An Ethical Analysis of a Healthcare Scandal: Lessons from the "Newborn Gang" Case.

The "Newborn Gang" scandal, which emerged in Türkiye in 2024, exposed allegations of organized fraud, manipulation of neonatal intensive care referrals, financial exploitation of the healthcare reimbursement system, and preventable infant deaths. While previous publications have contributed important perspectives on the case, including its political, economic, and ethical dimensions, there remains a need for a more comprehensive ethical analysis integrating clinical, organizational, and health system perspectives. This study reconstructs the chronology of the case using publicly available docu

Service users' perspectives on primary eye care services following performance-based financing in Rwanda: an exploratory qualitative study.

BACKGROUND: Visual impairment is a major global public health concern, disproportionately affecting populations in low- and middle-income countries (LMICs), where access to eye care services remains limited. In 2019, the Rwandan Ministry of Health integrated eye care services into the Performance-Based Financing (PBF) framework to improve accessibility, service quality, and utilization. However, service users' perspectives on this initiative remain underexplored. This study aimed to examine service users' perceptions of accessibility, quality, affordability, satisfaction, and challenges in acc

Impact of early interruption from pay-for-performance program on progression and medical utilization for patients with early chronic kidney disease.

BACKGROUND: Most patients with chronic kidney disease (CKD) benefit from early intervention programs designed to slow disease progression and reduce medical usage. The Early-CKD Pay-for-Performance (Early-CKD P4P) program, implemented by Taiwan's National Health Insurance Administration since 2011, has proven effective in this regard. However, there is limited research on outcomes for patients who interrupt their participation in the program. The study aimed to compare risks of emergency department visits, hospitalizations, and dialysis between those who continued versus those who interrupted

Effect of Patient Financial Incentives on Statin Adherence and Lipid Control: A Randomized Clinical Trial.

IMPORTANCE: Financial incentives can improve medication adherence and cardiovascular disease risk, but the optimal design to promote sustained adherence after incentives are discontinued is unknown. OBJECTIVE: To determine whether 6-month interventions involving different financial incentives to encourage statin adherence reduce low-density lipoprotein cholesterol (LDL-C) levels from baseline to 12 months. DESIGN, SETTING, AND PARTICIPANTS: This 4-group, randomized clinical trial was conducted from August 2013 to July 2018 among several large US insurer or employer populations and the Univer

Quality indicators for the diagnosis and management of chronic rhinosinusitis.

BACKGROUND: Chronic rhinosinusitis (CRS) has been identified as a high-priority disease category for quality improvement. To this end, this study aimed to develop CRS-specific quality indicators (QIs) to evaluate diagnosis and management that relieves patient discomfort, improves quality of life, and prevents complications. METHODS: A guideline-based approach, proposed in 2012 by Kötter et al. was used to develop QIs for CRS. Candidate indicators (CIs) were extracted from 3 practice guidelines and 1 international consensus statement on the diagnosis and management of CRS. Guidelines were eval

Does quality of care for hypertension in primary care vary with postcode area deprivation? An observational study.

BACKGROUND: Hypertension is a common major risk factor for stroke and coronary heart disease. Little is known about how achievement of financially incentivised and non-incentivised indicators of quality of care varies with deprivation, or about the effect of financial incentives on health inequalities in hypertension. General practices in the UK have received financial incentives for high quality care since 2004. This study set out to assess the variations in achievement of incentivised and non-incentivised quality indicators for hypertension by patient area deprivation, before and after the i

Introduction of European priority review vouchers to encourage development of new medicines for neglected diseases.

Every year 1 billion people worldwide are affected by traditionally neglected diseases, such as malaria, tuberculosis, leishmaniasis, and lymphatic filariasis, which impose tremendous public health burdens. Governments, foundations, and drug manufacturers have, however, started to support development of new treatments. European Union Member States have been leaders in implementing so-called push mechanisms (payment for drug development) and pull funding (reward for output), such as the advance market commitment, which creates a market for vaccines by guaranteeing prices. We propose an addition

The lack of creative destruction in healthcare systems makes innovation a cost driver.

Creative destruction, a concept developed by the economist Joseph Schumpeter, describes the process by which new innovations displace and replace existing technologies, business models and structures. While this approach has driven significant efficiency gains across various industries, it remains largely absent in healthcare. The disproportionate rise in health expenditure relative to care output points to structural inefficiencies and untapped potential. A cross-sectoral comparison with telecommunications and banking shows that technological progress in those industries generated tremendous

[Clinical gastroenterology--luxury or standard of service in gastroenterology?].

Gastroenterology is one of the important specialities in internal medicine. The reform of the training curriculum for internal medicine and the reimbursement for inpatient and outpatient services in gastroenterology threatens the existence of internal medicine and gastroenterology in Germany, too. The capacity for training in internal medicine and gastroenterology is reduced by a decrease in the number of hospital beds in academic and community training centres. The concentration on gastrointestinal endoscopy in outpatient gastroenterology will be a result of an increasing demand for gastroint

Implications of policy and management decisions on access, quality, and the type of services for the elderly in the United States.

America has no conscious overall health policy. What we have is a system which encourages doctors to perform medical procedures on people, especially in hospitals, and which places little or no emphasis on a person's responsibility for his or her own health. There is no incentive whatsoever for doctors, except in HMOs, to provide the elderly with primary care or consultations on preventative health care. there is no reimbursement under Medicare for this type of service, and a financial disincentive exists for the doctor in that a healthier patient is less likely to use a physician's services.

Pay for obesity? Pay-for-performance metrics neglect increased complication rates and cost for obese patients.

BACKGROUND: Rates of surgical complications are increasingly being used for pay-for-performance reimbursement structures. We hypothesize that morbid obesity has a significant effect on complication rates and costs following commonly performed general surgical procedures. METHODS: We studied 30,502 patients who underwent cholecystectomy for cholecystitis and 6,390 patients who underwent appendectomy for acute appendicitis using administrative claims data from seven Blue Cross and Blue Shield Plans over a 7-year period (2002-2008). We compared 30-day complications as well as total 30-day direct

Motivation, money and respect: a mixed-method study of Tanzanian non-physician clinicians.

Poor quality of care is a major concern in low-income countries, and is in part attributed to low motivation of healthcare workers. Non-physician clinicians (mid-level cadre healthworkers) are central to healthcare delivery in half of the countries in Africa, but while much is expected from these clinicians, little is known about their expectations and motivation to perform well. Understanding what motivates these healthworkers in their work is essential to provide an empirical base for policy decisions to improve quality of healthcare. In 2006-2007, we conducted a mixed-method study to evalua

Timely hospital discharge in TAVI patients: towards a patient-centered care pathway.

Transcatheter aortic valve implantation (TAVI) has evolved into the dominant therapeutic option for severe aortic stenosis, with procedural refinements and expanding indications increasingly supporting shorter post-procedural monitoring and discharge. International evidence from randomized trials, multi-center registries, and consensus guidelines demonstrates that reduced length of stay (LOS), including next-day or early discharge, can be safely achieved in selected patients when supported by standardized workflows, careful patient selection, and structured outpatient monitoring. In Germany, L

Coverage, pricing, and reimbursement of telemedicine: a qualitative study among implementers of the Akoma Pa mHealth-based telemedicine intervention in Ghana.

BACKGROUND: Healthcare systems are rapidly using digital health technologies, such as mHealth-based telemedicine, as digitalization in healthcare provision advances. However, there is limited knowledge of the pricing and reimbursement mechanisms for these health interventions, especially in the Global South. This study aimed to investigate healthcare professionals' perspectives on the coverage, pricing, and reimbursement of telemedicine services in Ghana. METHODS: This study used a qualitative exploratory design. Multistage sampling was employed. All 46 sampled Christian Health Association of

Impact of a national reimbursement policy on anti-osteoporosis medication prescription at discharge after hip-fracture surgery in Japan: an interrupted time-series analysis.

UNLABELLED: A 2022 pay-for-performance fee in Japan increased the rate of post-hip-fracture osteoporosis treatment (primarily oral agents) by 20.8 percentage points and tripled the annual uptake trend in an interrupted time-series analysis of 78,224 patients. Financial incentives embedded within national fee schedules may help close gaps in secondary fracture prevention. PURPOSE: Although osteoporosis treatment is essential for preventing secondary fractures, many patients remain untreated after hip-fracture surgery. Accordingly, Japan introduced a three-stage reimbursement add-on, the Second

Fast-track extubation after cardiac surgery in infants: Tug-of-war between performance and reimbursement?

OBJECTIVES: To compare the safety and resource-efficacy of the fast-track (FT) concept (extubation ≤8 hours after surgery) versus the conventional approach (non-FT, >8 hours postoperatively) in infants undergoing open-heart surgery. METHODS: Infants <7 kg operated on cardiopulmonary bypass between 2014 and 2018 were analyzed. Propensity score matching (1:1) was performed for group comparison (FT vs non-FT). Intensive care unit (ICU) personnel use and unit performance were evaluated. Postoperative outcome and reimbursement based on German diagnosis-related groups were compared. RESULTS: Of 71

Effect of Primary Care Value-Based Payment Implementation on Low-Value Care Among Medicaid Beneficiaries in North Carolina.

OBJECTIVE: To assess the impact of primary care value-based payment implementation on low-value service use among Medicaid beneficiaries. STUDY SETTING AND DESIGN: On July 1, 2021, North Carolina Medicaid launched the Advanced Medical Home (AMH) program to improve care quality and coordination under managed care. Health plans are now required to include financial incentives for care coordination and performance in contracts with the highest-tier primary care practices (Tier 3 AMHs). Leveraging the tiered structure of the program and using a difference-in-differences design, we compared change
