Fasciola hepatica infection in a 65-year-old woman.

Fascioliasis is an infectious disease caused by fasciola or liver fluke. Humans are accidental hosts to these flatworms. The World Health Organisation considers fascioliasis an important human parasitic disease. In Europe, Australia and Northern America, the disease is rare, but should have a high index of suspicion in patients who have lived in or travelled to endemic areas. Although it can be self-limiting, fascioliasis is associated with an increased risk of bile duct cancer. Before a clear-cut diagnosis is made using ELISA-based arrays, radiologic studies can provide the clinician with a n

Nonoperating Room Anesthesia and Procedural Sedation in Interventional Radiology: A Collaborative Framework for Safe Growth.

Interventional radiology (IR) has become one of the fastest expanding areas of procedural care, with increasing demand for image-guided diagnostic, vascular, oncologic, and emergency interventions. This growth has created parallel demands for both procedural sedation (PS) and anesthesiology services, the so-called nonoperating room anesthesia (NORA) within the IR suite. These services should not be viewed as competing models, but as different points on a continuum in which the level of sedation or anesthesia is matched to the patient, procedure, location, level of care, clinicians' qualificati

Code Conversion Impact Factor and Cash Flow Impact of International Classification of Diseases, 10th Revision, on a Large Multihospital Radiology Practice.

PURPOSE: The 2015 conversion of the International Classification of Diseases (ICD) system from the ninth revision (ICD-9) to the 10th revision (ICD-10) was widely projected to adversely impact physician practices. We aimed to assess code conversion impact factor (CCIF) projections and revenue delay impact to help radiology groups better prepare for eventual conversion to ICD, 11th revision (ICD-11). METHODS: Studying 673,600 claims for 179 radiologists for the first year after ICD-10's implementation, we identified primary ICD-10 codes for the top 90th percentile of all examinations for the e

Radiologic and Functional Outcomes in Unstable Thoracolumbar Fractures Treated With Short-segment Pedicle Instrumentation.

STUDY DESIGN: A retrospective review of radiographs, clinical charts, and health questionnaires of 40 patients. OBJECTIVE: To evaluate the radiologic and functional results of unstable thoracolumbar fractures treated with short-segment pedicle instrumentation (SSPI). A SUMMARY OF BACKGROUND DATA: Although earlier publications report a risk of correction loss or material failure after short-segment fixation in unstable thoracolumbar fractures, more current data suggest that improvements in this technique could offer good clinical and radiologic results. MATERIALS AND METHODS: We undertook a

Patient-safe radiological language: Reducing nocebo harm in orthopedic imaging reports.

As patients increasingly access radiology reports through electronic portals, imaging reports are no longer private technical communications between clinicians. They have become patient-facing texts that can shape how people understand pain, ageing, bodily damage, activity, and treatment choices. In orthopedics, terms such as degeneration, tear, collapse, compression, severe narrowing, and metaphors such as bone-on-bone may be technically familiar to clinicians but frightening or misleading to patients. This discussion paper argues that orthopedic imaging language should be considered part of

Development and Implementation of an Ambulatory Orders-Based Ophthalmology Imaging Workflow.

BACKGROUND: Ocular imaging is essential to the diagnosis and management of eye disease, yet standardized imaging workflows remain underdeveloped in the eye care setting. This manuscript describes the design and implementation of an orders-based imaging workflow for ambulatory ophthalmology integrated with the electronic health record and enterprise imaging systems. METHODS: We developed a DICOM-compliant workflow for pediatric ophthalmology imaging that supports HL7 integration, DICOM modality worklists, and enterprise archive storage. Workflow steps were automated where possible, including i

Shaping Vulnerable Bodies at the Thin Boundary between Environment and Organism: Skin, DNA Repair, and a Genealogy of DNA Care Strategies.

This paper brings together the history of risk and the history of DNA repair, a biological phenomenon that emerged as a research field in between molecular biology, genetics, and radiation research in the 1960s. The case of xeroderma pigmentosum (XP), an inherited hypersensitivity to UV light and, hence, a disposition to skin cancer will be the starting point to argue that, in the 1970s and 1980s, DNA repair became entangled in the creation of new models of the human body at risk - what is here conceptually referred to as the vulnerability aspect of body history - and new attempts at cancer pr

The American Board of Radiology Perspective on Maintenance of Certification: Part IV: Practice quality improvement in radiologic physics.

Recent initiatives of the American Board of Medical Specialties (ABMS) in the area of maintenance of certification (MOC) have been reflective of the response of the medical community to address public concerns regarding quality of care, medical error reduction, and patient safety. In March 2000, the 24 member boards of the ABMS representing all medical subspecialties in the USA agreed to initiate specialty-specific maintenance of certification (MOC) programs. The American Board of Radiology (ABR) MOC program for diagnostic radiology, radiation oncology, and radiologic physics has been develope

The ACR Managed Care Committee: focusing private payers on radiology issues.

The ACR Managed Care/Private Payer Relations Committee is an important committee of the ACR Commission on Economics. This report reviews the committee's mission, structure, and processes and some of its current recommendations to ACR membership. The development of and participation in radiology advisory committees is a vital strategy in this process. Separating professional and technical charges, rather than submitting global charges, will help preserve radiology's professional integrity in the future. The Imaging Provider Report Card (IPRC) will allow radiology to define practice quality and

On-line delivery of continuing medical education over the World-Wide Web: an on-line needs assessment.

OBJECTIVE: On-line continuing medical education (CME) courses offer at least two potential advantages: They are extremely convenient and relatively inexpensive. Before expanding our department's 2-year-old on-line category 1 accredited CME program, we conducted a survey to assess the need for more courses of this type and to document those topics of most interest to our responders. MATERIALS AND METHODS: An on-line survey form was designed and linked to the home page of our departmental Web server. The 8-month survey asked whether responders were interested in earning category 1 CME hours, ho

Ultrasonography and developmental dysplasia of the hip: a cost-benefit analysis of three delivery systems.

The role of ultrasound (US) in the diagnosis and management of infants with developmental dysplasia of the hips (DDH) is becoming widely accepted. In our community, there exist three delivery systems for US-DDH: the radiology based, the combined radiology/orthopaedic based, and the orthopaedic office based. This study reviewed the costs and benefits of each delivery system and found that once expertise had been gained and start-up costs were met, the orthopaedic office-based system was the most convenient, efficient, and cost effective for the patient/family and treating physicians. This mirro

Non-invasive diagnosis of advanced fibrosis and cirrhosis.

Liver cirrhosis is a common and growing public health problem globally. The diagnosis of cirrhosis portends an increased risk of morbidity and mortality. Liver biopsy is considered the gold standard for diagnosis of cirrhosis and staging of fibrosis. However, despite its universal use, liver biopsy is an invasive and inaccurate gold standard with numerous drawbacks. In order to overcome the limitations of liver biopsy, a number of non-invasive techniques have been investigated for the assessment of cirrhosis. This review will focus on currently available non-invasive markers of cirrhosis. The

Feasibility of using teleradiology to improve tuberculosis screening and case management in a district hospital in Malawi.

PROBLEM: Malawi has one of the world's highest rates of human immunodeficiency virus (HIV) infection (10.6%), and southern Malawi, where Thyolo district is located, bears the highest burden in the country (14.5%). Tuberculosis, common among HIV-infected people, requires radiologic diagnosis, yet Malawi has no radiologists in public service. This hinders rapid and accurate diagnosis and increases morbidity and mortality. APPROACH: Médecins Sans Frontières, in collaboration with Malawi's Ministry of Health, implemented teleradiology in Thyolo district to assist clinical staff in radiologic imag

T-cell and NK-cell lymphomas in the lung.

While the lung is frequently involved by systemic lymphoma, primary pulmonary lymphoma accounts for less than 1% of all extranodal ymphomas. In particular, T-cell lymphoma is very rare in the lung, as a primary or secondary lesion. Patients with pulmonary T-cell lymphoma usually present with cough, dyspnea, pain, fever, recurrent infections, and hemoptysis. Typical radiologic features include pulmonary nodules, consolidation, solid pulmonary opacities, cystic changes, hilar adenopathy, and pleural effusions. Patients with these clinical and radiologic findings are frequently presumed to have p
