The canine cystic endometrial hyperplasia (CEH)-pyometra is one of the prevalent uterine pathological conditions associated with infertility and often turn fatal. INTRODUCTION: Spontaneous uterine perforation due to pyometra is a rare but serious condition. Pyometra refers to the accumulation of purulent material within the uterine cavity, typically resulting from an obstruction of the cervical canal combined with infection. When left untreated, the pressure from the accumulated fluid and the inflammatory response can lead to spontaneous uterine rupture. This condition often presents as an acute abdomen and can be life-threatening particularly in postmenopausal women.

Hematological parameters, plasma C-reactive protein (CRP), and tumor necrosis factor alpha were analyzed in 64 dogs with a presumptive diagnosis of pyometra. Spontaneous perforation is a very rare complication of pyometra. We report herein the case of an 88-year-old woman who presented with muscular rigidity and free air on abdominal X-ray films. Perforation of the gastrointestinal tract was diagnosed preoperatively, and an emergency laparotomy was performed. The peripheral plasma progesterone levels in bitches with spontaneously developed pyometra did not differ significantly from those in normally cycling or pregnant bitches. The macroscopic ovarian morphology gave no indication of an increased corpus luteum activity. Ovarian follicular cysts were found in 4 out of 31 cases. There was no detectable difference of results between 14 bitches that had received hormonal medication 10 years to 1 week previous to the pyometra diagnosis, and the other patients.
