Managing residual refractive error after cataract surgery.

We present a review of keratorefractive and intraocular approaches to managing residual astigmatic and spherical refractive error after cataract surgery, including laser in situ keratomileusis (LASIK), photorefractive keratectomy (PRK), arcuate keratotomy, intraocular lens (IOL) exchange, piggyback IOLs, and light-adjustable IOLs. Currently available literature suggests that laser vision correction, whether LASIK or PRK, yields more effective and predictable outcomes than intraocular surgery. Piggyback IOLs with a rounded-edge profile implanted in the sulcus may be superior to IOL exchange, bu

Management of residual refractive error after cataract surgery.

PURPOSE OF REVIEW: To provide a review of the recent literature on the management of residual refractive error after cataract surgery. RECENT FINDINGS: Laser in-situ keratomileusis (LASIK) is the most accurate procedure to correct residual refractive error after cataract surgery. Lens-based procedures, such as intraocular lens (IOL) exchange or piggyback lens implantation, are also possible alternatives in cases with extreme ametropia, corneal abnormalities, or in situations where excimer laser is not available. In this review, we found that Piggyback IOL were safer and more accurate than IOL

An investigation into the validity of self-reported classification of refractive error.

PURPOSE: To assess the validity of questionnaire use in the self-identification of refractive status. METHODS: Two hundred and forty adults (21-60 years of age) presenting for a routine eye examination at various optometric practices in Northern Ireland were asked to complete one of two questionnaires. Both questionnaires used identical questions to ascertain age, gender, current spectacle use, age of first spectacle use and level of education. For the identification of refractive status, Questionnaire 1 used layman's terminology whilst Questionnaire 2 combined optometric terminology with des

Nature and nurture: the complex genetics of myopia and refractive error.

The refractive errors, myopia and hyperopia, are optical defects of the visual system that can cause blurred vision. Uncorrected refractive errors are the most common causes of visual impairment worldwide. It is estimated that 2.5 billion people will be affected by myopia alone within the next decade. Experimental, epidemiological and clinical research has shown that refractive development is influenced by both environmental and genetic factors. Animal models have showed that eye growth and refractive maturation during infancy are tightly regulated by visually guided mechanisms. Observational

Longitudinal changes in refractive error of children with infantile esotropia.

AIM: the aim of this study was to quantify changes in refractive status over time in children with infantile esotropia (ET) and to analyse a number of clinical factors associated with infantile ET to determine how they may affect emmetropisation. METHODS: longitudinal cycloplegic refraction data were collected for 5-12 years from 143 consecutive children enroled in a prospective study of infantile ET by 6 months of age. Changes in refractive error with age were summarised with descriptive statistics and the influence of amblyopia, undercorrection of hypermetropia, accommodation, and binocular

Standardized patient methodology to assess refractive error reproducibility.

PURPOSE: Standardized patient (SP) methodology is the gold standard for evaluating clinical practice. This approach was used to investigate the content of typical optometric eyecare in England and the reproducibility of refractive error measurement using prescriptions obtained by three SPs. METHODS: The three SPs were independently examined by three to four expert optometric clinicians to obtain "benchmark" estimates of refractive error. One hundred two community optometrists consented to be visited by three SPs who were trained to provide accurate responses during the examinations. The spect

Correlation of aphakic retinal detachment and refractive error with gender.

I analyzed the aphakic refractive errors of men and women separately in a statistical study of patients with aphakic retinal detachments. I compared the distribution of aphakic refractive errors in a population of 81 adult patients with nontraumatic aphakic retinal detachment with that of a randomly selected control population of 93 adult patients with aphakia. As a group, women who developed aphakic retinal detachments were significantly more myopic than female controls (11.41 vs 12.37 diopters of spherical equivalent; P = .004). In contrast, the refractive error in men did not differ between
