Re-thinking the biology of essential tremor: from models to morphology.

Remarkably little has been written on the biology of essential tremor (ET), despite its high prevalence. The traditional pathophysiological model for ET, the olivary model, states that ET is a primary electrical//electrophysiological entity, the result of pacemaking neurons in the inferior olivary nucleus that begin firing in a coupled and rhythmic manner, and thus, through an abnormal olivo-cerebellar output, produce tremor. Though this model is based on several sound neurophysiological observations, there are major problems as well. Despite its shortcomings, however, the model persists. With

Evidence-based management of traumatic thoracolumbar burst fractures: a systematic review of nonoperative management.

OBJECT: The overall evidence for nonoperative management of patients with traumatic thoracolumbar burst fractures is unknown. There is no agreement on the optimal method of conservative treatment. Recent randomized controlled trials that have compared nonoperative to operative treatment of thoracolumbar burst fractures without neurological deficits yielded conflicting results. By assessing the level of evidence on conservative management through validated methodologies, clinicians can assess the availability of critically appraised literature. The purpose of this study was to examine the level

Discovery of novel biomarkers and phenotypes by semantic technologies.

BACKGROUND: Biomarkers and target-specific phenotypes are important to targeted drug design and individualized medicine, thus constituting an important aspect of modern pharmaceutical research and development. More and more, the discovery of relevant biomarkers is aided by in silico techniques based on applying data mining and computational chemistry on large molecular databases. However, there is an even larger source of valuable information available that can potentially be tapped for such discoveries: repositories constituted by research documents. RESULTS: This paper reports on a pilot ex

[Incidence of breast cancer in women with multiple sclerosis: systematic review and meta-analysis of observational cohort studies].

AIM: To determine if women with multiple sclerosis (MS) reported an increased risk of breast cancer compared with the general female population. PATIENTS AND METHODS: We conducted a systematic review and meta-analysis of observational cohort studies. We performed a search in PubMed/Medline of studies where standardized incidence ratios (SIRs) were provided. Overall estimates of the SIRs (meta-SIRs) were calculated using DerSimonian-Laird random effects models. Sources of heterogeneity were explored by means sub-groups analyses. RESULTS: Five studies met the inclusion criteria. Women with MS

Neurosurgical treatment of bipolar depression: defining treatment resistance and identifying surgical targets.

OBJECTIVES: Bipolar disorder (BD) is a complex psychiatric disorder that is often underrecognized, misdiagnosed, and challenging to detect. During the past decade, substantial progress has been made in the development of pharmacotherapeutic and psychosocial interventions for various phases of BD. Notwithstanding these developments, the majority of BD individuals, and particularly patients with bipolar depression, receiving guideline concordant care do not experience syndromal or functional recovery, underscoring the need for novel treatments. Early success with deep brain stimulation (DBS) in

Antipsychotic agents in anxiety disorders: An umbrella review.

BACKGROUND: Although not approved for the treatment of anxiety disorders (except trifluoperazine) there is ongoing off-label, unapproved use of first-generation antipsychotics (FGAs) and second-generation antipsychotics (SGAs) for anxiety disorders. There have been systematic reviews and meta-analyses on the use of antipsychotics in anxiety disorders, most of which focused on SGAs. OBJECTIVE: The specific aims of this umbrella review are to: (1) Evaluate the evidence of efficacy of FGAs and SGAs in anxiety disorders as an adjunctive treatment to traditional antidepressant treatments and other

State of the art review with literature summary on gastric peroral endoscopic pyloromyotomy for gastroparesis.

Gastroparesis is a debilitating progressive disease that significantly impacts a patient's life with limited and challenging treatments available. Although the pathogenesis is multifactorial, pylorospasm is believed to have a major underlying role. Several therapeutic interventions directed to the pylorus have been developed over the last decade, including intra-pyloric injections of botulinum toxin, transpyloric stenting, and surgical pyloroplasty. All of these treatment options had limited and disappointing results. More recently, gastric peroral endoscopic myotomy (G-POEM) has been reported

Neural pathways for colorectal control, relevance to spinal cord injury and treatment: a narrative review.

STUDY DESIGN: Narrative review. OBJECTIVES: The purpose is to review the organisation of the nerve pathways that control defecation and to relate this knowledge to the deficits in colorectal function after SCI. METHODS: A literature review was conducted to identify salient features of defecation control pathways and the functional consequences of damage to these pathways in SCI. RESULTS: The control pathways for defecation have separate pontine centres under cortical control that influence defecation. The pontine centres connect, separately, with autonomic preganglionic neurons of the spina

Delayed Occipital Artery Pseudoaneurysm Following Blunt Force Trauma.

BACKGROUND: Occipital artery pseudoaneurysms are extremely rare pathologies that manifest after traumatic injury; only 11 cases have been reported in the literature. Because of their low incidence and vague symptoms, the initial diagnosis can be difficult. However, for correctly diagnosed occipital artery pseudoaneurysms, many successful treatment modalities exist. METHODS: We review the pathology of occipital pseudoaneurysms, elucidate the reasons for their rarity, discuss effective diagnostic measures, and discuss the currently available treatment options. We also present a case of a 16-yea

Does D-cycloserine enhance exposure therapy for anxiety disorders in humans? A meta-analysis.

The treatment of anxiety is on the edge of a new era of combinations of pharmacologic and psychosocial interventions. A new wave of translational research has focused on the use of pharmacological agents as psychotherapy adjuvants using neurobiological insights into the mechanism of the action of certain psychological treatments such as exposure therapy. Recently, d-cycloserine (DCS) an antibiotic used to treat tuberculosis has been applied to enhance exposure-based treatment for anxiety and has proved to be a promising, but as yet unproven intervention. The present study aimed to evaluate the

We conducted a systematic review and meta-analysis to characterize the relationship between smoking and miscarriage. We searched the PubMed database (1956-August 31, 2011) using keywords and conducted manual reference searches of included articles and reports of the US Surgeon General. The full text of 1,706 articles was reviewed, and 98 articles that examined the association between active or passive smoking and miscarriage were included in the meta-analysis. Data were abstracted by 2 reviewers. Any active smoking was associated with increased risk of miscarriage (summary relative risk ratio

Review of ethanol dispersion, distribution, and elimination from the fetal compartment.

BACKGROUND: This study is a review of alcohol dispersion into and elimination from the fetal compartment. METHODS: PubMed searches were conducted for all years and all languages for relevant papers. We also hand searched the reference list of papers and text books for additional references. RESULTS: Alcohol concentration is determined by body water (49% for women), grams of ethanol consumed, and duration of drinking. The fetus has very limited metabolic capacity and transfer from the fetal compartment to maternal circulation is the major pathway to reduce fetal exposure. Vasoconstriction of

Sex differences in rates of obesity in bipolar disorder: postulated mechanisms.

OBJECTIVE: The increased standardized mortality ratio (SMR) from cardiovascular disease (CVD) in women with bipolar disorder (BD), relative to men with BD and individuals of both sexes in the general population, provides the impetus to identify factors that contribute to the differential association of obesity with BD in women. METHODS: We conducted a selective PubMed search of English-language articles published from September 1990 to June 2012. The key search terms were bipolar disorder and metabolic syndrome cross-referenced with gender, sex, obesity, diabetes mellitus, hypertension, and d

Parietal cortex matters in Alzheimer's disease: an overview of structural, functional and metabolic findings.

Atrophy of the medial temporal lobe, especially the hippocampus and the parahippocampal gyrus, is considered to be the most predictive structural brain biomarker for Alzheimer's Dementia (AD). However, recent neuroimaging studies reported a possible mismatch between structural and metabolic findings, showing medial temporal lobe atrophy and medial parietal hypoperfusion as biomarkers for AD. The role of the parietal lobe in the development of AD is only recently beginning to attract attention. The current review discusses parietal lobe involvement in the early stages of AD, viz. mild cognitive

The Movement Disorders task force review of dysautonomia rating scales in Parkinson's disease with regard to symptoms of orthostatic hypotension.

Orthostatic hypotension is defined as a blood pressure fall of > 20 mm Hg systolic and/or 10 mm Hg diastolic within 3 minutes of an upright position. The Movement Disorders Society commissioned a task force to assess existing clinical rating scales addressing symptoms of orthostatic hypotension in Parkinson's disease. Seven neurologists and a clinimetrician assessed each scale's previous use and critiqued its clinimetric properties. A scale was "recommended" if it had been applied to populations of patients with Parkinson's disease, with data on its use in studies beyond the group that develop

Levodopa-induced dyskinesias.

Levodopa-induced dyskinesias (LID) are common and difficult to treat. This review focuses on three issues related to LID: clinical features, classification and rating, pathophysiology and pathogenesis, and management. The three primary clinical syndromes are OFF-period dystonia, peak-dose dyskinesia, and diphasic dyskinesia. Several other forms also occur, making the evaluation and choice of treatment complicated. A core component of the pathophysiology of LID is overactivity of the direct striatal output pathway. This pathway provides a direct GABAergic connection by which the striatum inhibi

Evidence-based practice: an example related to the use of splinting in a patient with carpal tunnel syndrome.

The purpose of this article was to illustrate a limited search for evidence and its application in the context of a patient with CTS. The search was by no means exhaustive, as it was performed and interpreted in less than an hour. My conclusions are not definitive because my evidence was based almost exclusively on abstracts, and many key details about the studies are unknown. I do not know how closely my specific patient matches those in a particular study. My search allowed me, however, to make an informed response to a patient with legitimate questions. Integrating the evidence with my own
