfil the criteria for RA, it is difficult to decide which patients would benefit most from an early and aggressive intervention. Good biomarkers

Association of Serum Uromodulin and Carbamylated Albumin with All-Cause and Cardiovascular Mortality in Patients with No or Mild Chronic Kidney Disease.

BACKGROUND: Serum uromodulin (SUmod) and carbamylated albumin (C-Alb) are emerging biomarkers for chronic kidney disease (CKD) progression and mortality. SUmod reflects tubular health, while C-Alb is associated with excretory kidney function, CKD progression, and cardiovascular (CV) mortality. We hypothesized that the combined use of these markers would improve mortality risk assessments. METHODS: We analyzed the associations of C-Alb and SUmod levels with the estimated glomerular filtration rate (eGFR) along with their combined predictive value for assessing mortality in 3316 participants of

Proteomic analysis of oxidative modifications in human atherosclerotic plaques and control arteries.

Cardiovascular disease is a major cause of worldwide mortality. A prime underlying cause is atherosclerotic plaque formation at specific arterial locations. Considerable evidence links plaques with chronic inflammation and leukocyte activation, with this potentially resulting in enhanced oxidant-mediated damage to arterial proteins. In this study, human carotid artery plaques and control tissue (superior thyroid artery) were analyzed by liquid chromatography-mass spectrometry (LC-MS/MS) for the presence of post-translational modifications (PTMs) generated by redox reactions on proteins. Of 715

Ethoxyformylation and guanidination of snake venom phospholipases A2: effects on enzymatic activity, lethality and some pharmacological properties.

Lysine residues in the basic and relatively toxic N. nigricollis phospholipase A2 and in the acidic and relatively nontoxic N. n. atra phospholipase A2 were modified by acylation with ethoxyformic anhydride (in the presence or absence of the substrate dihexanoyl lecithin) or guanidination with O-methylisourea. Ethoxyformylation gave rise to some protein fractions in which enzymatic activity was preserved to a greater degree than intraventricular lethality. Guanidination had little effect on the isoelectric point or catalytic activity of either enzyme or on the lethal potency of the N. n. atra

Biochemical markers of chronic alcoholism.

Usefulness of several biochemical markers for the monitoring of chronic alcoholism were studied. Among generally used markers, only gamma-GTP showed a significant difference between alcoholic and non-alcoholic liver diseases. Serum glutamate dehydrogenase (GDH) activity was significantly high in alcoholic liver disease. When the ratios of GDH to ornithine carbamyl transferase (OCT) were calculated, differences between alcoholic and non-alcoholic liver diseases became clearer without overlapping of any value. Serum desialo-transferrin was found in about 60% of the alcoholics, and disappeared by

Post-translationally modified proteins bind and activate complement with implications for cellular uptake and autoantibody formation.

INTRODUCTION: Autoimmune diseases, such as rheumatoid arthritis (RA), are characterized by the presence of autoantibodies including those targeting self-proteins modified by post-translational modifications (PTMs). The complement system is known for its role in innate immune defense, but also in clearing debris and induction of antibody responses. We therefore hypothesized that complement could directly bind to PTMs and target PTM-modified proteins for clearance, or stimulate (chronic) inflammation and development of anti-PTM autoimmunity. METHODS: Six PTMs were investigated: nitration (Nt),

Update on autoantibodies to modified proteins.

PURPOSE OF REVIEW: This article provides an update on the recent findings on autoantibodies to modified proteins in rheumatoid arthritis (RA). RECENT FINDINGS: In the past few years, the knowledge on the autoantibodies to citrullinated antigens has expanded considerably. More specifically, it is now clear that many different citrullinated protein antigens present in the synovial compartment can be recognized by anticitrullinated protein antibody (ACPA). This is most likely a consequence of the cross-reactivity the ACPA response displays to citrullinated proteins. It is now also clear that the

The pathogenic potential of autoreactive antibodies in rheumatoid arthritis.

Rheumatoid arthritis (RA) is an autoimmune disease affecting ∼1 % of the population. Although major advances have been made in the treatment of RA, relatively little is known about disease pathogenesis. Autoantibodies, present in approximately 60 % of the patients with early disease, might provide indications for immunological mechanisms underlying RA. Among the RA-associated autoantibodies, especially anti-citrullinated protein antibodies (ACPAs) have been studied intensively in the last decade. The discovery of ACPAs resulted into novel insight in RA pathogenesis and allowed division of the
