[Emergency rescue in accidents with HDR afterloading units].

PROBLEM: HDR brachyradiotherapy has minimized the exposure to radiation of the personnel working in this field. Nonetheless there are periodically reported troubles with afterloading units concerning the retraction of sources that require immediate action for the limitation of possible damage. LEGAL PRINCIPLES ACCORDING TO THE GERMAN REGULATION CONCERNING PROTECTION AGAINST RADIATION (STRAHLENSCHUTZVERORDNUNG = STRLSCHV): If in afterloading brachyradiotherapy the radiation source remains extended through malfunction we deal with an emergency according to the StrlSchV. The rescue personnel shou

Optic disc and peripapillary tumors.

This review covers the tumors either originating or seen close to the optic disc in the peripapillary area. Most of the optic disc tumors are diagnosed on clinical examination. In a few atypical cases, ancillary imaging, such as optical coherence tomography, fluorescein angiography, and ultrasonography, may be required to demonstrate characteristic features. The majority of these tumors have systemic associations, and hence, a multidisciplinary approach involving ocular oncologists, neurophysicians, radiation oncologists, and medical oncologists is the need of the hour. The treatment of optic

A multidisciplinary approach to gastrointestinal bleeding in cancer patients.

Gastrointestinal bleeding remains one of the most challenging areas in medicine today, especially in the patient with cancer. New techniques and technology have added much to the diagnosis and management of gastrointestinal bleeding. Although gastroenterologists remain at the center of the evaluation of these patients, the incorporation of new techniques has meant that interventional radiologists and nuclear medicine specialists often have important roles in assessing and treating these patients as well. Because patients with cancer are members of a unique group, gastrointestinal bleeding may

[Surgical treatment of vertebral metastases].

The multiplicity of clinical presentations and the lack of consensus explain that the treatment of spine metastasis remains controversial. Optimal treatment requires a truly multidisciplinary approach, involving oncologists, interventional radiologists, radiation oncologists, rheumatologists and spine surgeons. Recent progress in all these areas have allowed to provide safe and effective therapeutic solutions tailored to each situation. We remind, in this work, the main progress in the surgical field, specifying the role of surgery in the current therapeutic arsenal.

Prediction and Monitoring of Adaptive Radiation Therapy Timing Using Two-dimensional X-ray Image-based Water Equivalent Thickness.

BACKGROUND/AIM: This study aimed to predict the optimal timing for adaptive radiation therapy (ART) using two-dimensional X-ray image-based water equivalent thickness (2DWET). PATIENTS AND METHODS: Forty patients with oropharyngeal and hypopharyngeal cancer underwent Computed Tomography (CT) rescanning during treatment. An adaptive score (AS) was proposed to guide ART decisions based on changes in four dose indices: target coverage, spinal cord dose, parotid gland dose, and over-dose volume. Delivered dose distributions were reviewed by two head and neck radiation oncologists, with ART requir

Managing Patients with Inflammatory Bowel Disease Who Develop Prostate Cancer.

Prostate cancer is the most common cancer among men in the USA. Interestingly, recent studies suggest that patients with inflammatory bowel disease (IBD) are at increased risk of developing prostate cancer. Importantly, patients with IBD who develop prostate cancer require thoughtful care when using immunosuppressants to treat the IBD in the setting of malignancy. Further, consideration must be given to the proximity of the prostate to the gastrointestinal tract when treating with radiation where there is concern for the effects of inadvertent exposure of radiation to the diseased bowel. In ge

25-year perspective on prostate cancer: Conquering frontiers and understanding tumor biology.

Major changes in the field of prostate cancer over the last 25 years include the implementation of prostate specific antigen screening and the recognition that BRCA confers hereditary risk of prostate cancer. Quality of life and survivorship have driven risk stratification for localized prostate cancer, facilitated by molecular signatures and leading to increased acceptance of active surveillance as a mainstream treatment option. Advances in technology have improved efficacy and reduced toxicity in both radical prostatectomy and radiation therapy for localized prostate cancer. Improved underst

Role of radiation therapy in the treatment of benign ocular diseases.

Radiation therapy, although a prime treatment modality for malignant disease, has few definitive indications for some benign ocular diseases too. Orbital pseudotumour, Graves ophthalmopathy and pterygium are the conditions where the radiation oncologist has got an important role to play. This article reviews the current status of the role of radiation and its effectiveness in the management of these diseases.

Role of chemotherapy in the management of advanced thymic tumors.

Chemotherapy has an important role in the treatment of advanced thymic tumors. Early stage tumors are successfully treated with surgery. Locally advanced tumors (Masaoka stage III and IVA) are often treated with combined modality treatment including surgery, radiation, and chemotherapy. For patients with curable thymic tumors, the ability to attain a complete resection is a critical prognostic factor. Locally advanced tumors have a relatively high risk of recurrence and decreased rates of long-term survival. A multimodality approach including induction chemotherapy and postoperative radiation

Radiation sigmoiditis mimicking sigmoid colon cancer after radiation therapy for cervical cancer: the implications of three-dimensional image-based brachytherapy planning.

External-beam radiation therapy with intracavitary high-dose-rate brachytherapy is the standard treatment modality for advanced cervical cancer; however, late gastrointestinal complications are a major concern after radiotherapy. While radiation proctitis is a well-known side effect and radiation oncologists make an effort to reduce it, the sigmoid colon is often neglected as an organ at risk. Herein, we report two cases of radiation sigmoiditis mimicking sigmoid colon cancer after external-beam radiation therapy with intracavitary high-dose-rate brachytherapy for uterine cervical cancer with

Recommendations for radiotherapy in patients with lung cancer and interstitial lung disease: A Delphi consensus process of the Italian association of radiotherapy and clinical oncology (AIRO).

AIM: Interstitial lung diseases (ILD) are a group of disorders affecting the lung parenchyma. ILD patients have a higher risk of lung cancer and worse prognosis, with increased susceptibility to radiotherapy (RT)-related toxicities due to ILD exacerbations. There are no clear guidelines for managing lung cancer patients with ILD undergoing RT, alone or with systemic therapies. This work aims to establish a consensus on ILD assessment, patient selection, risk/benefit analysis, and clinical management for those eligible for thoracic RT. METHODS: Using the Estimate-Talk-Estimate method, a board

Practical MRI interpretation of intracranial metastases before and after multidisciplinary management: a pictorial review.

Intracranial metastases represent the most common form of brain tumors in adults. Their diagnosis and post-treatment evaluation are routine tasks in neuroradiology. In this setting, radiologists collaborate with neuro-oncologists, neurosurgeons, radiation oncologists, and other specialists to guide multidisciplinary treatment strategies and assess their response. A well-appointed radiologist should therefore be familiar with current treatment paradigms and their respective manifestations on imaging. This pictorial review begins by reviewing imaging findings of intracranial metastases, emphasiz

Benefit from preoperative radiotherapy in rectal cancer treatment: disease-free patients' and oncologists' preferences.

Preoperative radiotherapy (PRT) in resectable rectal cancer improves local control but increases probability of faecal incontinence and sexual dysfunction. Consensus was reached in 2001 in the Netherlands on a guideline advising PRT to new patients. Purpose was to assess at what benefit oncologists and rectal cancer patients prefer PRT followed by surgery to surgery alone, and how oncologists and patients value various treatment outcomes. Sixty-six disease-free patients and 60 oncologists (surgical, radiation, medical) were interviewed. Minimally desired benefit from PRT (local control) was as
