Metacognitive Interpersonal Group Therapy for Adolescents With Emerging Personality Disorders: The Case of Nora.

An increasing number of adolescents attending child and adolescent mental health services present symptoms consistent with an emerging personality disorder (PD). Early identification and intervention are critical to preventing the devastating progression of symptoms, reducing the duration of untreated disorders, and supporting the recovery, development, and future quality of life of these individuals. The primary aim of this paper is to introduce a group therapy proposal designed for the entire spectrum of adolescent PDs: Metacognitive Interpersonal Group Therapy for Adolescents (MIT-GA). MIT-

Dropout from psychological treatment for borderline personality disorder: a multilevel survival meta-analysis.

BACKGROUND: Dropout from psychotherapy for borderline personality disorder (BPD) is a notorious problem. We investigated whether treatment, treatment format, treatment setting, substance use exclusion criteria, proportion males, mean age, country, and other variables influenced dropout. METHODS: From Pubmed, Embase, Cochrane, Psycinfo and other sources, 111 studies (159 treatment arms, N = 9100) of psychotherapy for non-forensic adult patients with BPD were included. Dropout per quarter during one year of treatment was analyzed on participant level with multilevel survival analysis, to deal w

A benchmark study of a combined individual and group anorexia nervosa therapy program.

OBJECTIVE: Anorexia nervosa (AN) is a treatment-resistant illness. Up to half of the patients show limited or no significant treatment response. First, we aimed to describe a new stepped-care outpatient treatment combining group and individual therapy (CIGAN) with a strong focus on normalizing eating and weight and in vivo meal support for adults with moderate to severe AN. Second, we aimed to compare CIGAN with specialized treatments of Specialized Supportive Clinical Management (SSCM) and the Maudsley model of AN (MANTRA). METHOD: The study includes 137 adults with moderate to severe AN and

A healing herd: benefits of a psychodynamic group approach in treating body-focused repetitive behaviors.

Body-focused repetitive behaviors, such as trichotillomania and skin-picking, are not well understood in clinical communities. Research and practice with a population exhibiting these behaviors have focused almost exclusively on the usefulness of cognitive-behavioral therapy in symptom reduction. The author specializes in treating patients with body-focused repetitive behaviors and suggests that a holistic, psychodynamic model of treatment for this population creates possibilities for deeper change. Group therapy can be an important component of this work, helping clients to overcome resistanc

Psychological treatment of eating disorders. A review of the literature.

The aim of this study is to review the existing literature (PubMed database) on the psychological treatments for eating disorders (EDs), subdivided in individual, group and family therapies. Moreover new approaches and directions in this field are addressed. An extensive literature review is performed to identify the psychological treatment trials in anorexia nervosa (AN) and bulimia nervosa (BN) published over the past 2 decades. Eighty-two studies focused on psychotherapeutic treatment of EDs are reviewed. Only a minor part of these studies are randomised and controlled. While there is evide

ACT-enhanced group behavior therapy for trichotillomania and skin-picking disorder: A feasibility study.

OBJECTIVE: To evaluate the feasibility and efficacy of ACT-enhanced Group Behavior Therapy (AEGBT) for mixed diagnosis groups including patients with trichotillomania (TTM) and skin-picking disorder (SPD) in routine psychiatric care. METHOD: Adult patients (N = 40) with TTM and/or SPD received 10 weeks of AEGBT followed by five booster sessions. The primary outcome measure for TTM was the Massachusetts General Hospital Hairpulling Scale (MGH-HPS) and for SPD the Skin Picking Scale-Revised (SPS-R), assessed at posttreatment and at booster sessions. RESULTS: Results showed significant reductio

Group treatment of the hyperventilation syndrome.

Hyperventilation (hv) is increasingly recognized as being significant in a number of psychological and medical conditions. The core of treatment for hv is breathing retraining, usually on an individual basis. This article describes a group therapy for breathing retraining for patients with hv-induced panic reactions. An analysis of group process suggests that such treatment is helpful in ways impossible for individual retraining and that further exploration of this modality is warranted.

Psychological Treatment of the Paraphilias: a Review and an Appraisal of Effectiveness.

Initially, this paper notes that treatment for the paraphilias has been most thoroughly described and evaluated within the context of treating sex offenders (i.e., child molesters, rapists, and exhibitionists). We note that the literature does not always carefully distinguish "pedophiles" from other child molesters and that rapists are often identified as having a "Not Otherwise Specified" paraphilia. Both these practices appear problematic. We then outline current approaches to the treatment of sex offenders which have typically been seen as relevant to dealing with all types of paraphilias.

Brief group psychoeducation for bulimia nervosa: assessing the clinical significance of change.

Forty-one women with bulimia nervosa completed a brief intervention that was designed to promote symptom management. Cutoff scores for the determination of reliable and clinically significant change were calculated for several psychometric instruments that are commonly used as outcome measures in the field of eating disorders. Using these statistical procedures to assess therapeutic change, this study documents the diversity of outcomes that individuals reported following their participation in the intervention. As expected, there was a differential reporting of clinically significant change i

Contextual analysis and treatment of phobic behavior as it changes.

Contextual therapy of phobias is based upon the observation, analysis and conceptualization of phobic behavior as it changes in its natural contexts. Detailed, tape-recorded studies of phobic behavior changing in context were used to develop concepts and treatment of the processes governing change. Contextual therapy has been applied to individual, group, and self-help treatment of phobias.

Modeling vulnerability and intervention targets in the Borderline Personality Disorder system: A network analysis of in silico and in vivo interventions.

Modeling psychopathology as a complex dynamic system represents Borderline Personality Disorder (BPD) as a constellation of symptoms (e.g., nodes) that feedback and self-sustain each other shaping a network structure. Through in silico interventions, we simulated the evolution of the BPD system by manipulating: 1) the connectivity strength between nodes (i.e., vulnerability), 2) the external disturbances (i.e., stress) and 3) the predisposition of symptoms to manifest. Similarly, using network analysis we evaluated the effect of an in vivo group psychotherapy to detect the symptoms modified by

The Masters and Johnson treatment program for dissatisfied homosexual men.

Certain individuals who want to change their homosexual preference can be helped by a short-term intensive intervention adapted from the Masters and Johnson model for treating heterosexual disorders. Pretreatment counseling aims at improving the patient's dating, sexual, and intimacy skills; creative problem solving; and stress management. Then for 2 weeks the patient and an opposite-sex partner, living in social isolation, meet daily with a therapy team to address fears and anxieties about intimacy as they arise. The directive psychotherapy format alternates between confrontation of maladapti

[What is psychotherapy? An interdisciplinary proposal for a definition].

The definition of psychotherapy has wide-ranging implications for research and clinical care as well as for legal issues, as set out in the German psychotherapy guidelines. It can determine which services are covered by health insurance and who is authorized to provide the services. In light of the ongoing debate regarding the evidence base of psychotherapy, a group of experts from various professional disciplines with different theoretical and methodological backgrounds and focusing on different age groups, has worked together with individuals with personal experience of psychotherapy and you
