Anatomical Changes in a Case with Asymmetrical Bilateral Maxillary Sinus Hypoplasia.

Background and Objectives: The maxillary sinus hypoplasia (MSH) is an occasional variation of the maxilla, occurring either unilaterally or bilaterally. Previous studies dealing with MSH have not detailed the consequent anatomical changes of the maxilla and adjacent fossae. Materials and Methods: A 58-year-old female case was scanned in Cone Beam Computed Tomography and found to have asymmetrical bilateral MSH, who was then further evaluated anatomically. Results: The maxillary sinuses were hypoplastic and had mild mucosal thickenings. The orbital floors were curved. The uncinate process and t

Endoscopic vidian neurectomy assisted by power instrumentation and coblation.

Vidian neurectomy has been used to manage intractable vasomotor rhinitis for decades. After the introduction of endoscopic sinus surgery in the 1980s, transnasal endoscopic vidian neurectomy (EVN) was subsequently reported. The most common problem in performing EVN was excessive bleeding from the pterygopalatine fossa. The complexity and vascularity of the pterygopalatine fossa can cause bloody surgical fields and prevent complete neurectomy. In response to this surgical problem, a procedure was developed to use powered instrumentation and coblation during EVN. There were eight cases of EVNs (

Endoscopic-assisted en-bloc pterygomaxillectomy: Identifying an efficient and safe location for the pterygoid osteotomy.

BACKGROUND: For aggressive maxillary sinus and pterygopalatine fossa (PPF) tumors, an en-bloc pterygomaxillectomy may be indicated. METHODS: Five head specimens were used to study the feasibility of an en-bloc pterygomaxillectomy. Eighty-five non-pathological CT scans were used to compare the superior edge of the inferior turbinate (IT) and the middle turbinate tail (MT) as landmarks for the pterygoid osteotomy. RESULTS: Through a combined sublabial-subperiosteal incision and transoral route, a mid-sagittal osteotomy through the hard palate and an axial osteotomy below the infraorbital foram

Endoscopic prelacrimal approach to lateral recess of sphenoid sinus: feasibility study.

BACKGROUND: Various pathologies, including cerebrospinal fluid leaks and meningoencephaloceles, may arise in the lateral recess of the sphenoid sinus (LRSS), which may be accessed via an endonasal transpterygoid approach. The objective of this study was to evaluate the feasibility of accessing the LRSS via an endoscopic prelacrimal approach. Furthermore, we hypothesized that this approach may protect the pterygopalatine ganglion and vidian nerve. METHODS: Five cadaveric heads (9 sides) with a well-pneumatized LRSS were identified and an endonasal prelacrimal approach was performed. The infrao

Infratemporal fossa and maxillary sinus metastases from colorectal cancer: a case report.

Head and neck metastases from colorectal carcinoma are very rare. We report on a 47-year-old woman who had a vegetating tumor located at the distal sigmoid colon with initial liver metastases. She underwent palliative transverse colostomy to prevent intestinal obstruction. She was then treated with 5-fluorouracil, leucovorin and CPT-11. At the third cycle she presented with toothache and numbness of her upper lip. Computed tomography (CT) of the head and neck showed a gross mass involving the sphenoid sinus, left maxillary sinus, left pterygopalatine plate, pterygopalatine and infratemporal fo

Extraoral anatomy in CBCT – a literature review. Part 3: Retromaxillary region.

In this review about extraoral anatomy as depicted by cone beam computed tomography, the retromaxillary region is discussed. A medium-sized (6 x 6 cm) or large (≥ 8 x 8 cm) field of view of the maxilla will inevitably depict the retromaxillary region that can be considered a «transition» zone between the viscerocranium and the neurocranium. Major structures of the region include the sphenoid bone and the pterygopalatine fossae. The sphenoid bone is a single but complex bone located between the maxilla and the brain. It is composed of a central body, bilateral greater and lesser wings, and pter

Multi-compartment skull base orbital cavernous venous malformation: A rare presentation of a common orbital mass.

PURPOSE: We present a unique case of an orbital intraconal cavernous venous malformation that extended along the trigeminal nerve to the pterygopalatine and middle cranial fossa. Our aim is to describe an atypical presentation of this common orbital vascular mass. OBSERVATIONS: A 57-year-old female presented with right eye proptosis. Orbital magnetic resonance imaging demonstrated a lobulated contrast-enhancing mass involving the right intraconal orbital space, pterygopalatine fossa, and right middle cranial fossa, radiographically presumed to be a schwannoma. Intraoperative and histopatholog

Topography of the third portion of the maxillary artery via the transantral approach in Asians.

The maxillary artery (MA) passes over the lateral pterygoid muscle in the infratemporal fossa and enters the pterygopalatine fossa through the pterygomaxillary fissure. Refractory epistaxis is managed by ligation of the sphenopalatine artery via a transmaxillary-transantral approach; there is considerable risk of complications associated with such invasive surgical approaches. The aim of this study was to describe the gross anatomy and variations therein of the MA and its branches at the pterygopalatine fossa. One hundred hemifaces of embalmed Korean adult cadavers were dissected to establish

Laser-Doppler blood flowmetry measurement of nasal mucosa blood flow after injection of the greater palatine canal.

Laser-Doppler blood flowmetry was used to measure the mucosal blood flow of the inferior turbinate before and after injection of the greater palatine canal with 2 ml of a 0.5 per cent bupivicaine hydrochloride solution. Injection of the greater palatine canal is a useful technique in the control of posterior epistaxis. The arterial blood supply to the inferior turbinate is via a single descending branch of the sphenopalatine artery. We conjectured that injection of the pterygopalatine fossa, via the greater palatine canal, would result in a reduction of blood flow to the inferior turbinate. In

Endoscopic multiport resection of a transspatial, retromaxillary tumor.

Tumors of the retromaxillary tissues can grow to involve multiple deep, skull base spaces, including the pterygopalatine fossa, infratemporal fossa, and the parapharyngeal space. Management of these tumors must account for heterogeneous pathology as well as the critical neurovasculature in and surrounding these spaces. Traditionally, these tumors have been approached the open skull base or anterior craniofacial approaches. Modern endoscopic endonasal and multiport approaches have increasingly become favored for certain tumors in the retromaxillary skull base region. In this report, we present

Loss of reflex tearing after maxillary orthognathic surgery: a report of two cases.

BACKGROUND: Few reports have described the ophthalmic complications that occur after maxillary orthognathic surgery. Since cases of decreased reflex tearing after maxillary orthognathic surgery are extremely rare, we describe 2 cases of loss of reflex tearing after maxillary orthognathic surgery. CASE PRESENTATION: Two Asian women, an 18-year-old and a 32-year-old, suffered from unilateral dryness and irritation caused by maxillary orthognathic surgery. In both patients, Schirmer test (II) showed reduced reflex tearing in 1 eye. Computed tomography showed that the pterygoid plate had been fra

The orbitozygomatic infratemporal fossa approach: a quantitative anatomical study.

Using detailed cadaveric dissections this study has demonstrated and quantified the increase in exposure and additional access gained by using the orbitozygomatic infratemporal fossa approach for neurosurgical access. The surgical window of exposure can be increased by up to 300% when this technique is utilised to facilitate access via either a subtemporal (to access the P2 segment of the posterior cerebral artery) or transsylvian (to access the basilar bifurcation) approach. In addition the distance between the surgeon and the operative field can be decreased by approximately 2-3 cm. The orbi

Endoscopic anatomy of the postganglionic pterygopalatine innervation of the posterolateral nasal mucosa.

BACKGROUND: Recent anatomic dissections have redefined the sinonasal parasympathetic pathway suggesting that individual rami project from the pterygopalatine ganglion (PPG) to innervate the nasal mucosa via multiple small nerve fascicles. The purpose of this study is correlate these anatomic descriptions with endoscopic findings and to define the pattern and location of the posterolateral neurovascular rami. METHODS: Eight cadaver heads were utilized for a total of 16 individual sides. A lateral nasal submucosal flap was elevated while preserving all neurovascular bundles perforating the pala
