Retrograde intramedullary supracondylar nails have been added to the orthopedic armamentarium for treatment of distal femoral fractures. Major complications of this new technique have not been noted. We report a case of a femoral fracture at the proximal end of a retrograde intramedullary supracondylar nail. We hypothesize that this complication may be due to the proximal end of the nail acting as a stress riser, with the cortical holes drilled for the interlocking screws compounding this effect.

A retrospective study of the data on 27,370 hospital discharges of patients who had been admitted to non-federal Maryland hospitals from 1979 through 1988 for a fracture of the proximal part of the femur and who had been at least sixty-five years old at the time of the fracture showed that the ratio of trochanteric fractures to fractures of the femoral neck increased linearly with age in white and black women. For men, this ratio was stable across age-intervals, being slightly more than one in white men and less than one in black men. Black patients who had a fracture of the hip were more like

INTRODUCTION: Dynamic hip screw (DHS) and proximal femoral nail (PFN) are both used to fix extracapsular proximal femoral fractures, with implant selection in routine practice typically guided by fracture morphology. This study describes perioperative outcomes and the pattern of implant selection in older adults treated at a single UK trauma centre.
MATERIALS AND METHODS: This single-centre retrospective descriptive study included all eligible patients aged 65 years or older undergoing fixation of extracapsular proximal femoral fractures during the 12-month study period (1 January to 31 Decem

INTRODUCTION: Intertrochanteric (IT) fractures require stable fixation and early mobilization. This study evaluated the functional and radiological outcomes of proximal femoral nailing (PFN) in adults with IT fractures.
MATERIALS AND METHODS: A prospective observational study was conducted over 18 months in 48 patients aged 18-80 years with radiologically confirmed IT fractures treated with PFN. Functional outcome was assessed using the Modified Harris Hip Score (HHS) at 3 and 6 months. Radiological outcome was assessed using the radiographic union score for hip (RUSH), fracture alignment, an

CASE: We report on a 22-year-old patient with a comminuted femoral neck fracture because of a low-velocity gunshot wound. Treatment consisted of reduction and internal fixation with a fixed angled blade plate and immediate valgus osteotomy. He went on to uneventful healing and an excellent clinical result.
CONCLUSIONS: A comminuted gunshot femoral neck fracture in a young patient is a rare and potentially devastating injury. If stable, and there are no other associated emergent injuries, reduction, internal fixation, and immediate valgus osteotomy should be considered.

INTRODUCTION: A high mortality rate is associated with hip fractures in the elderly. This is because their bones are osteoporotic with implants having less hold and there are more co-morbidities associated with the elderly. Osteosynthesis with the proximal femoral nail (PFN) features the advantages of high rotational stability of the head-neck fragment. However, the use of the nail is technically ambitious and is accompanied by some risks of error, which can lead to failure. This study aims to understand the technical difficulties related to PFN and methods to mitigate them and radiological in

BACKGROUND AND AIMS: Cardiovascular complications are common in hip fracture patients but the role of fracture type and operative method in these is unclear. This prospective cohort study aimed to evaluate the impact of fracture- and operative characteristics on perioperative cardiovascular complications and prognosis in unselected hip fracture patients.
MATERIAL AND METHODS: During a 7-month trial registration period, a population of 197 consecutive hip fracture patients (median age 84 years) diagnosed with femoral neck or pertrochanteric fracture was formed. The exclusion criteria were pati

BACKGROUND: The aim of this study is to compare the clinical results of proximal femoral nail (PFN) and proximal femoral anatomic plate (PFA) methods used in the treatment of proximal femoral fractures through a multicenter retrospective review, and to determine which method is more advantageous for specific patient groups. The study aims to evaluate the effectiveness of PFN and PFA techniques and presents findings that can guide clinical decision-making by revealing the differences between these two methods.
METHODS: Between 2016 and 2021, 106 patients with proximal femur fractures classifie

BACKGROUND: Proximal femoral fracture (PFF), such as intertrochanteric femoral fracture or femur neck fracture, and its management are crucial issues to surgeons. PFF has been dramatically is becoming exponentially prevalent, and it is at high risk of complication and mortality because it is frequently associated with serious trauma and advanced age, especially in patients treated with anticoagulants or antiplatelet agents. Surgical management is essential for the treatment of PFF. Unfortunately, current surgical procedures have been related to accompanied by vascular complications, including

Peritrochanteric fractures are the most common fragility fractures for which patient are admitted in hospital and often require surgical interventions. With increasing life expectancy and early age of presentation, revision surgeries are increasing due to re-trauma, implant failure, infections etc. Here we present the case of a 65 years female, with inter trochanteric fracture femur right side with ipsilateral malunited proximal femur fracture, which was managed with customised proximal femoral nail. This case exemplifies the need for novel techniques and implants in our armamentarium to deal

BACKGROUND: Preoperative posterior tilt is a risk factor for fixation failure in femoral neck fractures. This study aimed to evaluate the configuration of anterior positioning screw in proximal femoral plating in the treatment of retroverted femoral neck fractures in terms of resisting posterior tilt.
METHODS: We retrospectively analyzed patients with retroverted femoral neck fractures who were fixed by proximal femoral plating from January 2014 to August 2019. All patients were divided into two groups according to screw configuration: anterior long-threaded screw (ALTS, n = 36) and normally

INTRODUCTION: Trochanteric fractures represent a substantial proportion of hip fractures in the elderly and are associated with significant morbidity and mortality. Dynamic hip screw fixation has shown higher failure rates in unstable fractures, leading to the use of intramedullary implants such as the proximal femoral nail (PFN) and the Halifax (Gamma) nail.
METHODS: A cross-sectional study was conducted on 134 patients aged 60-80 years with unstable trochanteric fractures admitted to the Trauma Center of the Institute of Medical Sciences, Banaras Hindu University, Varanasi, Uttar Pradesh, I

BACKGROUND: Proximal femoral nails (PFNs) are the most common method for the treatment of unstable intertrochanteric femoral fractures (IFFs), but postoperative bed rest is required. There is a large amount of blood loss during the operation. Osteoporosis in elderly patients may cause nonunion of fractures and other complications. Arthroplasty can give patients early weight bearing and reduce financial burden, but whether it can replace PFNs remains controversial.
AIM: To compare the clinical outcomes of arthroplasty and PFNs in the treatment of unstable IFFs in elderly patients.
METHODS: A

The unreamed femoral nail system (UFN) features highly versatile proximal interlocking options for the treatment of a wide range of femoral fracture patterns and combinations. Besides standard interlocking modes for diaphyseal fractures, special interlocking options for subtrochanteric fractures and ipsilateral neck/shaft fractures are available. Two aiming arms can be attached to the insertion handle of the nail in a modular fashion. The standard aiming arm is designed for transverse static or dynamic locking. The special aiming arm permits spiral blade, 130 degrees, and miss-a-nail interlock

AIMS: The use of cephalomedullary nailing (CMN) to treat extracapsular hip fractures is a well-established fixation method. Nationally, the use of CMN has increased from 67.7% in 2012 to 90.9% in 2025 for subtrochanteric fractures (National Hip Fracture Database). Although rare, implant failure is a recognised complication which poses risk to patients who are otherwise already burdened with high morbidity and mortality. The purpose of this study is to review the outcomes of CMN fixations for proximal femoral fractures, with particular focus on implant failure. We analysed patient demographics,

Introduction We aimed to retrospectively evaluate the effectiveness of osteoporosis treatment in patients aged ≥90 years with proximal femoral fractures, with a focus on secondary fracture prevention. Methods This retrospective cohort study included 247 nonagenarians (mean age: 93.5 years) who underwent surgery for proximal femoral fractures at our hospital between 2017 and 2022. Patients were categorized into three groups: those for whom new treatment was initiated after the fracture, those who had pre-existing treatment before the fracture, and those without treatment. The primary outcome wa
