Evaluating CBCT-Guided Adaptive Radiotherapy for Pancreatic Cancer Using Synthetic CBCT Data.

Ethos adaptive radiotherapy is employed frequently in the pelvis to improve treatment accuracy by adapting to daily anatomical changes. The use of this CBCT-guided platform for abdominal treatments is made challenging by motion-related image artifacts that are detrimental to the Ethos auto-contouring process. We present a preliminary in silico study enabled by synthetic CBCT data of Ethos adaptive radiotherapy for pancreatic cancer. Simulation CT and daily CBCT images were collected from nonadaptive patients treated on Ethos. Contoured CBCTs drove structure-guided deformable registration from

Decision-making processes in image guided radiotherapy: A think aloud study.

INTRODUCTION: 3D Image Guided Radiotherapy (IGRT) using Cone Beam Computer Tomography (CBCT) has been implemented for a range of treatment sites across the UK in the last decade. A paucity of evidence exists to understand how radiation therapists (RTTs) make clinical decisions during image interpretation as part of the IGRT process. The aim of this study was to investigate the decision-making processes used by RTTs during image interpretation of IGRT. METHOD: Case study methodology was adopted utilising a think aloud observational method with follow-up interviews. 12 RTTs were observed and in

Technical note: Dose voxel resolution determination for Monte Carlo electron beam simulation in thin targets.

BACKGROUND: Monte Carlo particle simulation has become the primary tool for designing low-energy miniature x-ray tubes due to the difficulties of physically prototyping these devices and characterizing their radiation fields. Accurate simulation of electronic interactions within their targets is necessary for modeling both photon production and heat transfer. Voxel-averaging can conceal hot spots in the target heat deposition profile that can threaten the integrity of the tube. PURPOSE: This research seeks a computationally-efficient method of estimating voxel-averaging error in energy deposi

A small footprint couch-top support device for image-guided radiotherapy of heavy patients.

PURPOSE: Patients with body weights close to or above 400 lbs present unique challenges in radiation therapy since the weight limit of most treatment couches decreases as the couch-top extends toward the treatment gantry. The purpose of this work was to develop a small footprint couch-top support platform to safely perform image-guided radiotherapy (IGRT) for extremely heavy patients. METHODS: One way to protect the couch-top from damage and prevent a catastrophic breakdown is to provide additional support as the couch extends toward the treatment gantry. To allow a maximal range of gantry mo

Development of dedicated phantom for end-to-end testing of multiple image-guidance configurations incorporating light-section-based surface guidance system.

PURPOSE: Radiotherapy devices with multiple image-guidance systems, such as surface-guided radiation therapy (SGRT), have been widely used in recent years. However, in the case of SGRT devices using the light-section method, coordinate coincidence evaluation using a phantom for SGRT devices with patterned light projection is not appropriate. Hence, this study aims to develop a dedicated phantom able to evaluate both the detection accuracy and coordinate coincidence of multiple IGRT configurations, including light-section-based SGRT devices. MATERIALS AND METHOD: First, we developed an end-to-

Late Sequelae of Radiotherapy—The Effect of Technical and Conceptual Innovations in Radiation Oncology.

BACKGROUND: Approximately half of all patients with tumors need radiotherapy. Long-term survivors may suffer from late sequelae of the treatment. The existing radiotherapeutic techniques are being refined so that radiation can be applied more precisely, with the goal of limiting the radiation exposure of normal tissue and reducing late sequelae. METHODS: This review is based on the findings of a selective search in PubMed for publications on late sequelae of conventional percutaneous radiotherapy, January 2000 to May 2020. Late sequelae affecting the central nervous system, lungs, and heart a

Task-based image quality and dosimetric validation of HyperSight CBCT for adaptive radiotherapy.

BACKGROUND: Cone-beam computed tomography (CBCT) is increasingly used in adaptive radiotherapy (ART), yet the dosimetric accuracy and image quality of advanced CBCT imaging platforms-such as Halcyon HyperSight 4.0 (HH), which combines an upgraded detector panel with advanced iterative reconstruction, scatter-correction, and metal-artifact-reduction algorithms-remain insufficiently characterized clinically. PURPOSE: To evaluate HH CBCT reconstruction algorithms for ART by assessing image quality, dose accuracy, and in vivo verification using optically stimulated luminescence dosimeters (OSLDs)

Definitive treatment to pyriform sinus with minimum cord exposure--a case report.

In every Radiation Oncology Department there is a group of patients referred for curative radiotherapy and within this group of patients there is a subset of patients who present the most difficult of treatment scenarios, maximum tumor dose and minimum spinal cord dose. The case to be presented involved the use of a head and neck five field technique of which three fields were treated isocentrically with the remaining two fields being electron boosts. It is from these patients that we extend to the maximum our planning capabilities.

Development of a phantom for assessing the precision of setup in skin mark-less surface-guided radiotherapy.

BACKGROUND: Surface-guided radiotherapy (SGRT) is adopted by several institutions; however, reports on the phantoms used to assess the precision of the SGRT setup are limited. PURPOSE: The purpose of this study was to develop a phantom to verify the accuracy of the irradiation position during skin mark-less SGRT. METHODS: An acrylonitrile butadiene styrene (ABS) plastic cube phantom with a diameter of 150 mm on each side containing a dummy target of 15 mm and two types of body surface-shaped phantoms (breast/face shape) that could be attached to the cube phantom were fabricated. [Field-integrated dose modification. 1: Clinical significance of the procedure].

A method is derived from a previously published compensator-integrated modification proceeding which maintains the principle of a field-integrated dose modification without needing CT input data. Three typical clinical examples are presented in order to describe this considerably simplified technique thus suited for everyday practice. The authors discuss the interesting radiobiological aspects involved.

Reirradiation Collaborative Group (ReCOG) consensus on standards for dose evaluation and reporting in patients with multiple courses of radiation therapy: an AAPM/ACRO/ASTRO/CARO/COMP/CADRA/CPQR/ESTRO/NRG-endorsed consensus statement.

As cancer survivors live longer, technologies improve, and reirradiation (reRT) becomes more common, standardised methods for the assessment and reporting of cumulative radiation doses are needed to allow treatment optimisation and integration with other medical specialties managing these complex patients. This consensus statement, developed by an international collaboration of radiation oncologists, physicists, and other experts in the Reirradiation Collaborative Group, proposes a framework for consistent evaluation, documentation, reporting, and clinical decision making in reRT. An optimized knowledge-based planning method for craniospinal irradiation integrated with auto-contouring.

BACKGROUND: Craniospinal irradiation (CSI) is a complex treatment requiring precise delineation of extended plan target volumes (PTV) and multiple organs at risk (OARs). The process traditionally demands substantial time and expertise from physicians and medical physicists. While artificial intelligence (AI) has shown promise in auto-contouring and auto-planning in other tumors, its application to CSI remains limited due to challenges in model generalizability and outlier sensitivity. METHODS: We developed an automated workflow integrating deep learning-based auto-contouring and machine learn

Dose optimization with fully flexible vertex positioning for LATTICE radiotherapy.

BACKGROUND: Lattice radiotherapy (LATTICE) is a technique of spatially fractionated radiation therapy (SFRT) that delivers high radiation doses to specific regions (vertices) within a large tumor, forming a spatially modulated "lattice" pattern, while surrounding areas receive lower doses to minimize damage to healthy tissues. Although the original conception of LATTICE did not prescribe any rigorous symmetry, and early clinical implementations relied on manual vertex placement tailored to tumor shape and anatomical constraints, more recent automated approaches have introduced regular patterns

Minibeam-pLATTICE: A novel proton LATTICE modality using minibeams.

BACKGROUND: LATTICE, a form of spatially fractionated radiation therapy (SFRT) that delivers high-dose peaks and low-dose valleys within the target volume, has been clinically utilized for treating bulky tumors. However, its application to small-to-medium-sized target volumes remains challenging due to beam size limitations. PURPOSE: To address the challenge of applying LATTICE radiation therapy to small-to-medium-sized targets, this work proposes a novel proton LATTICE (pLATTICE) modality using minibeams, namely minibeam-pLATTICE, that can extend the LATTICE approach for small-to-medium targ

Nibbling toward optimal sphere packing for lattice spatially fractionated radiotherapy of large and bulky unresectable tumors.

BACKGROUND: The Spatially Fractionated Radiotherapy (SFRT) treatment technique is characterized by its highly heterogeneous dose distribution, with an alternating pattern of high- and low-dose areas inside large tumors. For lattice SFRT, a technique that utilizes high-dose spheres, a common obstacle is maximizing the number of high-dose spheres within the tumor while maintaining spacing rules to preserve the low-dose regions. PURPOSE: We propose a novel algorithm that uses the Rödl nibble methodology to optimize the number of lattice vertices placement for SFRT planning. By increasing the num
