Psychological Readiness and Return to Sport.

Psychological readiness in return to sport has been recently identified as an important factor in addition to physical function. A number of psychological factors have been identified; however, fear of reinjury (ie, kinesiophobia) has been identified as an important risk factor associated with worse outcomes related to return to sport. Several validated questionnaires and surveys are available to evaluate psychological factors. Return to sport following shoulder instability and anterior cruciate ligament reconstruction is an important area of study. There are many factors, including gender, th

Evaluation and Preparation of the Baseball Player in the Athletic Training Room.

A critical aspect of the care of the baseball player involves understanding injury patterns, workload tolerance, and objective markers that can aid in identifying predisposing factors to injury. After injury, a well-structured rehabilitation program can return many players to the field. The greatest challenge is not only returning them to the previous level of performance but also preventing reinjury and maintaining durability. Potential factors implicated in the most common injuries include deficits in range of motion, balance, core strength, lumbopelvic control, thoracic rotation, posture, a

Introducing Skeletally-Mature Anterior Cruciate Ligament Reconstruction Technique Using Reinforcement (SATURN) With Iliotibial Band Autograft.

Anterior cruciate ligament (ACL) injuries are increasingly common in the adolescent age group. Recent literature has endorsed combining ACL reconstruction with lateral extra-articular procedures to minimize residual rotatory knee instability and reduce the risk of reinjury in this age group. This technique describes a single-bundle combined anatomical ACL reconstruction and lateral extra-articular reinforcement performed with a single iliotibial band autograft. Also this technique allows for the ACL reconstruction and lateral extra-articular stabilization to be performed through a single surgi

The Burden of the "Simple Ankle Sprains": A Review of the Epidemiology and Long-Term Impact.

Lateral ankle sprain (LAS) is not as simple as it was believed to be as it has substantial negative impacts on the active sporting population. The negative impact on physical function, quality of life (QoL) and economic burden is significant with increased risk of reinjury, development of chronic lateral ankle instability and posttraumatic ankle osteoarthritis resulting in functional deficits, decreased QoL and chronic disabilities. Economic burden from a societal perspective demonstrated notably higher indirect costs from productivity loss. Preventative interventions with early surgery for a

Adductor Strains in Athletes.

Acute adductor injuries are a common occurrence in sport. The overall incidence of adductor strains across 25 college sports was 1.29 injuries per 1000 exposures, with men's soccer (3.15) and men's hockey (2.47) having the highest incidences. As with most muscle strains there is a high rate of recurrence for adductor strains; 18% in professional soccer and 24% in professional hockey. Effective treatment, with successful return to play, and avoidance of reinjury, can be achieved with a proper understanding of the anatomy, a thorough clinical exam yielding an accurate diagnosis, and an evidence-

Suture Anchor Repair with V-Y Plasty and Achilles Allograft Augmentation for Chronic Quadriceps Tendon Injury.

Despite an overall incidence that remains low, quadriceps tendon injuries represent a common disorder of the knee extensor mechanism. Persistent pain, weakness, and loss of terminal extension can lead to poor clinical outcomes and significant functional impairment. In the acute setting, approach to management often includes primary repair of the injured tendon that can lead to excellent clinical outcomes. However, a delayed or missed diagnosis can bring forth a clinical scenario that can be difficult to manage thereafter. In the chronically injured tendon, challenges can be linked to limited t

Return to Play After Thigh Muscle Injury: Utility of Serial Ultrasound in Guiding Clinical Progression.

Return to play after thigh muscle strain injury is difficult to predict and recurrent injury rates are high. There is currently no agreement on the optimal return to play protocol following muscle injury and utility of imaging is debated. Ultrasound has become an increasingly utilized imaging modality and offers several potential advantages for monitoring the return to play process following muscle injury. An ultrasound scanning protocol is presented which provides real-time guidance on muscle healing and may assist with identification of those athletes at risk of reinjury who otherwise appear

Clinical concepts for treatment of the elbow in the adolescent overhead athlete.

Injuries to the adolescent elbow are common because of the repetitive overuse inherent in many overhead sport activities. The management of these patients is greatly facilitated through a greater understanding of the demands placed on the upper extremity kinetic chain during these overhead activities as well as a detailed examination and rehabilitation for the entire upper extremity kinetic chain. Particular emphasis on improving rotator cuff strength and muscular endurance, along with scapular stabilization, is a critical part of elbow rehabilitation in these patients. In addition, the use of

Time to return to full training and recurrence of rectus femoris injuries in elite track and field athletes 2010-2019; a 9-year study using the British Athletics Muscle Injury Classification.

Rectus femoris (RF) injuries are common in sports requiring maximal acceleration and sprinting. The British Athletics Muscle Injury Classification (BAMIC) describes acute muscle injury based on the anatomical site of injury and has been associated with return to play in hamstring and calf muscle injury. The aim of this study was to describe and compare the time to return to full training (TRFT) and injury recurrence for BAMIC-classified RF injuries sustained by elite track and field (T&F) athletes over a 9-year period. All rectus femoris injuries sustained by elite T&F athletes on the British

Dual-Task Gait Stability after Concussion and Subsequent Injury: An Exploratory Investigation.

Persistent gait alterations can occur after concussion and may underlie future musculoskeletal injury risk. We compared dual-task gait stability measures among adolescents who did/did not sustain a subsequent injury post-concussion, and uninjured controls. Forty-seven athletes completed a dual-task gait evaluation. One year later, they reported sport-related injuries and sport participation volumes. There were three groups: concussion participants who sustained a sport-related injury (n = 8; age =15.4 ± 3.5 years; 63% female), concussion participants who did not sustain a sport-related injury

A Narrative Review of Anterior Cruciate Ligament Reconstruction Graft Choice and Its Influence on Return to Sport.

PURPOSE OF REVIEW: Anterior cruciate ligament (ACL) rupture is among the most common knee injuries in active individuals and can lead to persistent instability, secondary intra-articular damage, and an elevated risk of osteoarthritis. Because return to sport (RTS) is a major goal for most active patients and graft selection remains one of the most debated aspects of ACL reconstruction, this narrative review summarizes the contemporary evidence on how graft choice influences RTS, focusing on the four most common grafts: bone-patellar tendon-bone (BTB), hamstring tendon (HT), quadriceps tendon (

Clinical outcomes after surgical treatment of open thigh muscular tears in skeletally immature patients.

Open thigh muscle lacerations in skeletally immature patients are rare, and evidence on optimal management and outcomes is limited. Among sports, skiing represents a frequent mechanism of injury due to sharp ski edges, yet data on pediatric cases remain scarce. We aimed to describe injury patterns, surgical management, and recovery of pediatric patients with open quadriceps tears. In a retrospective two-center cohort, we included skeletally immature patients with open thigh muscular lacerations treated surgically. Mechanism of injury, contamination status, muscle involvement, and complications

The Current State of Anterior Cruciate Ligament Reconstruction: Graft Selection, Lateral Augmentation, Rehabilitation, and Return-to-Play.

PURPOSE OF REVIEW: Anterior cruciate ligament reconstruction (ACLR) reliably restores anteroposterior stability; however, the rate of return to prior level of play and incidence of recurrent ACL injury have improved little over the past two decades. The purpose of this review is to describe the current state of ACLR with respect to graft selection, lateral extra-articular augmentation, rehabilitation and return to play, including the role of psychological readiness, and the specific rehabilitation protocol utilized by the senior author. RECENT FINDINGS: Graft utilization has shifted substanti

Hamstring graft remodelling after ACL reconstruction is slower in paediatric population compared to adults.

PURPOSE: Anterior cruciate ligament (ACL) injuries are increasing in children and adolescents and reconstruction (ACLR) is now frequently performed in this population. Paediatric patients remain at higher risk of reinjury, possibly due to delayed graft remodelling. The current study aims to compare magnetic resonance imaging (MRI)-based graft remodelling between paediatric and adult populations at 1 year postoperatively. METHODS: We conducted a single-centre comparative pilot study including a prospectively collected cohort of 52 paediatric and 59 adult patients undergoing ACLR with a semiten

Psycho-social-contextual landscape of return to sport after hamstring strain injuries: a scoping review.

OBJECTIVES: Hamstring strain injuries (HSI) are a leading cause of time loss and reinjury in elite sport, yet return-to-sport (RTS) decisions remain largely based on physical criteria. This scoping review aimed to map the psychological, social and contextual factors associated with RTS after HSI and to examine how psychological readiness has been conceptualised in RTS after HSI. METHODS: Following Joanna Briggs Institute methodology, seven databases (MEDLINE, Embase, PsycINFO, CINAHL, SPORTDiscus, Scopus and ProQuest) were systematically searched for studies published until September 2025 in

Running and Jumping After Muscle Fatigue in Subjects with a History of Knee Injury: What Are the Acute Effects of Wearing a Knee Brace on Biomechanics?

The knee is one of the most frequently injured joints, involving various structures. To prevent reinjury after rehabilitation, braces are commonly used. However, most studies on knee supports focus on subjects with anterior cruciate ligament (ACL) injuries and do not account for muscle fatigue, which typically occurs during prolonged intense training and can significantly increase the risk of injury. Hence, this study investigates the acute effects of wearing a knee brace on biomechanics in subjects with a history of various unilateral knee injuries or pain under muscle fatigue. In total, 50 s

A reconditioning program to lower the recurrence rate of brachial plexus neurapraxia in collegiate football players.

ObjectiveTo present clinical techniques for managing class I brachial plexus neurapraxia (BPN) in collegiate football players.BackgroundDuring a football career, up to 50% of college football players develop BPN. It is difficult to return the player to practice and competition without many recurrences during the same season. Postseason reconditioning programs may improve the outcome by allowing the injured nerves a chance to heal adequately. Flexibility and strength of the neck and shoulder girdle are the focus of this proposed postseason BPN reconditioning program. This clinical technique is
