Six-canal video head impulse test in patients with labyrinthine and retrolabyrinthine pathology: detecting vestibulo-ocular reflex deficits.

BACKGROUND: Abnormal gains in six-canal video head impulse test are attributed to semi-circular canal deficits. However, as video head impulse test responses are linked to the vestibulo-ocular reflex, it was hypothesised that abnormal gains can be caused by vestibulo-ocular reflex pathway deficits. METHODS: This study compared video head impulse test gains in 20 patients with superior semi-circular canal dehiscence (labyrinthine cause) and 20 side- and gender-matched patients with vestibular schwannomas (retrolabyrinthine cause), and investigated correlations between them (Mann-Kendall trend

Vestibulo-ocular reflex dynamics with head-impulses discriminates spinocerebellar ataxias types 1, 2 and 3 and Friedreich ataxia.

OBJECTIVE: Although the diagnosis of inherited ataxias is ultimately genetic, this usually means an extensive and expensive process. This justifies the search for distinct clinical signs that may potentially help orient molecular diagnosis. METHODS: We explored the vestibulo-ocular reflex (VOR) with the video Head Impulse Test in patients diagnosed with spinocerebellar ataxia (SCA) type 3 (n = 15), type 1 (n = 4) and type 2 (n = 4), Friedreich's ataxia (FA) (n = 9) and healthy controls (n = 40). We estimated the latency, regression (VORr) and instantaneous VOR gain at 40, 60 and 80 ms (VOR40,

Adaptation of the vestibulo-ocular reflex, subjective tilt, and motion sickness to head movements during short-radius centrifugation.

Head movements made while the whole body is rotating at unusually high angular velocities (here with supine body position about an earth-vertical axis) result in inappropriate eye movements, sensory illusions, disorientation, and frequently motion sickness. We investigated the acquisition and retention of sensory adaptation to cross-coupled components of the vestibulo-ocular reflex (VOR) by asking eight subjects to make headturns while being rotated at 23 rpm on two consecutive days, and again a week later. The dependent measures were inappropriate vertical VOR, subjective tilt, and motion sic

Modelling non-linearities in the vestibulo-ocular reflex (VOR) after unilateral or bilateral loss of peripheral vestibular function.

We recorded the vestibulo-ocular reflex (VOR) in 18 normal subjects, 50 patients with unilateral loss of vestibular function and 18 patients with bilateral loss of vestibular function. The unilateral cases had either partial loss (i.e. vestibular neuronitis or Meniere's disease) or total loss (i.e. vestibular nerve section), whereas bilateral cases had only partial loss (i.e. due to ototoxicity or to suspected microangiopathy, secondary to severe kidney disease). Tests were performed at 1/6-Hz passive head rotation in the dark, with peak head velocities ranging from 125 to 190 degrees/s. We re

Age-related decline in vestibulo-ocular reflex gain is associated with an increased sway response to electrical vestibular stimulation.

Despite considerable loss of peripheral vestibular input with age, there is relatively little change in function. This has led to speculation that increasing central neural gain mitigates such losses. Here, we test this hypothesis by comparing the gain of the vestibulo-ocular reflex (VOR) with the response to electrical vestibular stimulation (EVS) across the lifespan. The VOR assesses the full vestibulo-motor pathway, whereas EVS bypasses the peripheral vestibular mechanics. Hence, any dissociation between the two may reveal a difference in central versus peripheral function. A rotating chair

Horizontal Vestibulo-Ocular Reflex Deficit as a Biomarker for Clinical Disease Onset, Severity, and Progression of Machado-Joseph Disease.

Because of the crucial importance of finding a useful biomarker for further clinical trials in Machado-Joseph disease (MJD), and based on our previous studies, we aimed to evaluate whether the horizontal vestibulo-ocular reflex (VOR) gain could be a reliable neurophysiological biomarker for the clinical onset, severity, and progression of the disease. Thirty-five MJD patients, 11 pre-symptomatic genetically confirmed MJD subjects, and 20 healthy controls underwent a detailed epidemiological and clinical neurological examination including the Scale for the Assessment and Rating of Ataxia (SARA)

Visual suppression of vestibulo-ocular reflex in patients treated with carotid artery revascularization: A potential biomarker for cerebral perfusion.

BACKGROUND: Carotid endarterectomy (CEA) and carotid artery stenting (CAS) are common treatments for ischemic stroke prevention in patients with carotid artery stenosis. However, the beneficial effects of CEA/CAS for cerebral hypoperfusion due to carotid artery stenosis have yet to be fully established. As dizziness is a common symptom in patients with carotid artery stenosis, we aimed to evaluate the effects of CEA/CAS on cerebral function in patients with carotid artery stenosis, using equilibrium function tests. METHODS: This prospective observational cohort study included 50 patients who

Variations in gravitoinertial force level affect the gain of the vestibulo-ocular reflex: implications for the etiology of space motion sickness.

Recordings of horizontal nystagmus were obtained on 16 male subjects exposed to repeated patterns of horizontal angular acceleration, constant velocity rotation, and sudden-stop deceleration in the laboratory and in the free-fall and high-force periods of parabolic flight. Nystagmus intensity was a clear function of gravitoinertial force level: slow phase velocity and beat frequency increased during exposure to high force levels and decreased in free-fall compared to values obtained at I G. These findings indicate that the gain of the vestibulo-ocular reflex decreases in free-fall. This fact l

Assessment of the Vestibulo-Ocular reflex in polycystic Ovary Syndrome: a Case-Control study using video head impulse testing.

BACKGROUND: Polycystic ovary syndrome (PCOS) is a common endocrine disorder that may affect multiple organ systems. While cochlear and otolith involvement has been investigated, the impact on semicircular canal-mediated vestibulo-ocular reflex (VOR) function remains unclear. AIMS/OBJECTIVES: To evaluate canal-specific, high-frequency VOR function in women with PCOS using the video head impulse test (vHIT) and compare findings with healthy controls. MATERIAL AND METHODS: This single-center case-control study included 42 women with PCOS (Rotterdam criteria) and 35 age-matched controls. vHIT wa

Mathematical Methods for Measuring the Visually Enhanced Vestibulo-Ocular Reflex and Preliminary Results from Healthy Subjects and Patient Groups.

BACKGROUND: Visually enhanced vestibulo-ocular reflex (VVOR) is a well-known bedside clinical test to evaluate visuo-vestibular interaction, with clinical applications in patients with neurological and vestibular dysfunctions. Owing to recently developed diagnostic technologies, the possibility to perform an easy and objective measurement of the VVOR has increased, but there is a lack of computational methods designed to obtain an objective VVOR measurement. OBJECTIVES: To develop a method for the assessment of the VVOR to obtain a gain value that compares head and eye velocities and to test

Effects of Long-Term Contact Lens Use on Vestibulo-Ocular Reflex Using Rotary Chair and Video Head Impulse Testing.

PURPOSE: This study investigated whether long-term contact lens wear influences vestibulo-ocular reflex (VOR) gain and suppression across a range of stimulus frequencies. Despite frequent use of contact lenses in clinical populations, it remains unclear how optical correction affects VOR assessment outcomes. METHOD: Twelve myopic adults (four men, eight women; 21-25 years) who wore contact lenses at least 12 hr per day, 5 days per week, participated. VOR gain and suppression were measured using sinusoidal harmonic acceleration (SHA) rotary chair testing at 0.04-0.64 Hz, and VOR gain at high f

Assessment of vestibulo-ocular reflex and its adaptation during stop-and-go car rides in motion sickness susceptible passengers.

Motion sickness is a physiological condition that negatively impacts a person's comfort and will be an emerging condition in autonomous vehicles without proper countermeasures. The vestibular system plays a key role in the origin of motion sickness. Understanding the susceptibility and (mal) adaptive mechanisms of the highly integrated vestibular system is a prerequisite for the development of countermeasures. We hypothesize a differential association between motion sickness and vestibular function in healthy individuals with and without susceptibility for motion sickness. We quantified vestib

Impaired Vestibulo-Ocular Reflex on Video Head Impulse Test in Superior Canal Dehiscence: "Spontaneous Plugging" or Endolymphatic Flow Dissipation?

Surgical plugging of the superior semicircular canal (SSC) represents an effective procedure to treat disabling symptoms in superior canal dehiscence (SCD), despite resulting in an impaired vestibulo-ocular reflex (VOR) gain for the SSC. On the other hand, SSC hypofunction on video head impulse test (vHIT) represents a common finding in patients with SCD exhibiting sound/pressure-induced vertigo, a low-frequency air-bone gap (ABG), and enhanced vestibular-evoked myogenic potentials (VEMPs). "Spontaneous canal plugging" has been assumed as the underlying process. Nevertheless, missing/mitigated

Recovery of Vestibulo-Ocular Reflex Symmetry After an Acute Unilateral Peripheral Vestibular Deficit: Time Course and Correlation With Canal Paresis.

BACKGROUND: We investigated how response asymmetries and deficit side response amplitudes for head accelerations used clinically to test the vestibular ocular reflex (VOR) are correlated with caloric canal paresis (CP) values. METHODS: 30 patients were examined at onset of an acute unilateral peripheral vestibular deficit (aUPVD) and 3, 6, and 13 weeks later with three different VOR tests: caloric, rotating chair (ROT), and video head impulse tests (vHIT). Response changes over time were fitted with an exponential decay model and compared with using linear regression analysis. RESULTS: Recov
