[Right pyriform sinus fistula with infection of left neck: a case report].

In this article, a case of right pyriform fossa fistula, with left cervical infection as the main manifestation is reported. Ultrasound showed that there was an inhomogeneous echo mass in the dorsal side of the left lobe of thyroid. MRI showed that there were infectious signal behind the bilateral thyroid gland, but mainly on the left side. Fistula was found in the right pyriform fossa under suspension laryngoscope, and CO₂ laser cauterization was performed to cauterize this fistula.

Differences in the diagnosis and management of pyriform sinus fistula between newborns and children.

Pyriform sinus fistula (PSF) is a rare congenital entity. We hypothesized that clinical features, diagnosis and treatment may differ between newborns and children. 190 patients diagnosed with PSF were divided into two groups: neonatal (n = 15) and childhood (n = 175). The medical records including demographic and clinical data were retrospectively analyzed. There were 102 boys and 88 girls in this study. Most patients presented as a left-sided neck lesion. A neck mass, with or without infection and respiratory distress, was the common finding in newborn patients. Prenatal diagnosis was made in

Microlaryngoscopic surgery for pyriform sinus fistulas in children: a report of two cases.

BACKGROUND: A pyriform sinus fistula (PSF) is a rare branchial anomaly that causes recurrent cervical infections. Open neck surgery has widely been accepted as a definitive treatment for PSFs, and endoscopic surgery has been reported in recent years. However, both approaches are not satisfactory because of high recurrence rates and postoperative complications. Microlaryngoscopic surgery (MLS) is a transoral surgical technique involving the use of an operating microscope. In this report, we present a new procedure involving MLS for resection and closure of a PSF without a skin incision. CASE P

Clinical outcomes of weekly cisplatin chemoradiotherapy for patients with pyriform sinus cancer.

BACKGROUND: Pyriform sinus squamous cell carcinoma (SCC) has one of the worst prognoses of all upper aerodigestive tract cancers. Improving clinical outcomes for patients with hypopharyngeal SCC has been particularly challenging for head and neck surgeons and oncologists. METHODS: We investigated 30 patients with pyriform sinus SCC to verify the effectiveness of weekly cisplatin chemotherapy with concurrent radiotherapy. Cisplatin was administered at a dose of 40 mg/m(2) on weeks 1, 2, 3, 5, 6, and 7 during definitive radiotherapy with the aim of preserving the larynx. RESULTS: All 30 patien

[Remaining laryngeal flap reconstruction of the hypopharynx in pyriform sinus carcinoma resection].

OBJECTIVE: To discuss the feasibility of using remaining laryngeal flap to reconstruct the hypopharyngeal defect in pyriform sinus carcinoma resection. METHOD: Seven patients with pyriform sinus carcinoma to reconstruct hypopharyngeal defect with remaining laryngeal flap were summarized. Half the hypopharynx and half the larynx were resected and unaffected half the larynx was reserved. On the unaffected side, thyroid, cricoid and arytenoid cartilages were removed and inner perichondrium was reserved. A 4.0 cm x 2.0 cm laryngeal flap was made with a broad base on the base of tongue. A new role for computed tomography in the diagnosis and treatment of pyriform sinus fistula.

Neck abscess located in or around the thyroid gland should raise the suspicion of acute suppurative thyroiditis, pyriform sinus fistula, a 3rd or 4th branchial cleft anomaly. Differentiating between these entities on a clinical basis is difficult. After treating the initial infection, computed tomography, barium esophagography, ultrasound, and endoscopy can be used in search of the fistulous tract that can be associated with a pyriform sinus fistula or a 3rd or 4th branchial anomaly. We present a case of a pyriform sinus fistula involving the use of oral contrast, combined with computed tomogr

Congenital cysts of the third and fourth pharyngeal pouches or pyriform sinus cysts.

Cysts arising from the III and IV pharyngeal pouches, although uncommon, are typical in their presentation. They occur in neonates, invariably in the left anterior neck and cause respiratory distress. Excision of the cyst with ligation of the tract at the level of the pyriform sinus is curative.

[On the mechanism of hemilaryngeal fixation in pyriform sinus carcinoma and its clinical significance].

To approach the mechanism of hemilaryngeal fixation in pyriform sinus carcinoma (PSC), surgical specimens from total laryngectomy and partial hypopharyngectomy in 43 cases of pyriform sinus carcinoma were studied using organ subserial sections. Invasion of intralaryngeal muscles and the upper and outer aspects of arytenoid cartilage via paraglottic space was found to be the main cause of hemilaryngeal fixation in these cases. Presence of tumor on the medial wall of the pyriform sinus makes involvement of intralaryngeal structures easy, and thus is more frequently seen with hemilaryngeal fixati

The use of pyriform sinus mucosa for reconstruction after vertical partial laryngectomy.

This prospective study included 30 patients with glottic carcinoma of the larynx who were treated primarily by vertical partial laryngectomy. The pyriform sinus mucosa was used to reconstruct the new laryngeal wall and to form a pseudo-vocal fold at the side of resection. Our results showed that 90 per cent of patients were decannulated, 80 per cent were able to eat a normal diet, 70 per cent developed excellent voice quality after surgery and a 97 per cent three-year survival was achieved. The pyriform sinus mucosa proved to be a reliable material for reconstruction after vertical partial lar

[Acute thyroiditis caused by Eikenella corrodens and abnormality of the left pyriform sinus].

A case of suppurative thyroiditis is reported. The initial course was insidious and mimicked De Quervain subacute thyroiditis. The abscess was surgically drained. It contained numerous Eikenella corrodens bacilli. Fibroscopy of pharyngo-laryngeal region showed that the left pyriform sinus was abnormal. Surgical removal of a fistula of the fourth branchial pouch was performed in order to prevent recurrence of the thyroiditis.

Squamous cell carcinoma of the pyriform sinus treated with radical radiation therapy.

This is an analysis of 54 patients with squamous cell carcinoma of the pyriform sinus treated with radical irradiation at the University of Florida between October 1964 and March 1984. There is a 2-year minimum follow-up on all patients and 85% have at least 5 years of follow-up. Patients were excluded from analysis of disease control at the primary site, neck, and/or above the clavicles if they died less than 2 years from treatment with the site(s) continuously disease-free. All patients were included in the analysis of complications and survival. The rates of local control following initial

Combined therapy vs curative radiation in the treatment of pyriform sinus carcinoma.

An attempt has been made to compare the long-term results using preoperative radiation and surgery with radiation therapy alone in the treatment of squamous cell carcinoma of the pyriform sinus. There is a 33% survival for the combined therapy group and a 6% survival for the curative radiation group. Large primary lesions and the presence of cervical adenopathy portend a grave prognosis. The various surgical procedures used are presented with the extent of the primary lesion as the determining factor.

Bocca's Technique Hemipharyngo-Total-Laryngectomy for Right Pyriform Sinus Squamous Cell Carcinoma.

This report describes the step-by-step surgical technique of Bocca's hemipharyngo-total laryngectomy for right pyriform sinus squamous cell carcinoma in an elderly patient with significant comorbidities. The procedure allowed oncologically radical resection with immediate neohypopharyngeal reconstruction using the preserved hemilaryngeal mucosa, avoiding flap reconstruction. Bocca's technique represents a valuable option in carefully selected cases, providing adequate functional outcomes while reducing operative complexity.

Endoscopic electrocauterization for congenital pyriform sinus fistula treatment in paediatrics. Case series.

Pyriform sinus fistulas are rare anomalies of the branchial arches. Most of them are located on the left side. They extend from the apex of the pyriform sinus of the hypopharynx to the thyroid gland or adjacent tissues. The diagnosis is suspected in the presence of acute suppurative thyroiditis or recurrent cervical abscesses, and is confirmed by endoscopic visualization of the fistula hole. The traditional treatment consists of excision of the fistulous tract, with or without thyroid lobectomy, by cervical approach. However, less invasive alternatives that obliterate the path of the fistula h

Endoscopic cauterization of congenital pyriform fossa sinus tracts: an 18-year experience.

IMPORTANCE: Congenital pyriform fossa sinus tracts predispose to neck masses and neck abscesses in pediatric and occasionally adult patients. Traditional management involves open excision with substantial potential morbidity. Endoscopic management allows an alternative, less morbid treatment approach. OBJECTIVE: To evaluate the long-term effectiveness of endoscopic cauterization as definitive treatment for pyriform fossa sinus tracts. DESIGN, SETTING, AND PATIENTS: Retrospective review of the medical records of 23 children (aged 7 months to 14 years) with pyriform fossa sinus tracts treated
