Severe Aqueous-Deficient Dry Eye Following Herpes Zoster Ophthalmicus With Periocular Inflammation: A Case Series.

This series aimed to describe the clinical characteristics and course of severe aqueous-deficient dry eye (ADDE), a condition characterized by tear deficiency and reduced corneal sensation, following ophthalmic herpes zoster infection with periocular inflammation. Four cases of ADDE that developed after ophthalmic herpes zoster infection were retrospectively examined. Clinical data, including disease course, medical history, imaging findings, tear volume assessment, corneal sensitivity testing, and treatment outcomes, were collected. Our patients exhibited severe tear deficiency, significant c

Filamentary keratitis: A review.

Filamentary keratitis (FK) is a clinical sign of underlying ocular and systemic conditions. FK can cause significant irritation, tearing, and photophobia in the eye. It is a refractory debilitating condition caused by dry eye that affects the day-to-day activities of patients. The etiopathogenesis of FK is not well known; there are numerous predisposing causes. The condition starts as a sub-epithelial or Bowman's membrane dysfunction and leads to the shedding of epithelial cells that take a strand-like form and attach to the cornea. These strands are surrounded by mucin and continue to elongat

Current Advances in Regenerative Strategies for Dry Eye Diseases: A Comprehensive Review.

Dry eye disease (DED) is an emerging health issue affecting millions of individuals annually. Ocular surface disorders, such as DED, are characterized by inflammation triggered by various factors. This condition can lead to tear deficiencies, resulting in the desiccation of the ocular surface, corneal ulceration/perforation, increased susceptibility to infections, and a higher risk of severe visual impairment and blindness. Currently, the clinical management of DED primarily relies on supportive and palliative measures, including the frequent and lifelong use of different lubricating agents. W

Sustainable Solutions for Eye Drop Plastic Waste: Challenges, Innovations, and Environmental Impact.

The widespread use of single-use plastic eye drop bottles in ophthalmology presents a significant environmental challenge. While essential for preventing contamination and ensuring patient safety, these bottles contribute to plastic waste due to their small size, multimaterial composition, and inadequate recycling infrastructure. This review examines the environmental impact of eye drop packaging across various ophthalmic conditions, including dry eye disease, cataract surgery, and glaucoma, highlighting the substantial cumulative burden of plastic waste and carbon emissions. The manuscript ex

[Evaluation of canalization of the canaliculus with digital photo system for treating dry eye].

PURPOSE: We developed a simple evaluation technique of canalization of canaliculus by observing sequential images of the meniscus height after instillation of fluorescein solution. SUBJECTS AND METHODS: We used nine eyes of eight patients [one male eye, eight eyes of seven females, age: 68.3 +/- 15.9 (mean +/- standard deviation)] whose punctal plugs were extruded due to granulation formation within the canaliculi. Fluorescein solution was instilled into the eyes, until the tear meniscus was of sufficient height. Then the tear meniscus at the center of the lower lid margin was photographed wi

Wavefront sensing and the dynamics of tear film.

PURPOSE: To summarize our previous studies regarding the influence of tear-film dynamics on optical quality after blinking. METHODS: Sequential higher-order aberration (HOA) measurements were performed by using a wavefront sensor. During the measurements, subjects were asked to blink every 10 seconds. The obtained aberration data were analyzed in the central 4-mm diameter for coma-like, spherical-like, and total HOAs up to the sixth-order Zernike polynomials. RESULTS: Serial measurements of HOAs in healthy eyes showed variations even in clinically normal subjects and were classified into 4 g

When ocular surface treatment fails: exploring neuropathic ocular pain in aqueous-deficient dry eye due to Sjögren's syndrome treated with mirogabalin: a case study.

Dry eye disease can be classified as aqueous-deficient, evaporative and mixed-type. Recently, neuropathic pain has been indicated to be involved in the subjective symptoms of aqueous-deficient dry eye. However, the mechanism underlying neuropathic ocular pain in such patients remains elusive. Here, we present the case of a patient with aqueous-deficient dry eye disease and severe corneal and conjunctival epithelial damage due to Sjögren's syndrome. Despite considerable improvements in ocular surface conditions with local treatments, such as punctal plugs, autologous serum eye drops, rebamipide

New strategies for the management of ocular surface disease in glaucoma patients.

PURPOSE OF REVIEW: Glaucoma patients commonly suffer from ocular surface disease (OSD). As treatment strategies, medications and devices for the treatment of OSD as well as glaucoma surgical approaches evolve rapidly, it is important to consider their application to these patients. RECENT FINDINGS: OSD in glaucoma patients may lead to reduced reliability of diagnostic tests, decreased medication compliance, poor surgical outcomes, and overall decreased quality of life. Chronic use of topical glaucoma medications has been linked to the development of limbal stem cell deficiency, and the role o

Meretoja's Syndrome: Lattice Corneal Dystrophy, Gelsolin Type.

Lattice corneal dystrophy gelsolin type was first described in 1969 by Jouko Meretoja, a Finnish ophthalmologist. It is caused by an autosomal dominant mutation in gelsolin gene resulting in unstable protein fragments and amyloid deposition in various organs. The age of onset is usually after the third decade of life and typical diagnostic triad includes progressive bilateral facial paralysis, loose skin, and lattice corneal dystrophy. We report a case of a 53-year-old female patient referred to our Department of Ophthalmology by severe dry eye and incomplete eyelid closure. She had severe bil

Update on the Medical Treatment of Primary Open-Angle Glaucoma.

Glaucoma comprises a group of progressive, neurodegenerative disorders characterized by retinal ganglion cell death and nerve fiber layer atrophy. Several randomized controlled trials have consistently demonstrated the efficacy of intraocular pressure lowering to slow or halt the measurable progression of the disease. Medical therapy, in places where it is easily accessible, is often the primary method to lower intraocular pressure. We review the medical options currently available and possible future options currently in development. The 5 contemporary classes of topical agents in use include

Lacrimal Canaliculitis: A Major Review.

Lacrimal canaliculitis is an inflammation of the proximal lacrimal drainage system. It classically presents with symptoms of redness, mucopurulent discharge, medial canthal swelling, epiphora, and pouting punctum. Despite having classical clinical characteristics it is frequently misdiagnosed. The cause can be primarily due to various infectious agents or secondary mostly due to the use of punctal plugs. There are no universally accepted guidelines for the management of canaliculitis but different medical and surgical options have been employed with varying success rates and it is notorious fo

Recent advances in ophthalmic drug delivery.

Topical ocular drug bioavailability is notoriously poor, in the order of 5% or less. This is a consequence of effective multiple barriers to drug entry, comprising nasolacrimal drainage, epithelial drug transport barriers and clearance from the vasculature in the conjunctiva. While sustained drug delivery to the back of the eye is now feasible with intravitreal implants such as Vitrasert (-6 months), Retisert (-3 years) and Iluvien (-3 years), currently there are no marketed delivery systems for long-term drug delivery to the anterior segment of the eye. The purpose of this article is to summa
