Role of surgery in hypertensive intracerebral hematoma. A comparative study of 305 nonsurgical and 154 surgical cases.

The indications for surgery in hypertensive intracerebral hematoma are still controversial. The reason for this may be: 1) lack of adequate and comparable data in conservative and surgical therapy from the same institution; 2) lack of adequate close follow-up monitoring over an extended period of time; or 3) lack of proper classification of hematomas for comparison of results from different institutions. The authors have treated 459 cases of hypertensive intracerebral hematoma between October, 1975, and July, 1983. The hematomas have been classified according to their mode of extension on comp

Brain herniations in patients with intracerebral hemorrhage.

OBJECTIVES: To study the types, frequency and clinical correlates of brain herniations in patients with intracerebral hemorrhage (ICH). METHODS: In 24 patients with ICH (putaminal 22 and thalamic 2) features of raised intracranial pressure (ICP), such as hyperventilation, extensor rigidity, pupillary asymmetry and pyramidal signs on the non-hemiplegic side, were recorded. Depth of coma was assessed by using the Glasgow Coma Scale (GCS) and severity of stroke by using the Canadian Neurological Scale (CNS). On MRI, evidence of herniation, horizontal and vertical shifts and the edema-hematoma co

[A case of transient parkinsonism due to mesencephalic hemorrhage].

A 68-year-old man was admitted because of sudden onset gait disturbance and bradykinesia. He experienced left putaminal bleeding following mild right hemiparesis and emotional incontinence without any difficulties in his daily life since three years before admission. On neurological examination the patient was alert and oriented. He showed forced crying. Myerson's sign was positive. There were no abnormal findings in ocular movements, pupillary reflexes and other cranial nerves. Muscle tone was increased with cog-wheel phenomenon in bilateral upper extremities. Coordination was preserved. He s

Intracerebral Hemorrhage During the Chronic Phase of Eosinophilic Granulomatosis With Polyangiitis: A Case Report Emphasizing Cerebrovascular Vulnerability.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare form of small-vessel vasculitis typically associated with eosinophilia, asthma, and systemic inflammation. Although peripheral neuropathies are relatively common, central nervous system involvement, especially intracerebral hemorrhage during the remission phase of EGPA, is uncommon, and its clinical course and management remain poorly understood. The female patient had been diagnosed with EGPA 6 months earlier based on marked eosinophilia of 16,380/μL, late-onset asthma, and mononeuritis multiplex, fulfilling the 2022 American Coll

[Decreased cerebral blood flow in remote areas in deep-seated cerebrovascular lesions].

Recent positron or single photon emission computed tomographic studies have demonstrated that the impairment of regional hemodynamics or metabolism in cerebrovascular disease is not limited to the focal lesion, but also involves more remote areas. The present study evaluated cerebral blood flow in areas remote from the primary lesion in patients with subcortical cerebrovascular disease, and assessed correlations with the clinical features. Twenty patients aged 61-89 (mean, 74.7 +/- 6.7 years) were divided into the L group (lacunar infarction in deep penetrating arterial territory, n = 7), the

Extended superciliary approach for middle cerebral artery embolectomy after unsuccessful endovascular recanalization therapy: technical note.

OBJECTIVE: Although an intra-arterial mechanical embolectomy extends the therapeutic time window for restoration of the cerebral blood flow, its suboptimal recanalization rate also necessitates a salvage procedure if the patient is still within the therapeutic time window. As such, a surgical embolectomy can be performed in a rapid and less invasive manner for an acute middle cerebral artery (MCA) occlusion. The technical details of this procedure are discussed and demonstrated. METHODS: Four patients with an acute MCA occlusion were treated using a novel surgical embolectomy technique after

Imaging findings after methanol intoxication (cohort of 46 patients).

OBJECTIVES: Our goal is to demonstrate the variability of imaging findings, primarily in the MRI, in 46 patients who survived acute methanol poisoning. This cohort of patients is the largest such sample group examined by MRI. METHODS: Patients were examined by means of imaging methods (42 patients by MRI and 4 by CT). All had an identical protocol of MR examination (T2WI, FLAIR, T1WI with or without application of contrast medium and T2WI/FFE, DWI in the transversal plane of the scan, and with focus on the optic nerves in the coronal plane of the scan in T2WI-SPIR). RESULTS: Imaging methods

Dialysis-induced worsening of cerebral edema in intracranial hemorrhage: a case series and clinical perspective.

BACKGROUND: Intracranial hemorrhage (ICH) is not an uncommon complication of end-stage renal disease (ESRD), and may be complicated by cerebral edema. Hemodialysis (HD) may induce rapid osmolar and fluid shifts, increasing brain water content with the potential to worsen cerebral edema. The dangers of HD in patients with acute ICH have only been highlighted in isolated reports. METHODS: Case series and review of relevant literature. RESULTS: Two patients with ESRD presented with acute ICH (one with putaminal hematoma, the other with bilateral subdural hematomas) and developed fatal/near-fata

Pure motor hemiplegia: CT study of 30 cases.

Pure motor hemiplegia (PMH) is a well defined syndrome usually caused by ischemic lesions of lacunar type located either in the internal capsule or in the pons. Angiography and isotope scanning are usually normal. CT scan reveals small deep infarcts and appears to be the most reliable investigative method. The CT scan findings are described of thirty patients with PMH of rapid onset (less than 36 hours). In 29 of the 30 cases a lesion was found which could explain the PMH. Small hemorrhages (2 cm in diameter) in the posterior limb of the internal capsule were noted in two cases. Ischemic lesio

[Spinal myoclonus in association with brain death].

A 56-year-old, hypertensive man was admitted to our hospital because of confusion. He had been well unit half an hour before admission, when he had suddenly developed left hemiplegia. Although he became deep coma soon after admission, his vital signs were preserved. CT scan revealed a large right putaminal hemorrhage and a ventricular perforation. The condition was too severe for surgical approaches and conservative therapy for brain edema was performed. On the 2nd hospital day, corneal and light reflexes were disappeared and an anisocolia appeared. On the 3rd hospital day, right papilledema a

Gait Training after Stroke with a Wearable Robotic Device: A Case Report of Further Improvements in Walking Ability after a Recovery Plateau.

BACKGROUND: Conventional rehabilitation is known to improve walking ability after stoke, but its effectiveness is often limited. Recent studies have shown that gait training combining conventional rehabilitation and robotic devices in stroke patients provides better results than conventional rehabilitation alone, suggesting that gait training with a robotic device may lead to further improvements in the walking ability recovered by conventional rehabilitation. Therefore, the aim of this report was to highlight the changes in kinematic and electromyographic data recorded during walking before a

Auditory agnosia caused by bilateral putamen haemorrhage.

A 55-year-old right-handed man with a history of hypertension suddenly fell and developed right hemiparesis. Neurological examination revealed that he was alert, but did not appropriately respond to verbal questions and commands. Detailed examination revealed that he could correctly respond to written commands. His speech was almost fluent, showing no paraphasia and normal articulation. His written sentences were legible. Pure tone audiometry showed that his auditory acuity was relatively preserved. His brainstem auditory evoked potential components from I to V were recorded bilaterally with n

[Cerebrovascular disorder and the language areas].

Recent progress of imaging techniques has achieved precise descriptions of aphasic syndromes associated with cerebrovascular disorders. Infarction of the left anterior cerebral artery brings about transcortical motor aphasia. Occlusion of each branches of the left middle cerebral artery produces characteristic language symptoms. Recently, cases with infarction of the areas of prefrontal and precentral arteries were reported to manifest fluent aphasia indistinguishable from the classical transcortical sensory aphasia. Rare cases with lesions restricted to the Broca's area (territory of the prec

[Osteoma of the sphenoid sinus--report of two cases].

Although osteoma of the paranasal sinus is comparatively common, involvement of the sphenoidal sinus region is extremely rare. Lame reported that a total of twelve cases had been described in the European literature since 1800. Mikaelin, Kulczynski and Dolan, since then, described three cases. This paper deals with two surgical cases of sphenoidal sinus osteoma, reviewing literature on paranasal sinus osteoma and differential diagnosis in the sphenoidal sinus lesion. Case 1. A 20-year-old man was referred to us by an ophthalmologist, complaining of left visual disturbance (visual acuity: 0.02)

Stimulating neural plasticity with real-time fMRI neurofeedback in Huntington's disease: A proof of concept study.

Novel methods that stimulate neuroplasticity are increasingly being studied to treat neurological and psychiatric conditions. We sought to determine whether real-time fMRI neurofeedback training is feasible in Huntington's disease (HD), and assess any factors that contribute to its effectiveness. In this proof-of-concept study, we used this technique to train 10 patients with HD to volitionally regulate the activity of their supplementary motor area (SMA). We collected detailed behavioral and neuroimaging data before and after training to examine changes of brain function and structure, and co
