Successful minimally invasive management of primary maxillary central incisors after lateral luxation: a case report.

Lateral luxation in primary maxillary incisors is a traumatic injury that can lead to darkening of the tooth, pulp necrosis, root resorption, and damage to the permanent teeth. Management of this kind of trauma typically includes pulpectomy or extraction in cases of root resorption. However, in young children, removal of the tooth may be psychological stressing, especially for the parents, while increased mobility can make immediate endodontic treatment difficult to perform. This article outlines a severe palatal luxation on the maxillary right central incisor treated with conservative managem

Rickets - A case report.

Rickets is a disorder caused by a lack of vitamin D, calcium or phosphate. It leads to softening and weakening of the bones. Dental manifestation of rickets includes enamel hypoplasia and delayed tooth eruption. The most important oral findings are characterised by spontaneous gingival and dental abscesses occurring without a history of trauma or caries. Radiographic examination revealed large pulp chambers, short roots, poorly defined lamina dura and hypoplastic alveolar ridge. These dental abscesses are common, and therefore, extraction and pulpectomy are the treatment of choice. Oral manife

Assessment of matrix metalloproteinase-9 in pulpal blood as a biomarker of inflammation in primary teeth: An in vivo pilot study.

BACKGROUND: The dental pulp, derived from mesenchymal tissue, undergoes complex inflammatory changes during the progression of dental caries. Accurate assessment of pulpal status is essential for successful pulp therapy in primary teeth, which are inherently more susceptible to pulpal involvement due to their anatomical characteristics. Conventional diagnostic methods are often unreliable in pediatric patients, particularly when treatment is performed under general anesthesia. Biomarker-based evaluation of pulpal blood may provide an objective alternative for assessing pulpal inflammation. AI

Quantitative analysis of immunoglobulins and inflammatory factors in human pulpal blood from exposed pulps.

The levels of immunoglobulins (IgG, IgA, and IgM) and inflammatory factors (elastase, prostaglandin E2, interleukin-1 alpha, interleukin-1 beta, interleukin-6, and tumor necrosis factor-alpha) in the blood of human dental pulp were quantified, using enzyme-linked immunosorbent assays. The pulpal blood samples were obtained with nylon fiber pellets from normal and inflamed dental pulp at pulp sites exposed on pulpectomy. Statistically significant differences between normal and inflamed pulp were found in the levels of IgG (p < 0.01), IgA (p < 0.05), IgM (p < 0.01), elastase (p < 0.05), and pros

A two-year retrospective study on traumatic dental injury in the primary dentition.

The aim of this study was to analyze the causes, clinical characteristics, social factors, and current status of treatment of traumatic dental injury (TDI) in the primary dentition. A retrospective analysis was performed on 144 children (213 teeth) with TDI in the primary dentition from our hospital between December 2017 and June 2020. Data were analyzed using the chi-square test and the Mann-Withney-Wilcoxon test. Boys accounted for 68.1% (98/144) and girls for 31.9% (46/144) of all 144 children with TDI in the primary dentition, with a boy-girl ratio of 2.13:1. The primary age of TDI in deci

Vital Pulp Therapy as a Conservative Approach for Management of Invasive Cervical Root Resorption: A Case Series.

INTRODUCTION: Clear treatment guidelines with predictable outcomes for invasive cervical root resorption (ICRR) do not exist. Recent studies show that an internal approach for the excavation and repair of resorptive lacunae using bioactive materials might be preferred over external surgical approaches in cases with extensive ICRR. The procedure appears to interrupt and stop the resorptive process and preserves the periodontium. The aim of this case series was to report on the clinical and radiographic outcomes in cases with ICRR treated with vital pulp therapy techniques using calcium-enriched

Immunohistochemical examination on the localization of macrophages and plasma cells in induced rat periapical lesions.

To investigate the role of plasma cells and macrophages in the development of periapical lesions, we immunohistochemically examined the distribution of these inflammatory cells in experimental induced rat periapical lesions after pulpectomy. The number of ED1-positive mononuclear cells increased rapidly, reached a plateau which remained stable between days 10 and 60, and subsequently decreased. Immunoglobulin (Ig)-bearing plasma cells appeared after 60 days, and, of these, IgG-bearing plasma cells were predominant after 90 days. The radiographic and histopathological findings indicated the dev

Anatomical challenges, electronic working length determination and current developments in root canal preparation of primary molar teeth.

Paediatric endodontics is an integral part of dental practice that aims to preserve fully functional primary teeth in the dental arch. Pulpectomy of primary molars presents a unique challenge for dental practitioners. Negotiation and thorough instrumentation of bizarre and tortuous canals encased in roots programmed for physiological resorption are the main challenges for this treatment approach. Consequently, numerous in vitro and in vivo studies have been conducted to validate the application of some contemporary endodontic armamentarium for effective treatment in primary molars whilst maint

Exposed human pulp caused by trauma and/or caries in primary dentition: a histological evaluation.

The aim of this study was to compare the pulpal reactions after exposure due to trauma and/or caries and to suggest the appropriate treatment options for the management of pulp, in the light of the histological findings in the primary teeth. Fifteen primary maxillary incisors with pulpal exposure were examined with light microscope in a blind study. After the histological evaluation, teeth were divided into three groups for the etiological factors due to the exposure according to the patient's files: trauma group, caries group and caries and trauma group. The inflammatory infiltration was dimi

Sodium hypochlorite pulpotomies in primary teeth: a retrospective assessment.

PURPOSE: In 2007, the University of Iowa's advanced training program in pediatric dentistry replaced the traditional formocresol vital pulpotomy technique with a 5% sodium hypochlorite (NaOCl) technique. The purpose of this study was to evaluate the clinical/radiographic success over 21 months of 5% NaOCl as the medicament in primary molar pulpotomies compared to published data for formocresol and ferric sulfate pulpotomies. METHODS: A retrospective chart audit was performed to evaluate results for all primary molar pulpotomies completed during a 12-month period using NaOCl. Dental records we

Determinants of Definitive Full Pulpotomy Adoption in Symptomatic Irreversible Pulpitis: A Multivariable Analysis of Dental Trainee Decision-Making.

BACKGROUND: Full pulpotomy has gained increasing attention as a conservative treatment option for managing complicated crown fractures and pulp exposures in mature permanent teeth. However, little is known about how undergraduate dental students perceive this treatment approach and which factors influence their willingness to adopt it in clinical practice. OBJECTIVE: This study aimed to evaluate undergraduate dental students' knowledge, attitudes, and preferences regarding full pulpotomy and to identify factors associated with willingness to use full pulpotomy as a definitive treatment option

The Impact of Vital Pulp Therapy on Normal Root Development in Immature Teeth: A Case Report.

Vital pulp therapy is one of the least invasive and simplest and most economically acceptable treatment alternatives for immature teeth with pulpitis. The success rate of this treatment, with calcium silicate-based cements, ranges from 85% to 100%. Vital pulp therapy prevents the development of apical periodontitis and promotes normal root development, allowing the tooth to remain in the dentition and perform its functions. The patient was a nine-year-old boy with pain on chewing and a positive response to cold. The panoramic radiograph showed an immature permanent lower molar with deep caries

Dental Management of a Child With Sickle Cell Anemia: A Case Report.

Sickle cell disease (SCD) has been identified as one of the most prevalent genetic conditions. It alters the shape and function of red blood cells. This brief case report presents a case of a five-year-old male with sickle cell disease who complained of pain in the left mandibular region due to deep proximal caries. Before dental management, a complete fitness evaluation was performed with the help of a pediatrician, followed by informed consent. Dental management includes pulpectomy followed by stainless steel crown placement and Glass ionomer cement (GIC) restoration for superficial caries.

Evaluation of Clinical Efficiency of Two Single-file Ni-Ti Rotary Systems in Performing Root Canal Treatment in Primary Teeth: A Randomized Clinical Trial.

BACKGROUND: The conventional procedure using stainless steel hand files is very time-consuming and has associated disadvantages. A literature review demonstrates the effectiveness of nickel-titanium (Ni-Ti) rotary files in this regard. Technological advancements have led to the development of various single-file Ni-Ti rotary systems, such as Kedo-S and XP-Endo Shaper. AIM: To compare the instrumentation time and quality of obturation in primary molars, assessed by the Coll and Sadrian criteria, using two single-file Ni-Ti rotary systems, Kedo-S and XP-Endo Shaper. Generalised hypertaurodontism involving all primary molars in a non-syndromic child.

Generalised hypertaurodontism involving all primary molars is an extremely rare finding, particularly in non-syndromic children. This case describes a male child presenting with pain in the mandibular posterior region. Clinical examination revealed multiple carious lesions while radiographic evaluation demonstrated taurodont morphology affecting all primary first and second molars. Quantitative analysis using the Shifman and Chanannel index confirmed hypertaurodontism in all eight primary molars. Comprehensive clinical assessment excluded associated syndromic conditions. The symptomatic tooth

Case Report: Root resorption caused after pulp death of adjacent primary molar.

Necrotic decayed primary molars with necrotic pulp tissues may show periapical involvement and root resorption. In this case report, a pediatric patient with a very common chief complain and clinical picture of necrotic badly decayed molar, introduced a very interesting case when radiographic investigation was performed, which showed that root resorption of the adjacent healthy molar occurred. The current report is, to the best of our knowledge, the first to report such finding in primary dentition.
