Bilateral Intra-Articular Radiofrequency Ablation for Cervicogenic Headache.

Introduction. Cervicogenic headache is characterized by unilateral neck or face pain referred from various structures such as the cervical joints and intervertebral disks. A recent study of patients with cervical pain showed significant pain relief after cervical medial branch neurotomy but excluded patients with C1-2 joint pain. It remains unclear whether targeting this joint has potential for symptomatic relief. To address this issue, we present a case report of C1-2 joint ablation with positive outcomes. Case Presentation. A 27-year-old female presented with worsening cervicogenic headache.

Targeted Intraspinal Radiofrequency Ablation for Lumbar Spinal Stenosis.

INTRODUCTION: By using a combination of magnetic resonance imaging (MRI) and computed tomography (CT) of the lumbar spine, it is possible to distinguish between spinal stenosis caused by bone compression and specific soft tissue epidural intraspinal lesions that cause localized spinal canal stenosis and neural compression. Examples include facet cysts and yellow ligament hypertrophy. Many of these patients are elderly with medical comorbidities that make open surgery problematic. MATERIALS & METHODS: This is a study of patients with predominantly soft tissue stenosis being treated with target

Case 222: Pericardioesophageal Fistula after Cardiac Radiofrequency Ablation.

History A 56-year-old woman presented to the emergency department after a cross-country flight. While in flight, she experienced sudden onset of epigastric and midsternal chest pain with shortness of breath. Her symptoms improved markedly after she received oxygen on board, and she finished the flight without further incident. Once home later that night, the pain recurred, and she was taken to the hospital. Her history included chronic pain syndrome, hypertension, and refractory paroxysmal atrial fibrillation, for which she had undergone radiofrequency ablation on two occasions. Her most recen

Bronchopleural fistula after radiofrequency ablation of lung tumours.

The present article describes two cases of bronchopleural fistula (BPF) occurring after radiofrequency ablation of lung tumors. Both procedures were carried out using expandable multitined electrodes, with no coagulation of the needle track. After both ablations, ground-glass opacities encompassed the nodules and abutted the visceral pleura. The first patient had a delayed pneumothorax, and the second had a recurrent pneumothorax. Both cases of BPF were diagnosed on follow-up computed tomography chest scans (i.e., visibility of a distinct channel between the lung or a peripheral bronchus and t

Intrahepatic radiofrequency ablation versus electrochemical treatment in vivo.

BACKGROUND: Radiofrequency ablation (RFA) and electrochemical treatment (ECT) are two methods of local liver tumour ablation. The objective of this study was to compare these methods when applied in proximity to vessels in vivo. METHODS: In a total of ten laparotomised pigs, we used ECT (Group A, four animals) and RFA (Group B, four animals) to create four areas of ablation per animal under ultrasound guidance within 10 mm of a vessel. Group C consisted of two control animals. Chemical laboratory tests were performed immediately before and after each procedure and on days 1, 3 and 7 after sur

Myoglobinuria after laparoscopic radiofrequency ablation of liver tumors.

BACKGROUND: There are scant data in the literature about myoglobinuria after radiofrequency ablation (RFA) of liver tumors. The aim of this study is to analyze the incidence and identify the risk factors involved in this complication after RFA. PATIENTS AND METHODS: An initial case of myoglobinuria and acute kidney injury (AKI) during laparoscopic liver RFA after 10 years of the liver ablation program led to the design of this study. Prospective data were collected on 41 consecutive patients undergoing laparoscopic RFA at our institution over a 9-month period. Urine myoglobin, serum creatinin

Progression of renal tumors after laparoscopic radiofrequency ablation.

OBJECTIVES: To describe 2 patients with tumor progression after treatment with radiofrequency ablation (RFA). RFA is a minimally invasive, Food and Drug Administration-approved technique used to treat solid organ tumors. However, few studies of RFA treatment failure have been reported. METHODS: As a part of an institutional review board-approved protocol, nine renal tumors were ablated with RFA during a 2-year period. Patients enrolled on the protocol were assigned to one of two arms--ablate and resect and ablate and follow-up--depending on the patient's health, tumor characteristics, and tum

[Typical atrial flutter: history, mechanisms and radiofrequency "ablation"].

yipical atrial flutter can now be permanently cured by a single session of radiofrequency ablation, a non pharmacological technique. The term "atrial flutter" is in fact somewhat confusing. A review of the history of this form of tachycardia shows that atrial flutter is indeed a multiple entity. While the reentrant nature of atrial flutter has long been known, most cardiologists refer to the typical ECG aspect and right atrial macro reentry circuit with counterclockwise rotation, as described by Puech. It is now possible to classify these flutters according to their electrocardiographic aspect

Outcomes after radiofrequency catheter ablation of atrial tachycardia.

The purpose of this study was to evaluate the efficacy, safety, and clinical benefit of radiofrequency catheter ablation (RFCA) in a large series of patients with atrial tachycardia (AT). The determinants of success or failure of RFCA in AT remain unclear. We evaluated the results of radiofrequency ablation in 73 women and 32 men (mean age 48 +/- 19 years) with AT. Mapping techniques were based on identification of the earliest endocardial atrial electrogram recorded during AT. AT originated from the right atrium in 91 patients and from the left atrium in 14. The cardiac ventricles were dilate

Ventricular tachycardia induced cardiomyopathy: improvement with radiofrequency ablation.

Cardiomyopathy and congestive heart failure due to ventricular and supraventricular tachycardia is a well-recognized phenomenon. The mechanism of such cardiomyopathy is not clear. Ablation of the supraventricular tachycardia focus, either surgically or electrically, has been reported to result in the resolution of the left ventricular dysfunction. Similarly, the treatment of ventricular tachycardia with surgery or antitachycardia pacing has also been reported to result in improved ventricular systolic function. We present here a case of improvement in left ventricular systolic function after s

Radiation doses during pediatric radiofrequency catheter ablation procedures.

Because RF catheter ablation procedures may be lengthy and are commonly performed in young patients, concern has arisen about radiation dose in this group of patients. This article investigates radiation doses in pediatric patients undergoing RF catheter ablation. Standard fluoroscopic equipment used for diagnostic electrophysiological catheterization studies is technologically capable of dose rates as high as 90 milligray (mGy) per minute to skin and adjacent lung and 260 mGy/min to vertebral bone. Dose rates of this magnitude when used for extended periods of time have been known to cause er

Radiofrequency catheter ablation of septal accessory atrioventricular pathways.

OBJECTIVE: Septal accessory atrioventricular pathways are recognised as being more difficult to ablate than pathways in other locations. This paper describes an experience of 48 consecutive patients with septal accessory pathways who had catheter ablation with radiofrequency current. PATIENTS AND METHODS: There were 28 male and 20 female patients, mean (SD) age 35 (17). 43 patients had a single accessory pathway and 5 patients had multiple accessory pathways. Pre-excitation was present in 37 patients, and 11 patients had concealed accessory pathways. 21 patients had had a previous electrophys

Radiofrequency catheter ablation for supraventricular tachycardias: Part I.

Ablative therapy in the treatment of arrhythmias is an exciting development of the last decade. Although direct-current, high-energy shock can be effective treatment for many supraventricular tachycardias, clinicians are concerned about potential problems associated with its use. Drug therapy for arrhythmias has been inadequate, ineffective, poorly tolerated and fraught with toxic or proarrhythmic reactions. Because of its relative technical ease, high success rate and low complication rate, RFCA remains the therapeutic procedure of choice for patients with atrioventricular nodal reentrant sup
